🟠 High orchestration-v2/ProviderSessionManager.ts:1434
A fresh matching ProviderAdapterThreadUnloadRefusedError still cannot reclaim an idle release marked unconfirmed, so prepareThreadAttachment rethrows the refusal and blocks reattachment. This branch only accepts the cached "refused" state; accept matching fresh refusal evidence here too.

ApprovabilityVerdict: Needs Human Approval Macroscope's review found this PR requires human approval — This PR makes a substantial production change to native-thread ownership, detach/release retries, reattachment fencing, and background-work handling across several session lifecycle paths. An unresolved High-severity finding also identifies a refusal-handling case that can block reattachment. Needs human approval because:

Keep ProviderSessionBackgroundWorkError internal
The error is constructed only inside ProviderSessionManager.ts. The surrounding paths convert it to ProviderAdapterProtocolError or ProviderSessionReleaseError, and no external TypeScript file imports it. Since knip:check analyzes exports, remove the unused value export.

Check that the conflicting owner released the thread before opening the target
open calls manager.detach for each conflicting Codex owner. It then continues without checking the result. In several cases, detach returns success without releasing the native writer:
- On Line 2598,
detachreturns early whenexpectedTargetInstanceIdis set, the active binding is not the target, and a run is active or the model selection differs. - On Lines 2700-2711, the
Ref.modifyreturnsOption.none()when the loaded record changed, and Line 2757 then returns.
In each case, the owner session keeps the native thread loaded. open then starts the target session with the same initialNativeThreadId. This is the writer collision that the code comment on Lines 2321-2322 says must not happen.
After the loop, scan sessions again. If an owner still holds the native thread as loaded or pending, fail with ProviderSessionOpenError.

Avoid marking a thread pending when unloadThread is unavailable.
If a runtime supports multiple provider threads but omits the optional unloadThread callback, detach still stores a pendingThreadReleases entry. Cleanup does not run, and confirmation returns without clearing the entry. Reattachment then sees the loaded record as pending, so resumeThread calls the underlying runtime instead of using the loaded fast path.

A detached or rebound projection can lose track of its loaded native writer. Retain that binding until confirmed release, preserve retryable cleanup, and fence stale work by runtime, owner, binding and generation. Before target open, reconcile other loaded owners even after prior detach retries exhausted. Reattach the unchanged writer only after typed evidence that no release began; ambiguous release remains blocked.
Rebuilt from @WISSAM492’s #19557, which identified native writer ownership and continuity requirements. The complete original diff was compared with this replacement. Stack: #21983 → #21984 → #21985, each targeting the preceding branch.
Combined-stack validation: 381/381 cases pass across six existing suites, zero skips. The final test-only union guard additionally passed all 182 adapter cases and server typecheck. Scoped lint/format and diff checks pass. Provider-core typecheck passed before the final adapter changes; that package is unchanged. Original 133 net-added cases became 33 retained behavior/control cases with four baseline rewrites: 29 net additions over the 352-case baseline. Focused regressions failed before their fixes. No repo-wide checks.
Fresh production-source proof used actual pinned Codex 0.156.1 with the managed runtime, native app servers, adapter, manager, orchestrator, outbox, starter and ingestor. Two synthetic accounts and loopback inference exercised A→B→A with the same parent/child and preserved history, child ordinals [1,2], an actual tool effect executed once, and a busy peer completing two turns in its original pooled process. Busy-child transfer first exhausted retries with zero target resume RPCs; a new message on the same selected account recovered after child completion. Matching child/parent closure preceded each resume. The final test guard changes no production source. A separate active-writer-denial probe passed in R3; it was not rerun in R4.
Four fresh full-stack parallel Opus/Sol review rounds completed. The final round found no confirmed code blockers. Earlier findings were fixed: busy-child retry recovery, uncertain history delivery, legacy lock-directory upgrades, typed refusal reattachment, and known/interrupted root classification.
Limits: injected auth/discovery/installer seams; no real OAuth/public inference, Windows/macOS, Electron SEA, browser/mobile, remote relay/tunnel or installer verification. Queued selection and uncertain history delivery have service coverage, not native UI/crash proof. Interrupted-root cancellation is proved by adapter protocol replay, not UI Stop or live native Stop. Stop old native writers using private lock directories before upgrade: legacy directory replacement is verified only for inactive homes, with no global exclusion or hot migration proof. Strict ordinary-detach failures retain pending cleanup for later retry/reattach; they can produce bounded outbox warnings while work remains.
Landing, human/platform approval and the final CI sweep belong to the coordinator. No merge or watcher is requested.
Implementation: Codex gpt-6.1-sol; independent tests: Codex gpt-6-astra; reviews: Claude claude-opus-5-5 and Codex gpt-6-sol. Harness: T3 Code.
