Problem

Every Codex thread shares one codex app-server. When that process dies, T3 never notices:

  • Every run on it stays running forever. Delegated Codex subagents keep a growing Lineage timer, and their parents wait on them indefinitely.
  • The dead session stays live, so every later Codex turn fails with Codex App Server transport operation 'read-process-exit-status' failed until the T3 server restarts.

Seen in production on 0.1.1-nightly.20261017. Five Codex subagent threads, two of them grandchildren, all froze at the same instant (updatedAt 11:37:45.165Z). No Codex process was left, and the T3 server had been up for two days. Lineage showed them running for 8.3 h until they were stopped by hand. This is the failure reported in #16502 and #17045.

Root cause

  1. effect-codex-app-server's client never passes the protocol's existing onTermination hook. A process exit is only visible to pending requests, and a running turn has none.
  2. CodexAdapterV2's session event queue is never ended or failed. So ProviderSessionManager's existing handling for a failed provider stream never fires, and neither does RunExecutionService's. That handling fails the runs, releases the session, and lets the next turn spawn a new app-server.

Fix

  • The Codex client exposes awaitTermination. The adapter fails its event stream when the app-server terminates, and the existing release path does the rest.

It also closes three gaps that the open PRs for this bug (#21013, #18764, #18139, #12777) don't cover:

  • Orphaned native subagents. If a Codex turn completes while its native subagents keep running, and the app-server then dies, RunExecutionService skipped the failure because the root run was already finalized. Those subagents stayed running forever. They are now failed.
  • Delegated task stuck waiting on them. That cascade writes no run.updated, so a delegated child waiting only on those subagents never settled its parent task, and the parent was never woken. The cascade write is tagged, and the orchestrator re-finalizes the delegated task in the same consumer that handles run updates.
  • Stale status on a reopened session. Once a dead session is released, the next turn reopens the same shared session id, often while the old release is still closing its process. The old release then overwrote the new session's ready with stopped or error. Its replacement check and record write now run under sessionOpen, as the retry path's already did. open()'s own failed-start cleanup, which already holds that lock, skips reacquiring it.

Evidence

Both builds ran on an isolated T3 home against a fake codex that streams a reply and then SIGKILLs itself mid-turn.

Before (0.1.1-nightly.20261017) After (this branch)
Thread keeps working after the app-server died Turn fails, retry runs on a new app-server
The thread keeps "Working" with no process left. The turn fails at once, and the retry completes on a fresh app-server.
Next Codex turn reuses the dead app-server
The next Codex turn fails against the dead app-server.
Delegated child stays running in Lineage Delegated child fails and the parent resumes
The delegated child stays "Running" in Lineage, and the parent waits forever. The child shows "Failed" within about 2 s, and the parent is woken and finishes.

Tests

Each new test fails without its fix:

  • client.test.ts: the mock app-server exits while the client is idle, and awaitTermination reports ProcessExited with code 7.
  • OrchestratorReplayRecovery.integration.test.ts: real orchestrator, Codex adapter and session manager against a replayed app-server that exits mid-turn. Run 1 is failed, not running, and the follow-up turn completes on a new app-server.
  • RunExecutionService.test.ts: a completed root's native subagent is failed when the stream then dies.
  • DelegatedTaskProviderExit.integration.test.ts: a delegated child's provider dies after its root turn completed, and the parent's task record settles.
  • ProviderSessionManager.test.ts: a session reopened while the old release is stuck closing keeps ready.

vp test run passes on the affected suites: 474 tests in 9 server files, plus 37 in effect-codex-app-server. Scoped typecheck, lint and format pass.

Not covered here: an app-server that hangs without exiting, which is #18764's liveness probe.

…p-server exits

Every Codex thread shares one app-server. When it died, the client never
reported the termination and the adapter's event stream never ended, so
runs stayed running forever and later turns reused the dead session.

- The Codex client exposes awaitTermination; the adapter fails its event
stream on it, so the session manager fails the runs and releases the
  session.
- A finalized run's still-open native subagents are failed when the
provider stream dies, and the orchestrator re-finalizes the delegated
  task waiting on them.
- A release no longer overwrites the status of a shared session reopened
while the old one was closing.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This change alters shared provider-session lifecycle and cross-thread run/delegated-task settlement across several production orchestration layers. The concurrency and durable event-state coordination make the runtime impact broader than a small isolated bug fix.

Review in Change Stack →

📝 Walkthrough

The change propagates Codex app-server termination into the orchestration stream, marks open provider-native subagents and turn items failed after a finalized run's stream fails, and finalizes associated delegated tasks. It also protects a reopened provider session's projected status from an earlier release write.

Changes: Provider Exit Recovery

Layer / File(s) Summary
Propagate app-server termination
packages/effect-codex-app-server/src/client.ts, packages/effect-codex-app-server/src/client.test.ts, packages/effect-codex-app-server/test/fixtures/codex-app-server-mock-peer.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts
The client exposes awaitTermination, and the Codex adapter watches for termination and fails its event queue with the session ID and cause. Tests cover termination after initialization.
Cascade failures after run finalization
apps/server/src/orchestration-v2/RunExecutionService.ts, apps/server/src/orchestration-v2/RunExecutionService.test.ts, apps/server/src/orchestration-v2/testkit/OrchestratorReplayRecovery.integration.test.ts
When provider-event ingestion fails after root finalization, the run service writes failed cascade events for open subagents, nodes, and turn items. Tests cover the cascade and replay recovery.
Finalize delegated tasks
apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/mcp/DelegatedTaskProviderExit.integration.test.ts
The orchestrator handles cascade subagent updates and finalizes the associated delegated task under the parent thread's dispatch lock. The integration test checks the resulting child, subagent, delegated-task, and parent-task states.

Provider Session Release

Layer / File(s) Summary
Preserve replacement session status
apps/server/src/orchestration-v2/ProviderSessionManager.ts, apps/server/src/orchestration-v2/ProviderSessionManager.test.ts
Release records check for a live replacement session while holding the session-open lock. Open-failure cleanup signals when it already holds the lock. A regression test checks that the reopened session remains ready.

Preserve release status for threads outside the replacement.

If the old session served threads A and B, and the replacement opens only for A, replaced suppresses the old release event for both threads. B has no replacement attachment event, so B’s projection remains ready after its session closes. Suppress the old status update only for threads attached to the replacement; write a release event for the others.

Prevent an older release from overwriting a completed replacement release.

If the old scope close stalls, a replacement can open and release before the old close times out. The map is then empty, so this check writes the old release status after the newer release status. For example, an old manual_shutdown can replace the newer session’s runtime_error with stopped. Fence status writes by session generation or the projected session’s identity, not only by whether a session is currently live.

Tested on Windows against a real Codex app-server: main reproduces both hangs, and this PR fixes both.

Setup: Windows 11, Node 24.19.0, Codex CLI 0.163.0 (npm global). Server run from source (node src/bin.ts serve with a scratch --base-dir), first at main 99c692b, then at main with this PR's head 3a2f884 merged (clean merge). I killed the shared codex app-server with Stop-Process -Force.

main (99c692b) main + this PR
Kill while idle, then send a turn Session still shows ready. Every later turn fails at once with CodexAppServerProcessExitedError: ... exited with code 4294967295, on the same thread and on a brand-new thread. No new app-server starts until the backend restarts. Session goes to error about 100 ms after the kill (Codex app-server exited; failing its provider session.). The next turn starts a fresh app-server and completes; later turns and a new thread work too.
Kill mid-turn, during a Start-Sleep -Seconds 120 tool call Run stays running for as long as I watched (2.5+ minutes), and the session still shows ready. Root turn is failed 190 ms after the kill. The next turn on the same thread starts a fresh app-server and completes.

The PR's tests also pass on Windows: the four server test files it touches (127 tests) and effect-codex-app-server's client.test.ts (3 tests).

One small leftover: in the mid-turn case, the failed run's in-flight tool_call node stays running with completedAt: null after the root turn fails. It doesn't block anything, but a client that renders node status could show that tool as still running.

This is the dead cached session I described in #17045 (comment).
