Claude compaction can report a structured failure and then finish /compact with a success-shaped local-command result. T3 discarded the structured outcome, marked the run completed, and released queued messages. It also omitted the running compaction item and labeled failed/stopped items as successful, making long compactions look hung.

Honor compact_result / compact_error, track the compaction item from start through its terminal outcome, and fail an explicit failed or unconfirmed /compact run so the existing queue hold applies. Automatic compaction failure updates the item without failing unrelated ordinary work. Keep the original start time across status/boundary events, ignore permission-only status:null frames, and terminate stale items on interrupt, crash, or hook skip. The main web timeline now shows elapsed time and a warning after 120 seconds; shared presentation supplies accurate failure/stopped labels and success-only token counts to the normal web/mobile timeline paths. There is no new scheduler or queue mechanism and no automatic timeout cancellation.

Related: #10959, whose maintainer triage established the missing Claude auto-compaction status. This uses the current V2 item lifecycle; that issue's earlier overlay proposal predates this implementation. The structured-failure/queue-release case is additional evidence, not a claim that the entire diff was pre-approved.

Evidence and validation

  • Local transcript investigation: four failed Claude compactions lasted approximately 1005–1011 seconds, each with 34 heartbeat frames; three were explicit /compact commands incorrectly completed by T3. Successful Claude samples took 21.6–102.7 seconds (median 48.1). Seven inspected Codex compactions completed without an intervening user message. These observations do not establish that sending a message itself triggers compaction.
  • Adapter regressions cover structured failure, unconfirmed completion, permissions-only status, start/boundary reconciliation, hook skips, interruption, and automatic compaction. 43 targeted adapter tests passed in implementation verification.
  • Five compaction replay fixtures passed, including two synthetic real-adapter → orchestrator scenarios: failed/unconfirmed compact holds queued text, then retry releases it exactly once. Mutating to the old behavior fails the regression.
  • Shared presentation: 110 tests passed; mobile thread activity: 100 passed. Scoped server, web, mobile, and client-runtime typechecks passed during implementation.
  • Final independent Opus review at 0c2d7ec870d41be64243b925c8f2b9b34f4e06ad: APPROVE; independently ran all 68 MessagesTimeline.browser.test.tsx cases, 183 tests across MessagesTimeline.logic, AgentElapsed, and V2LifecycleRow.logic, and the web typecheck.
  • The main-timeline timer regression fails on the preceding implementation (no timer) and passes here. Reproduce from apps/web: npx vp test run --project browser src/components/chat/MessagesTimeline.browser.test.tsx -t "ticks a running".

Visual evidence

These are synthetic UI fixtures, not a live provider replay. The before screenshot uses the base renderer at 1451e7cb23; after uses the reviewed renderer with the same fixture. The retained running item in the static app fixture belongs to a settled run, so its timer is intentionally absent. The separate browser-component recording renders an active run through the actual MessagesTimeline path, starts its clock at 118 seconds, and records the real two-minute transition. Recording-only viewport/dwell settings were restored; no evidence files are committed.

Live provider verification (2026-10-19)

Tested exact head 0c2d7ec870d41be64243b925c8f2b9b34f4e06ad through real T3 server processes, authenticated HTTP/typed WebSocket RPC, and Claude Code 2.1.303 / claude-sonnet-4-6 using subscription auth. Each scenario used an isolated T3 home; no projections were seeded. An independent Opus reviewer inspected native frames, provider logs, milestones, projections, proxy logs and the harness and returned APPROVE.

Scenario Observed result
Manual compact, no further input Completed in ~29s, 23,535 → 4,579 tokens. Exactly two user messages: seed and /compact.
Follow-up queued during compact Follow-up stayed queued until compact completed, then answered once. Repeated compacting frames produced one item. No duplicate run over the following 15s.
Failure and retry A child-process-only proxy injected one HTTP 400 into the summarisation request. The real CLI emitted compact_result: failed, followed by local-command is_error: false. T3 failed the compact run and held the queued follow-up for the 20s observation. Successful retry compacted 64,679 → 3,473 tokens, then the follow-up ran once.
Automatic compact With a child-only lowered threshold, a too_few_groups failure marked the item failed while ordinary work completed. A later automatic compaction completed, 24,685 → 4,460 tokens.

Sanitised result artifacts: manual/queue, failure/retry, automatic, runtime and limits.

Probe corrections: T3 rejects empty-thread /compact before starting the provider; its follow-up was sent after rejection, so this probe does not prove queue behavior during failure. An initial automatic probe never crossed the threshold. Another initially required every compaction item to succeed; the final assertion correctly allows failed automatic items while still requiring ordinary runs to complete AND at least one successful automatic compaction. Earlier harness hashes were not retained; the final artifact hash is retrospective. These unsuccessful probes are retained in local evidence, not counted as passing scenarios.

Limits

Known minor fallback-renderer inconsistency: V2LifecycleRow and v2EventPresentation still build raw before/after token details for non-completed items. The normal compaction timeline uses the shared success-only helper; these fallback branches are not fixed by this PR. CodeRabbit identified the same nonblocking detail limitation; it is deferred after exact-head approval and live verification.

Live provider scenarios above passed once each; flakiness is unmeasured. No native-mobile visual test, live-provider UI recording, or deployment was performed. Automatic compaction used a lowered threshold, and the injected fast HTTP 400 does not reproduce the original 16-minute network stall. The connection-refusal endpoint underlying the observed provider failures remains unidentified; this fixes outcome handling and visibility, not the network failure. Screen readers receive the correct lifecycle label but the existing separator semantics do not expose the elapsed clock or warning (nonblocking review finding). A pre-existing 120-second foreground-subagent steering test timeout was reproduced on base; broad replay runs exited 137. Video-enabled test teardown reported a browser-close timeout after the focused test passed; the normal final review runs passed independently.

…ehind a failed /compact

- Track each compaction as its own operation. A turn that ends, a status
cleared without an outcome, an interrupt or a CLI exit now settles the
running item as interrupted, failed or unconfirmed, never as successful.
- An outcome and a boundary arriving in either order update one item.
- An interrupted /compact stays interrupted. An unconfirmed /compact fails
  with code compaction_unconfirmed.
- Claude's structured failed outcome for "nothing to compact" fails the run
and holds the queue. The CLI has no structured no-op outcome to tell the
  two apart.
- Replay fixtures run failed and unconfirmed /compact through the
orchestrator scheduler. A held queue counts as idle in the scenario runner.
- The elapsed warning writes its text only when the text changes.
…d label failed compactions honestly

A permission mode change sends a status frame with a null status, which
ended an active compaction as hook-skipped and split one compaction into
a stopped item plus a completed one. Only a status frame without a
permission mode now ends a compaction without an outcome.

The shared compaction label and token counts claimed success for every
non-running status. Only a completed compaction now reports compacted
context; failed and stopped compactions say so on web and mobile.
…ne row

The standard timeline renders compaction work entries through
ContextCompactionTimelineRow; the timer added to V2LifecycleRow never ran
because compaction items are not standalone event rows. Carry the item's
startedAt on the row, render the elapsed clock (120s warning) outside the
shimmer overlay while active, and let long labels wrap.

📝 Walkthrough

Claude compaction operations now track status, outcomes, timing, errors, and token counts. Failed or unconfirmed /compact runs fail explicitly. Client presentation distinguishes compaction states, shows completed token counts, and displays elapsed time for active timeline rows.

Merge Risk: 🔵 Low · up to 64aff

Compaction rows in fallback renderers can show misleading token counts for failed or running compactions. The Claude adapter may also pass an unvalidated pre-compaction token count into the turn item. Both are minor and can be fixed in a follow-up.

Security architecture risk: 🔵 Low · up to 0c2d7

The change improves failure containment by holding queued work after failed or unconfirmed manual compaction. No new permissions or remotely callable operations were identified. Remaining uncertainty concerns unusual provider-frame ordering and recovery after a full server crash.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The demonstrated control effect is bounded to the active provider conversation and its thread’s queued work. The changed compaction path does not introduce a tenant selector, tool invocation or credential authority.

Trust Boundaries and Controls

  • observed — Provider outcomes are interpreted server-side. Failed or unconfirmed manual compaction becomes an existing classified failure, and production queue guards—not display labels—control subsequent execution.

Resilience and Maintainability Implications

  • observed — Provider messages are consumed serially under a shared lock, including buffered continuation replay. The 120-second warning only updates rendered text; it neither cancels compaction nor releases held work.

Hardening Proposals

  • proposed — Establish the provider’s duplicate and delayed-frame guarantees. If such delivery is supported, add frame-level deduplication and operation-association checks so an earlier success cannot settle a later operation. Distinct lifecycle frames already use different UUIDs, so UUID equality alone is not an operation-correlation rule.

Agent-written shipping handoff for 0c2d7ec870d41be64243b925c8f2b9b34f4e06ad: the PR is ready for review. Independent Opus code review and a separate audit of live Claude Code 2.1.303 scenario evidence both approved it. Manual compaction without further input, queue release after success, injected provider failure with queue hold/retry, and automatic compaction passed; evidence and limitations are in the PR body. CodeRabbit reports low merge risk and agrees the documented minor fallback-renderer cleanup is nonblocking.

The CI run and mobile workflows are still action_required awaiting maintainer approval, rather than failed test executions. This fork account cannot approve those runs or merge upstream. Please approve the workflows when appropriate and consider merging after CI and your review pass. No CI bypass or unverified shipping claim is intended.

🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Validate pre_tokens the same way as post_tokens.

afterTokenCount is used only when post_tokens is a finite positive number, and it is rounded. beforeTokenCount copies message.compact_metadata.pre_tokens with no check. The compaction turn item declares beforeTokenCount as NonNegativeInt. A non-integer, negative, or missing pre_tokens from the CLI therefore produces a turn_item.updated payload that breaks the contract. The completed label can also show a bad count. Normalize both values with one rule.

Proposed fix
-            operation.beforeTokenCount = message.compact_metadata.pre_tokens;
+            const preTokens = message.compact_metadata.pre_tokens;
+ operation.beforeTokenCount =
+              typeof preTokens === "number" && Number.isFinite(preTokens) && preTokens >= 0
+ ? Math.round(preTokens)

closing at @StiensWout's request under Establish the problem and scope first.

The cited issue approves automatic-compaction visibility while explicitly keeping lifecycle and settling unchanged. This PR also changes explicit /compact terminal outcomes and queue release, without maintainer triage establishing those additional failures and intended lifecycle behavior.

Obtain maintainer triage for the explicit-compaction failure/queue lifecycle and intended behavior, or submit the already approved visibility scope separately.

thanks for trying to help improve t3 code. if you think this closure was a mistake, please open a fresh pr addressing the requirement above, and link back to this one.
