fix(server): classify Codex 429 retry exhaustion as a usage limit - #17501

Conversation
Codex reports retry exhaustion as responseTooManyFailedAttempts, and only the embedded httpStatusCode says the last attempt ended on HTTP 429. All three failure-classification sites treated it as provider_error, so a turn stopped by rate limiting lost the Limited state and manual Resume controls instead of keeping the documented recovery path (pingdotgg#16288). Exhaustion on other statuses keeps the existing provider/transport classes; only the 429 variant is classified as a usage limit.

ApprovabilityVerdict: Approved at Macroscope's review found this PR approvable — This is a narrowly scoped Codex adapter bug fix that classifies existing retry exhaustion with HTTP 429 using the established usage-limit recovery path. It adds replay coverage for direct, contradictory, and incomplete error evidence without changing schemas, defaults, or other production components. You can add or adjust custom eligibility rules. Note Written by Hi! We are cleaning up open PRs, and this one names a harness (Codex) but not which model version created it. If this change is really important, we recommend rebuilding the PR with a newer model and noting the model in the PR description. |

@maria-rcks Thanks for reopening this. I've added explicit model/harness attribution to the description and independently re-reviewed the change rather than attributing an unrecorded model version to the original submission. The follow-up in 41be69b fixes the valid CodeRabbit finding and adds regression coverage. The original code fails six new classification cases; the fixed adapter passes all 176 tests, with a passing server typecheck and no new diagnostics. Validation evidence and the remaining live-provider/UI verification limitation are recorded in the PR description. The scope remains the same two-file fix for #16288. |

Taking over this one. I’ll reduce the tests to the useful coverage, update the implementation against current main, and verify it before publishing the changes. |

Problem
Codex retry exhaustion with an explicit structured HTTP 429 was reported as a generic provider error, hiding the existing usage-limit state. Cached notification evidence could also override a contradictory completion status.
Change
Classify
responseTooManyFailedAttemptswith numeric status 429 asusage_limitat both notification paths and root completion. Explicit completion status takes precedence over contradictory cached classification; missing status preserves earlier details and retry metadata. Non-429 exhaustion and unrelated transport errors keep their existing handling. The implementation uses one shared typed predicate and the generated Codex error-info union. Current root/descendant terminalization is preserved. Thanks @Hylouis233 for the original #17501 contribution and captured payload. Contributor history is preserved with main merged.
Scope and approval
Focused maintainer takeover addressing #16288. Two adapter source/test files change. No UI, other provider, automatic-resume policy or reset-time inference changes. No merge is requested.
Verification
At 49dacaa, with 253f139 merged:
No live Codex HTTP 429, durable orchestration replay, native client or end-to-end Limited/Resume UI was exercised. The structured payload shape comes from the contributor's captured app-server output and generated schema. Client presentation reuses the existing usage-limit path.
