After a V2 server restart, resuming a Codex parent can revive its existing native children while their T3 status and conversations remain stale. The adapter loses its in-memory child registrations, so their events have no routing context.

Fixes #15567. The maintainer triage identifies the lost registrations and the need to preserve each child's original identity.

Restore saved child threads, launch items, task identities, and native turn history when activity resumes. Direct children retain their app-thread parent across a native-thread handoff; nested children restore their saved ancestors. Keep already-running children with their existing owner and recover newly active children under the latest parent attempt.

Buffer early child notifications and replay them after registration. Parent completion waits for saved-child lookup and replay, so its event consumer cannot close before recovered messages arrive. Preserve newer live model metadata, ignore previously saved native turns, and keep typed lookup failures nonfatal. Ordinary starts, context handoffs, compaction, and native goal starts use the recovery path.

verification after updating against current main on october 11:

  • the initial update passed all 188 codex adapter and provider turn-start tests. after review, all 173 adapter tests pass, including a deferred-gated overlap between root recovery and a grandchild notification. that regression test fails on the previous head with two child registrations and passes with one after the fix.
  • adapted recovery to provider-core services and retained newer live model selections, including their options. competing restorations now keep the first registered child context and preserve newer turn ordinals.
  • scoped typecheck of all changed files passes. targeted lint reports only the two existing unused declarations. formatting and whitespace checks pass.

Verification uses protocol replay and retained in-memory projections. No live provider or client session was launched, and the change does not backfill arbitrary native history.

Implemented initially by GPT-6.1 Sol at xhigh through Codex in T3 Code. Updated by GPT-6 Astra at high through Codex in T3 Code. An independent GPT-6 Astra reviewer at high identified three additional issues; all were corrected, and its follow-up review found no remaining actionable issues.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — This change adds automatic Codex subagent recovery across restarts, handoffs, nested children, and concurrent notification ordering. It introduces substantial stateful production logic across the adapter, turn-start service, and shared provider contract, with a broader runtime surface than a routine bug fix.

The turn-start service provides saved child-thread state to Codex sessions. CodexAdapterV2 restores subagent contexts from projections and queues notifications until child registration. Tests cover recovery after restart, nested children, lookup failures, and child-event ownership across root turns.

Merge Risk: 🔵 Low · up to 90b26

The change is mergeable with owner awareness: an interrupted child restoration could leave earlier child activity untracked. Saved direct children can restore across parent handoffs.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 956cb

Restart recovery preserves child identities and existing approval ownership. No new security finding was established, but interruption and shutdown overlapping recovery remain incompletely verified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • observed — The added recovery path reaches saved child metadata, nested ancestry, and event routing for Codex sessions. The server loads provider-thread and turn metadata plus the parent launch records, without loading child conversation or tool-output projections.

Trust Boundaries and Controls

  • observed — Provider-reported native thread IDs select recovery candidates, but are not sufficient to establish ownership. Lookup requires durable subagent lineage and a matching provider-native launch record. Registration additionally checks Codex driver, provider instance, derived child identity, native task identity, and nested fork ancestry.

Resilience and Maintainability Implications

  • observed — Child initiation checks interrupted or failed ancestors and requests native interruption instead of reopening their descendant work. Interrupt handling also discovers late descendants and finalizes remaining active lineage. These are meaningful containment controls, but do not establish every shutdown-versus-recovery interleaving.
  • observed — A completed child that resumes much later does not pin the session between turns, so idle release can precede its later activity. The same pending-work logic and documented limitation exist at the PR base; this review did not establish that recovery worsens their exposure.

Hardening Proposals

  • proposed — Extend recovery validation to interruption and session shutdown while saved-child lookup or replay is paused. Verify that interrupted ancestry cannot reopen work and that shutdown releases session resources without leaving recoverable routing state active.

Restore direct children by app-thread lineage after a provider-thread handoff.

When a handoff keeps the Codex provider session but starts a new root provider-thread ID, a saved direct child still names the old root provider-thread ID. This branch then tries to restore saved.subagent.threadId, which is the root app-thread ID. readNativeSubagentThread returns null for root threads, so restoration cannot register the child. The same-session run can still route notifications for routable child provider threads, leaving the child turn pending instead of projecting its activity. Use rootContext for a direct child and recurse only for a nested child.

2890-2911: 🚀 Performance & Scalability | 🔵 Trivial | ⚡ Quick win

Read the saved child once per turn/started, not once per tracked root.

rememberSubagentTurnStarted calls restoreSubagentThread(childThreadId, rootContext) once for each entry in latestRootTurns. Each call runs rootContext.input.readSubagentThread(childThreadId). The lookup is keyed only by childThreadId, so every iteration reads the same projection records. Only the validation in restoreSubagentThread depends on the root (saved.subagent.threadId compared with rootContext.projectionThreadId).

This path runs inside the turn/started notification handler, which is on the protocol reader. A shared app-server can serve N app threads. In that case, every turn/started from an unregistered child causes up to N projection transactions before the reader continues. Each new child spawn triggers this path, and any child the saved projection does not know triggers it as well. If a lookup fails, the adapter also logs orchestration-v2.codex-subagent-restore-read-failed up to N times for one notification.

Read the saved record once. Then select the single root whose projectionThreadId matches saved.subagent.threadId, or the root of the child's lineage for nested children. Pass that record into the restore step. One way to do this is to split restoreSubagentThread into a lookup step and a validate-and-register step that accepts the ProviderAdapterV2SubagentThread it already read.

addressed the repeated recovery reads. one turn-start notification now shares a temporary map across eligible roots and nested-parent recovery, including missing or failed lookups. the map is discarded afterward, so later activity can retry recovery with fresh projection data. pending-root ownership checks are unchanged.

all 172 codex adapter tests and the scoped typecheck pass.
