Treat a failed deduplication read as a cache miss.

isNodeStored runs before the existing event write. A ProjectionStoreReadError becomes ProviderEventNormalizeError. This can fail the run and release the provider session instead of allowing the node event to use the previous write path. Catch only the read error and continue with false; the existing node upsert safely handles the resulting duplicate write.

Review comment at @apps/server/src/orchestration-v2/ProviderEventIngestor.ts at
line 616:
Update the node deduplication check using projections.isNodeStored to treat
ProjectionStoreReadError as a cache miss and continue through the existing node
upsert path; preserve the early return when the node is stored and propagate
other errors.
