Problem

Every imported Claude Code session fails on its first follow-up with Session ID <id> is already in use, whether it came through the welcome wizard or a per-session import. The import writes the provider thread with a strong nativeThreadRef, nativeConversationHeadRef: null and no provider turns. In ClaudeAdapterV2.openQuery, shouldResume is true only when a conversation head exists, this session instance already opened the native thread, or providerTurnOrdinal > 1. None of those hold for the imported thread's first turn, so makeClaudeQueryOptions sends sessionId instead of resume and the CLI rejects the id it already owns. provider_resume_fallback never fires because the Claude resumeThread succeeds; the failure comes later, at query open.

Reported in #15243 with the CLI repro and logs; confirmed by triage on main at ce90eec1f.

Change

Both import paths now record where the native thread came from. AgentSessionImporter and the orchestrator's importedNativeThread branch stamp nativeMetadata.nativeThreadOrigin = "imported" on the provider thread they write, next to the existing itemIdentityVersion marker that ProviderTurnStartService already reads from the same object. ClaudeAdapterV2.openQuery treats a provider thread carrying that origin as resumable alongside the three existing signals, so the first follow-up opens with resume: <session id>.

Rows imported before this change carry no marker, so both already-imported paths of AgentSessionImporter (the scanner's AlreadyImported outcome and an existing v1_import thread) backfill it: they load the thread's provider threads and write one provider-thread.updated event stamping the origin when it is missing. The provenance lives on the persisted provider thread row, so it survives idle release and a failed first follow-up, and the orchestrator's run-start binding (providerSessionId, status, run ordinals) leaves it untouched. Threads this adapter minted carry no origin and keep taking the sessionId path, which is the failed-create retry the comment above openedNativeThreads protects. Codex is unaffected: it resumes any native ref already.

Triage named two directions. Honoring the stored resumeCursor would need the runtime row plumbed from persistence through ProviderTurnStartService into the adapter, and recording a conversation head would drive resumeSessionAt, which needs a message id the import does not have. Stamping the origin the import already knows is smaller than either and keeps the adapter decision local.

Review history: the first commit keyed the decision on providerSessionId === null at resumeThread. CodeRabbit pointed out that the orchestrator binds a provider session to the thread at run start, before the adapter is reached, so that guard could not fire in production; the second commit replaces it with the persisted origin.

Scope and approval

Bug fix for the triaged issue #15243. Triage confirmed the failure on current main, traced both code paths, and named shouldResume in ClaudeAdapterV2.openQuery as the fix area. One underlying problem; the change touches the provider-thread contract (one optional metadata field), the two import paths, the Claude adapter, and their tests.

Verification

Focused tests, run with vp test run in apps/server and packages/contracts:

  • ClaudeAdapterV2.test.ts, ClaudeAdapterV2 native session identity:
    • resumes an imported native thread on its first provider turn: a provider thread shaped the way production hands it to the adapter (bound provider session id, strong native ref, no conversation head, nativeThreadOrigin: "imported") goes through resumeThread, then a turn at providerTurnOrdinal 1 opens with resume: "imported-native-session" and no sessionId. Red on the first commit's providerSessionId === null guard (expected undefined to equal 'imported-native-session') and on main; green now.
    • still creates a minted native session that resumeThread re-attached: a thread this adapter minted passes through resumeThread and its first turn still opens with sessionId. Guards the failed-create retry; passes before and after.
  • AgentSessionImporter.test.ts asserts the written provider thread carries nativeMetadata.nativeThreadOrigin: "imported", and stamps the import origin on provider threads imported before it existed re-imports against a legacy row without metadata (exactly one stamp event) and against a stamped row (no write); ThreadLaunchService.test.ts asserts the orchestrator's imported-thread mapping carries it next to the caller's metadata.
  • Results: ClaudeAdapterV2.test.ts, AgentSessionImporter.test.ts, ThreadLaunchService.test.ts and ProviderTurnStartService.test.ts 188 passed; packages/contracts 493 passed.
  • pnpm run typecheck at the repo root (all packages, since the contract changed): 0 errors. vp fmt --check: clean. vp lint --report-unused-disable-directives: exit 0, no new warnings on changed lines.
  • Not checked: a live Claude Code import round-trip. The reporter's CLI command (claude -p --session-id <imported id> → already in use) is the behavior the resume path avoids.

Claude Fable 5.1 (claude-fable-5-1) in Claude Code did the implementation; the account holder reviewed and owns the commit.

…ow-up

An imported Claude Code session is written as a provider thread with a strong
native ref, no conversation head and no provider turns. `openQuery` only
resumed when one of those existed or this session instance had already
opened the native thread, so the first follow-up reopened the session with a
fixed `sessionId` and the CLI rejected it: "Session ID ... is already in use".

`resumeThread` is how the orchestrator re-attaches a provider thread that
already carries a native id. When the thread has never been bound by a
provider session of ours (`providerSessionId === null`, the shape both import
paths write), record its native id as adopted and let `openQuery` resume it.
Threads this adapter minted keep their provider session id, so a retry after
a failed first create still creates the session.

📝 Walkthrough

Walkthrough

Imported provider threads now include origin metadata. The Claude adapter uses this metadata to resume an imported native session on its first provider turn.

Changes

Imported session resumption

Layer / File(s) Summary
Mark imported provider threads
packages/contracts/src/orchestrationV2.ts, apps/server/src/project/AgentSessionImporter.ts, apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/project/AgentSessionImporter.test.ts, apps/server/src/orchestration-v2/ThreadLaunchService.test.ts
The metadata contract adds the optional nativeThreadOrigin value "imported". The importer stamps matching existing provider threads and initializes new ones with this value. The orchestrator retains supplied metadata while adding the marker. Tests cover imported metadata and repeat imports.
Resume imported Claude sessions
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter treats an imported thread as requiring session resumption, even when no provider turn has been persisted. Tests cover imported first-turn resumption and minted-thread behavior.

Comment thread apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts Outdated
The first version keyed the resume decision on `providerSessionId === null`
at `resumeThread`, but the orchestrator binds a provider session to the
provider thread when it starts the run, before the turn-start service ever
reaches the adapter, so the guard could not fire in production.

Both import paths now stamp `nativeMetadata.nativeThreadOrigin = "imported"`
on the provider thread they write, and `ClaudeAdapterV2.openQuery` reads it
from the turn's provider thread next to the existing resume signals. The
provenance is persisted with the row, so it survives idle release and a
failed first follow-up, and threads this adapter minted carry no origin and
keep creating their session.

CodeRabbit was right that the first commit's providerSessionId === null guard could not fire in production: the orchestrator binds a provider session to the provider thread at run start, and ProviderTurnStartService requires that binding before it reaches the adapter.

  • Both import paths (AgentSessionImporter and the orchestrator's importedNativeThread branch) now stamp nativeMetadata.nativeThreadOrigin = "imported" on the provider thread they write. The field is a new optional literal on OrchestrationV2ProviderThreadNativeMetadata, next to itemIdentityVersion.
  • ClaudeAdapterV2.openQuery reads it from the turn's provider thread; the resumeThread bookkeeping from the first commit is gone.

Tests: the adapter case now shapes the provider thread the way production hands it over (bound session id, strong native ref, no head, imported origin) and fails against the first commit (expected undefined to equal 'imported-native-session'); AgentSessionImporter.test.ts and ThreadLaunchService.test.ts assert the origin on both import paths. 188 server tests across the four touched files and 493 contracts tests pass; repo-wide pnpm run typecheck 0 errors; vp fmt --check and vp lint clean.

Comment thread apps/server/src/project/AgentSessionImporter.ts
…lier

Rows written before the import origin existed carry no marker, so their
first Claude follow-up still reopened the session with a fixed id. Both
already-imported paths of AgentSessionImporter now load the thread's
provider threads and stamp `nativeThreadOrigin: "imported"` on the row for
the imported session when it is missing; rows that already carry it are left
alone.

CodeRabbit's second point stands: rows imported before the origin marker existed have no marker. Both already-imported paths of AgentSessionImporter now backfill it, writing one provider-thread.updated event with nativeThreadOrigin: "imported" on the row for the imported session when it is missing and leaving stamped rows alone. New test stamps the import origin on provider threads imported before it existed covers both outcomes. Importer, Claude adapter and thread-launch suites 173 passed; pnpm run typecheck, vp fmt --check and vp lint clean.

Report failed origin stamps as incomplete.

If the orchestration_events insert for the origin stamp fails before commit, stampImportedOriginOrWarn logs the error and returns success. For an AlreadyImported thread, the importer has already incremented importedCount and does not increment skippedCount. When this is the only import outcome, WelcomeWizard marks the project imported and skips it on same-session retries. The missing marker can leave Claude’s first follow-up, when no other resume signal applies, on the fixed-sessionId path instead of resuming the native thread. Report the stamp failure as incomplete so the project remains retryable.

One upgrade path still seems uncovered at de8d7003: continuing a thread imported before the update without rerunning import. With the real importer and adapter plus injected storage/SDK boundaries, an existing imported row with a native ID, no origin marker, no conversation head and turn ordinal 1 still selects sessionId. Explicit reimport through either v1_import or AlreadyImported makes it select resume instead.

I reproduced both paths: the 135-test importer/adapter run passes, but the two assertions expecting direct continuation to resume fail. Tested main at a1d9d72a behaves the same on direct continuation, so this is an uncovered upgrade case rather than a new regression. Is reimport an intentional requirement, or should existing rows be repaired when continued? This assumes the native transcript exists; I verified query selection, not a live CLI round-trip.

Before review handoff, please resolve the existing upgrade-path question. At de8d700, the origin backfill runs through AgentSessionImporter, while the adapter's new resume condition requires that marker. The reported direct continuation of a previously imported, unmarked thread still chooses sessionId unless import runs again.

Is reimport an intentional requirement for those existing threads, or should direct continuation repair them? Please state the intended scope and provide the corresponding observed check under the problem/scope and verification requirements. Leaving this open pending that answer.
