OpenCode's read tool wraps directory listings in a // XML envelope, and T3 stored and rendered it verbatim, so tool rows showed raw tags instead of a file list.

The envelope is now stripped at the adapter boundary (with a display-layer fallback so already-stored threads render cleanly too), and the bare list/ls tools project as file search with a "Searched in <dir>" title instead of falling through to generic "Used N tools" groups.

With the fix:

📝 Walkthrough

Walkthrough

The changes classify OpenCode list tools as file searches and extract supported directory-entry or file-content envelopes from read output. The server adapter and client work log use the shared output cleaner.

Changes

OpenCode tool handling

Layer / File(s) Summary
Shared classification and output cleaning
packages/shared/src/toolActivity.ts, packages/shared/src/toolActivity.test.ts
classifyToolActivity now classifies list as search. The new cleanOpencodeReadOutput extracts directory entries or file content from supported envelopes and otherwise returns the input unchanged. Tests cover these cases.
OpenCode server tool projection
apps/server/src/orchestration-v2/Adapters/OpenCodeToolItems.ts, apps/server/src/orchestration-v2/Adapters/OpenCodeToolItems.test.ts
The adapter projects list and ls as file searches without a pattern. For listing tools, it can use the first output line as a fallback search root. It cleans defined dynamic-tool output. Tests cover projection kinds, bare-list titles, and directory-read output.
Work-log output formatting
packages/client-runtime/src/work-log/itemDetail.ts, packages/client-runtime/src/work-log/itemDetail.test.ts
The work log cleans string-valued dynamic-tool output before formatting. Omitted output still returns null, and non-string output is formatted as before. A test covers a legacy directory listing.

Change: Bug fix

Merge Risk

Merge Risk: ⚪ Minimal · up to 55f36

This change strips raw XML envelopes from OpenCode directory and file read output and shows list tools as searches. No actionable merge-blocking risk is visible in the supplied context.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 0c425

The change does not demonstrate broader tool permissions or a new execution path. However, cleanup intended for display can discard legitimate tool-result text before storage, affecting later readers and making restoration dependent on another source of the original output.

Retained concerns

  • Low · reliability · inferred: Lossy display normalization now changes durable tool-result data. Literal closing tags can truncate legitimate output before persistence, while the client fallback applies the same content-based transformation across providers and can normalize already-cleaned content again. The resulting timeline API text cannot recover discarded content by pagination or by reverting the display formatter alone. This is a bounded data-integrity and rollback concern, not a demonstrated privilege escalation; recovery from separately retained native output remains unverified.

Security review details

Security Blast Radius

  • inferred — The content-integrity exposure covers matching OpenCode dynamic-tool outputs in durable records and their existing timeline consumers. Display-only exposure is broader because the client fallback processes matching string outputs from any provider. The inspected paths do not demonstrate increased filesystem access, execution authority, or tenant reach.

Security Findings and Attack Paths

  • inferred — A producer able to influence matching tool-result text can influence which content survives normalization and reaches later readers. The supported consequence is loss of output integrity; execution, exfiltration, and authorization bypass are not established by this path.

Trust Boundaries and Controls

  • observed — The adapter preserves the supplied identity, ownership, status, and timestamp fields. Event ingestion retains its existing current-run and provider-thread-owner guarded write branches. Normalization itself does not grant authority or alter those lifecycle fields.

Resilience and Maintainability Implications

  • observed — The existing persistence handler replaces status and payload on conflict with the same turn-item ID rather than appending a duplicate row. It stores the supplied normalized payload without rerunning the cleaner. This preserves update identity but does not repair content already discarded before ingestion.

Hi! We are cleaning up open PRs, and this one does not say which model or harness was used to create it. If this change is really important, we recommend rebuilding the PR with a newer model and noting the model and harness in the PR description.

Please add the focused test commands and observed results, plus a before screenshot matching the supplied after image. The current description gives no check results, and the after image shows collapsed search/read rows rather than the XML contents being cleaned. Verification requires evidence of the changed behavior.

Also, how is changing bare list/ls projection and labeling necessary to remove the read XML envelope? The diff changes both. Please explain that causal relationship under one problem, or separate the independently useful change. Leaving this open for maintainer decision because Maria has commented.

Taking over this one. I’ll port the native read envelope fix to the extracted OpenCode adapter, keep unrelated listing behavior separate, and add focused proof and review before publishing a replacement on origin. Thanks @Ell1ott for identifying the rendering problem.

Thanks @Ell1ott for identifying the raw read envelopes and supplying their directory/file shapes. I rebuilt this as #21507 because the provider adapter has moved to its own package.

I compared the full original diff against the replacement: native directory/file cleanup now lives in the shared OpenCode adapter projection, including literal closing-tag and suffix preservation. The independent list/ls categorization/labels/root fallback are omitted; the generic historical client cleaner is also omitted because it could alter unrelated custom-tool output. Existing stored rows remain unchanged. Focused tests, independent reviews, and matching actual-main/head screenshots are in the replacement. Closing this original in favor of that focused PR; nothing has been merged.
