fix(server): keep remote webhook signing references local - #22540
Webhook signing-secret references are local to one server and work once. Forwarding one with schedule_task or update_scheduled_task cannot configure the destination's secret. Reject that request locally and direct the caller to configure the signed webhook from a destination thread; ordinary remote schedules still forward.
This ports @juliusmarminge's 8845874 onto retained #16684 at base 655a0548a. Only the guard, focused regression and two scheduling-tool descriptions change; current forwarding behavior and other guidance are preserved.
Scope and approval
Submitted under the focused obvious-bug exception: this rejects an unusable local handle at the existing remote-call boundary. No new secret-transfer mechanism or prior maintainer approval is claimed.
Verification
With the guard reverted, the new two-server test fails because the peer is contacted. Guarding only create or only update also fails; refusing every schedule fails the ordinary-routing assertion. An independent run of PeerForwarding.test.ts and orchestrator/tools.test.ts passed all 11 tests. Server typecheck and scoped lint/format pass. Ordinary scheduling is verified to reach the peer; the harness scheduler is mocked, so this does not prove real task creation.
No live two-machine or provider run. Existing plaintext signing-secret behavior is unchanged. This PR does not include the separate metadata-validation or Stop-result fixes.

A rename request without title now succeeds while leaving the thread title unchanged and consuming its clientRequestId. Rebuilding the schema with Schema.Struct({...ThreadMetadataMcpUpdateInput.fields}) drops the original struct-level check, so { action: "rename" } reaches the service without a title; preserve that validation when adding environmentId.

A stop: true call reports runId: null and status: "no_active_run" even when it successfully requests an interrupt; when queued messages produce a run.updated event, it can instead report a queued run as the interrupted run. The live-provider-session interrupt path does not emit run.updated for the active run, so this lookup cannot identify the stop target. Read the run ID from the interrupt-request event or return the stop target explicitly.

Macroscope's review found this PR requires human approval — This change adds a substantial cross-environment linking and remote MCP execution capability, including OAuth credentials, persistent peer state, authentication changes, and broad production routing behavior. It also contains unresolved high-severity correctness findings affecting thread metadata validation and stop-result reporting.

An OAuth client on /mcp is capped by the access it was approved with.
When another environment calls on behalf of one of its agents, that agent
may run under narrower modes, and the target has no way to know.
/mcp now reads `T3-Mode-Limit: <runtimeMode>/<interactionMode>` from OAuth
clients and caps the request at the narrower of the two. The header can
only narrow: a broader one changes nothing, and a malformed one is
refused with 400. The client's interaction mode can now be capped too;
before, it was always default.

A launch had no retry key, so an agent (or another environment calling on
its behalf) that lost the response could only guess whether the thread
was created, and a retry started a second one.
t3_thread_launch now takes an optional clientRequestId. The command,
thread and message ids derive from the caller and the key, and the launch
service already replays a command it accepted, so a retry returns the
first thread. Without a key every launch is new, as before.

An environment can now sign in to another one's /mcp as an outside agent,
with a pairing code from that environment, without a browser. It drives the
peer's existing MCP OAuth flow server-side, keeps the token in the server
secret store and the rest in peer_environment_links (migration 059). The
peer lists the link as "T3 Code · <label>" in Connections, where it can be
revoked.
The token is only ever sent to an address whose descriptor answers as the
linked environment. PeerMcpClient calls the peer's tools and carries the
caller's limits in T3-Mode-Limit on every request. Linking refuses this
environment itself and peers that cannot honour that header.
`t3 environment link|list|unlink` and the peerLinks.* RPCs manage links.
Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>
An agent can now work on a linked environment's threads through the same T3
MCP tools. t3_project_list, orchestrator_capabilities, t3_thread_launch,
t3_thread_list, t3_thread_read, t3_thread_send, t3_thread_wait and
t3_thread_interrupt take an optional environmentId; t3_environment_links
lists the links.
Each tool's own access declaration still runs here first, so a read-only or
ended caller is refused before anything leaves. The forwarded call carries
the caller's modes in T3-Mode-Limit, and the linked environment holds it to
those and the link's access. Retry keys are hashed with the caller's
namespace, since every caller here shares the link's one session there. A
forwarded wait goes out in pieces of at most 50 s so no route drops it idle.
A remote launch cannot carry attachments yet.

t3_project_create and t3_project_clone take environmentId, so an agent can
bring a repository to a machine before launching or delegating work there.
Both still need full access here, and the linked environment holds the call
to the link's access.

…s a linked environment
Forwarding was wired tool by tool, so only the first eleven tools took
environmentId. It now lives in McpToolAccess.toLayer: a tool whose
parameters take OrchestratorMcpEnvironmentTarget runs in the linked
environment it names once its declaration has checked the caller here.
The local handler never sees environmentId. Tools that act as the calling
thread itself cannot take one, and the layer refuses to build if they do.
Queues, pending requests, thread settings and organizing, forks and
merge-backs, scheduled tasks, pull requests, worktree refs, project
read/update/delete and environment preferences now reach a link too.

A linked environment that works can be shown with a misleading "last error" when one of its tools refuses a request, for example for an unknown thread. Forwarding itself is unaffected. This is a small status-reporting fix that can be made before or shortly after merge.

Full details: Title check
Explanation
The title "fix(server): keep remote webhook signing references local" matches the main change. The PR adds a guard that rejects remote scheduled-task requests carrying a local webhook signing-secret reference, so the remote peer never receives an unusable handle. The title uses the conventional commit form and names the affected area (server) and the specific behavior. The changeset also includes large peer-link and linked-environment work, but the PR objectives describe this PR as the narrow guard fix, so the title is accurate for this PR.
Full details: Description check
Explanation
The description covers the problem (a local signing-secret reference forwarded to a remote server cannot configure the destination secret), the change (reject locally and direct the caller to configure from a destination thread, with ordinary forwarding preserved), scope and approval (cites the focused obvious-bug exception and disclaims prior approval), and verification (the regression test, mutation checks, the 11 passing tests, typecheck and lint, and the limits of testing). It follows the template sections. The Problem section is not a separate heading, and the description does not name a reproduction environment in detail, but these gaps are minor. The description also says the harness scheduler is mocked, which is stated clearly.

Add named spans to the list, get and recordOutcome service operations.
list, get and recordOutcome are public service operations. Each one does SQL I/O, and list also probes every linked peer over HTTP. None of them has a named span. The other operations in this file use Effect.fn("PeerLinks.…"): link, unlink, resolve and toLink. A slow list is the most likely latency source in Settings and in t3_environment_links. Without a span, traces cannot show it.
Wrap get and recordOutcome in Effect.fn("PeerLinks.get") and Effect.fn("PeerLinks.recordOutcome") in the same way.
As per coding guidelines: "Default to named spans for service operations and I/O."

Reuse one mode-rank table instead of a third copy.
This change adds runtimeModeRank and interactionModeRank here. The same orderings already exist in OrchestratorMcpService.ts (lines 589-604) and DispatchModeLimit.ts. These tables decide what a caller can do, so a mode added to one copy but not the others would make the ceiling, the escalation checks and the dispatch-time recheck disagree.
Export one set of rank helpers from a shared module. Have clientModeCeiling, resolveRuntimeMode, resolveInteractionMode, withinLimits and exceededDispatchModeLimit use it.

Record only link and transport failures as the link's lastError.
Effect.tapError calls recordOutcome with error.message for every failure. This includes the case where the peer answered and its tool refused, for example thread_not_found from the isError branch at Line 209-218. That refusal is then persisted as the link's last_error. t3 environment list and Settings then show "last error: No such thread." for a link that works. The recordOutcome contract says it records "whether the peer answered last time", and a tool refusal is an answer. Record success when the peer returns HTTP 200 with a decodable body. Record an error only for failures to resolve, connect, authenticate or decode.
🐛 Proposed direction
- if (body.result.isError === true) {+ // The peer answered: the link works even if the tool refuses.+ yield* links.recordOutcome(input.environmentId, { error: null });+ if (body.result.isError === true) {
- Effect.tapError((error) =>- links.recordOutcome(input.environmentId, { error: error.message }),- ),- Effect.tap(() => links.recordOutcome(input.environmentId, { error: null })),+ // Tool refusals are recorded as success above; only link/transport failures reach here unrecorded.+ Effect.tapError((error) =>+ error._tag === "OrchestratorMcpFailure" && peerAnswered(error)+ ? Effect.void+ : links.recordOutcome(input.environmentId, { error: error.message }),+ ),
peerAnswered is one possible mechanism. Another is a local flag set after HTTP 200. Either one separates peer refusals from link failures.
