fix(server): load merge request diffs on older GitLab

Addresses the diff-loading failure in #15660. GitLab before 15.7 returns 404 for the MR /diffs endpoint, leaving the Code tab unable to load changes. This focused compatibility fix follows the issue triage and the GitLab changes API contract.

On a first-page MR not-found failure, retry /changes?access_raw_diffs=true, decode its wrapped changes, and return one terminal slice. Preserve overflow and changes_count ending in + as truncation signals, plus the existing 8 MiB output limit. Modern pagination, commit request routing, later-page failures, and other errors keep their existing behavior. The shared patch builder also marks skipped malformed file entries as truncated, so neither response shape presents missing files as complete. Generic error-message improvements remain out of scope.

Verification:

• Before the fix, the new boundary suite had 12 failures and 5 passing controls. After the fix and an additional malformed-modern-response control, all 132 tests passed across GitLabPullRequestCli.legacy.test.ts, GitLabPullRequestCli.test.ts, gitLabMergeRequestJson.test.ts, and sourceControl/GitLabCli.test.ts (vp test run with those four files).
• The new suite uses real GitLabCli request construction/error mapping and real JSON decoding, with fixture responses injected only at VcsProcess. It checks encoded project paths, the raw-diff query, 0/1/100/247-file terminal slices, both truncation flags, malformed wrappers, byte truncation, fallback failure, non-404 errors, commit/later-page exclusions, and modern pagination.
• Targeted vp lint and formatting on the three changed files; tsc --noEmit -p apps/server/tsconfig.json; server-scoped Knip exports check; git diff --check.
• CodeRabbit identified missing truncation metadata for skipped malformed file entries. Added modern and legacy regression cases (both failed before the follow-up fix, then passed), and retained valid patches while marking the slice truncated.
• Independent code review found no blocking issue. No live GitLab instance or browser was used; process error classification is supplied by the fixture, not exercised against glab stderr.

The change stays in the server GitLab adapter, so all clients and remote connection modes use the same behavior without a wire-contract or UI change. Other hosting providers and agent runtimes are unaffected.
