2026-10-23 verification at 5f82b5b454. Main was integrated without changing the bounded status-cache, eviction reload, turn-end refresh or subscriber retry behavior. Focused broadcaster tests and scoped lint/format pass. Older heap/performance measurements and hosted verdicts below describe their linked commits; they were not measured again.

Earlier validation counts, review verdicts, CI status and captures below refer to their original revisions; older “current head” wording refers to those revisions. These focused local checks do not certify the whole suite.

Problem

VcsStatusBroadcaster caches local and remote VCS status per cwd and never removes an entry. A long-running environment that visits many thread worktrees keeps every snapshot, including its changed-file list, for the life of the server, and every status update copies the whole map. Stream cleanup releases remote pollers but never the cache.

Change

  • The cache keeps the 256 most recently used cwds. Reads and updates mark an entry as recent; the oldest is evicted. All three cache-update paths share one helper.
  • After a turn, a cwd with an active remote poller still gets its PR refresh even if its snapshot was evicted (background policy still applies).

Scope and approval

A small, focused memory fix, so it uses the no-prior-issue exception. One service.

Earlier revision verification

The local test, lint, typecheck, heap-probe, and independent-review results below were recorded before the latest merge from main; they were not rerun locally for e087cd7.

  • New VcsStatusBroadcaster.test.ts cases on the real service: eviction and reload, hot and recent hits, turn-end PR updates after eviction (fetching disabled, background gating), and local, remote and full events for foreground and passive subscribers. On main 4 fail (log); with the fix, 28 pass (log), rerun after an earlier rebase onto main.

  • Heap probe (node --expose-gc, 100 changed files per snapshot, one sample per revision):

Finished subscriptions Retained snapshots, main → PR Heap growth, main → PR
    1,000 1,000 → 256 20.4 MB → 6.3 MB
    3,000 3,000 → 256 59.0 MB → 6.2 MB

    (main 3,000, PR 3,000)

  • vp lint and apps/server typecheck: clean.

  • Independent review by Claude Opus 5.5 (high) through T3 delegated tasks: round 1 found that the turn-end PR refresh was skipped after eviction for watched cwds (fixed, with a regression test); round 2 found no defects.

  • Subscriptions requesting remote status reload missing remote snapshots after eviction, including when a zero-interval poller already exists. Subscriptions with includeRemote: false retain their existing behavior.

Earlier follow-up: subscribers after eviction (cb65ed2, 2e04b3e)

Once a snapshot is evicted, a new subscriber could receive remote: null and never get the remote status, because a poller running with a zero interval only refreshes once. A new subscriber now triggers one scoped initial refresh (onlyIfMissing, under the remote-write lock), which is skipped when a remote status is already cached. Transient failures retry with the existing capped backoff, and the retry ends with the subscriber's scope. A new test with TestClock fails on the previous head and passes now. VcsStatusBroadcaster.test.ts: 30 passed. Reviewed by Claude Opus 5.5 (high), two rounds. The first round's retry gap is fixed, and the second found no actionable findings.

Earlier-head verification

The current head e087cd7 merges main into the reviewed subscriber fix. The CI workflow and all individual non-skipped checks have completed successfully, including Test Server 3. Macroscope approved this exact head. CodeRabbit's latest review covers changes through this head and reports no actionable comments; its formal approval remains attached to the earlier subscriber-fix revision.

No source changes or new local test/heap runs were made for this approval follow-up. The earlier verification and measurements above retain their stated revision limits.

Keep up to 256 recently read or updated cwd snapshots across all cache write paths. Eviction leaves status publication and remote pollers intact, and watched worktrees still receive turn-end PR refreshes when automatic fetching is disabled.

Walkthrough

The VCS status broadcaster now limits its cache to 256 entries and updates entry recency on reads and writes. It handles remote and pull-request status refreshes after eviction. Pi transcript candidates now use the sessions directory as their home path.

Merge Risk: Minimal · previous review 2607c

The reported formatting blocker is corrected in the current source. No actionable merge blocker remains on the available evidence; run the normal CI checks before merging.

Security architecture risk: Low · up to 841ac

The memory bound is narrowly scoped, but recovery retries can retain an earlier automatic-fetch setting after fetching is disabled. Existing authorization, cancellation, and project-level pull controls limit the exposure.

Retained concerns

  • Medium · reliability · inferred: The new recovery path reads the automatic-fetch interval once before starting retries and captures refreshUpstream from that value. If an attempt fails and automatic fetching is subsequently disabled while the subscription remains open, later retries still issue fetch-enabled refresh requests. The existing shared loop instead rereads the setting each iteration. Those stale-enabled retries can also reach automatic-pull logic when its separate project and working-tree checks allow it.
  • Low · reliability · inferred: When remote status is missing, each additional subscriber to an existing directory poller can create its own retrying recovery fiber. Persistent failures therefore allow subscriber-scaled retry work rather than one recovery schedule per directory. The directory lock serializes operations but does not coalesce failures. Successful cache population and subscription cancellation contain this behavior; its production availability impact is unmeasured.
Security review details

Security Blast Radius

  • inferred — The identified incremental exposure is authenticated subscription-driven work against directories handled by one server and their existing Git dependencies. The evidence does not establish unauthenticated access, new privileges, or a new cross-service authority boundary.

Security Findings and Attack Paths

  • inferred — An authorized subscriber encountering missing remote status and repeated upstream failures can activate the new retry path. While that stream remains open, retries can continue with a previously enabled fetch setting and can multiply across subscribers. This supports conditional control-drift and availability concerns, not a verified privilege-escalation or data-exposure finding.

Trust Boundaries and Controls

  • observed — The existing VCS subscription route requires orchestration-read scope and exposes only cwd and optional includeRemote. Immediate recovery is internally selected, not a new client-controlled authority flag. Passive subscriptions do not retain remote pollers.

Resilience and Maintainability Implications

  • observed — Recovery uses exponential retry delays, with the configured interval as a minimum and the exponential component capped at fifteen minutes; it has no finite retry-count limit. Per-directory locking prevents simultaneous remote writes, and successful cache recovery short-circuits later recovery calls. These controls reduce duplicate work without providing shared retry ownership during persistent failure.

Reuse remoteRefreshFailureDelay and log retry failures.

The inline schedule repeats the backoff formula from remoteRefreshFailureDelay: base 30 seconds, doubling, capped at VCS_STATUS_REFRESH_FAILURE_MAX_DELAY, and never shorter than the configured interval. If one copy changes, the two backoff policies will no longer match.

Effect.ignore also hides every failed attempt. makeRemoteRefreshLoop logs "VCS remote status refresh failed" with remoteRefreshFailureDiagnostics. This retry path logs nothing, so a cwd that keeps failing retries every 15 minutes with no warning.

Build the delay from the shared helper, using the retry attempt count. Then log each failed attempt the same way the main loop does.

Fixed the verified follow-up review findings in 9a16e7846d62c3352b4f7eaec59e580820428635 with a guarded normal descendant push. Read the actual failed CI Lint job 39414972595/118601671157; it reported formatting only in apps/server/src/vcs/VcsStatusBroadcaster.test.ts. Applied the repository formatter to that file and verified focused format/lint checks. This follow-up changes formatting only, so the previously recorded behavior tests remain applicable; no repo-wide checks or installs were run. Existing draft/product/evidence gates remain.

Verified in a real dev environment

Verified head 2607cc6b3d08ee95fc077dfbab4f55f471503e69 on this Linux VM, using repository Playwright with Google Chrome and real Codex gpt-6.1-sol turns through the web client. No source changes were needed for the exercised flows.

  • Reproduced on github/main (575579d68a) in a separate worktree first. Created the project and repository data through the app and Codex, then visited 269 distinct real repository directories through authenticated VCS WebSocket subscriptions. Read-only debugger inspection of the running broadcaster showed 272 retained snapshots on main versus 256 on this head. Revisiting the oldest directory returned its retained clean snapshot on main; this head reloaded the real uncommitted file change. This is a retention-count observation, not a heap-byte or timing benchmark.
  • Kept a remote subscriber open across eviction with automatic fetching disabled. A new subscriber received snapshot remote=null, followed by remoteUpdated; the existing subscriber received the update too, without reconnecting or manually refreshing.
  • Used a real bare local Git upstream to verify ahead/behind status. Codex switched from main (one ahead) to a new branch without upstream; the web client displayed the new branch and the server cleared the old upstream/ahead state. Another real turn changed a tracked file; the client completed the turn and Open diff displayed the exact new line.
  • Blacksmith ran vp i && vp test run apps/server/src/vcs/VcsStatusBroadcaster.test.ts: 30 tests passed. Reported non-skipped GitHub CI checks were also green at this head.

Runtime command transcript

Real Codex turn and client diff on the verified head

Real Codex edit, completed turn, and Open diff interaction

Not verified

Native desktop/iOS/Android clients, relay/tunnel connections, external forge PR metadata, real transient remote failures/retries, and Pi transcript discovery were not exercised. The branch-switch flow cleared old remote state, but the discussion's narrower raw-finalization stale-remote case was not isolated from client revalidation; this verification does not dismiss that finding. No heap-byte or performance timing claim is made.

For matching app-created data, the PR server reused main's isolated dev home. It warned about main's newer migration 76 being unknown to this build and continued; the exercised flows remained usable. The two dev servers ran sequentially.
