An intentional worktree handoff closes the old provider session. Treating that close as an unexpected stream failure holds the continuation forever; recording binding and continuation in separate transactions can also publish a delegated result before the continuation exists.

Record workspace binding and continuation intent atomically, forward a typed planned-detach reason, and finalize that detach as an interruption that permits the continuation to run. Cover detach before the provider's first turn, scope the reason to the correct thread/session, preserve receipt replay, and reject a new continuation without a changed worktree. Real provider failures and explicit Stop continue holding queued input until explicit resume.

Before and after

8-second captioned comparison

Before After
Before: provider stream error and continuation pending in Queued 1 After: planned workspace interruption, continuation delivered and completed, pending queue removed

The pending Queued 1 card disappears after completion. The small Queued badge in message history records how that delivered message was submitted; it does not represent pending work.

These are real T3 Browser captures of durable checkpoints produced by the real orchestrator, session manager, effect worker, ingestion, checkpoints and SQLite persistence with a deterministic exclusive provider adapter. The before control removes only planned-detach reason forwarding from a copied worker to reproduce the former failure. After uses this PR's production runtime unchanged. Queue, run, message and result state were generated through normal events, not manually edited. The video compares recorded checkpoints; it is not a live provider CLI recording.

Engine verification: held → running → completed, provider workspace starts, exactly one result

Validation

  • 114/114 focused tests across SelectionRestart.integration.test.ts, RunExecutionService.test.ts and WorktreeMcpService.test.ts on this standalone branch. Includes both queue/detach orderings, a true delegated child, early detach, wrong thread/session controls, receipt replay, unchanged binding rejection, and real crash/Stop controls retaining queued input.
  • The delegated integration test verifies one continuation, one final result transfer, exactly one visible assistant response, and one continuation start in the new workspace. Before/after snapshot tests and SQLite integrity checks passed; completing the parent for viewing leaves the child's captured state unchanged.
  • Server and contracts typechecks, changed-file formatting and scoped lint passed (existing unused-variable/parameter warnings). Independent GPT-6.1 Sol HIGH review found no remaining findings.
  • One commit directly on upstream main (4df84a7d03); no dependency on the delegated-state reconciliation PR. The lifecycle runs on the server for web, desktop and mobile. Installed provider CLIs and native mobile were not exercised. The installed app, live database and historical game queues were not changed.

Model and harness: GPT-6.1 Sol (HIGH), T3 Code through the Codex harness.

Rebase updates: GPT-6.1 Sol through the Codex harness in T3 Code.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — This PR introduces cross-component production behavior for atomic worktree handoffs, provider detaches, queue promotion, and run finalization. Unresolved integration concerns remain around selecting the new worktree for checkpoints and signaling early detaches in shared sessions.

📝 Walkthrough

Walkthrough

Worktree handoffs can include a continuation in the metadata update. Workspace-change detachments now produce a typed error that interrupts the matching run. Tests and documentation cover continuation timing and queued-input behavior.

Worktree handoff and continuation

Dispatch the binding and continuation

The metadata update accepts and validates a continuation and dispatches it with the worktree binding. The handoff reports delivery from stored events instead of sending a separate message.
Propagate planned detach interruptions
apps/server/src/orchestration-v2/EffectOutbox.ts, apps/server/src/orchestration-v2/EffectWorker.ts, apps/server/src/orchestration-v2/ProviderAdapter.ts, apps/server/src/orchestration-v2/ProviderSessionManager.ts, apps/server/src/orchestration-v2/RunExecutionService.ts, apps/server/src/orchestration-v2/RunExecutionService.test.ts
Workspace-change detach effects carry a reason. The session manager emits a typed detach error, and run execution treats a matching error as an interruption with a workspace-change message.
Verify handoff and queued-input behavior
apps/server/src/orchestration-v2/SelectionRestart.integration.test.ts, docs/internals/overview.md, docs/user/composer.md
Integration tests cover handoff timing, delegated continuations, and queued messages after provider failure or explicit Stop. The documentation describes automatic continuation and queue resumption.

Merge Risk

Merge Risk: 🟡 Moderate · up to 55e45

A worktree handoff can leave the old run waiting indefinitely in shared provider sessions. A queued continuation can also record checkpoints against the previous worktree. Both should be fixed before merging.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 55e45

The handoff now records its follow-up message together with the workspace change and checks thread/session identity before resuming. No new access-control bypass was established, but shared-session recovery and rollback workspace selection remain incompletely verified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The reachable public operation requires authenticated environment-level orchestration authority and targets one thread per command. Repetition is not bounded to one thread by that scope check; project or tenant isolation beyond the environment boundary was not established by this review.

Trust Boundaries and Controls

  • observed — Websocket upgrade authenticates the session, and RPC middleware requires orchestration-operate scope before command handling. The inspected revision comparison shows no change to these enforcement files.
  • observed — The new public payload is converted into an agent/MCP message with no attachments and queued delivery. The previous handoff used the same agent/MCP provenance through sendToThread. The payload does not carry notification or delegated-completion fields, whose separate provenance controls remain enforced.

Resilience and Maintainability Implications

  • observed — Queue scheduling refuses held queued input and retains recovery gating after non-validation provider failures. Focused integration tests cover planned handoff ordering, early exclusive-session detach, explicit Stop and provider-failure controls.
  • observed — The new typed subscriber failure is sent when an exclusive session is released. Shared multi-thread sessions instead unload native threads without that signal. Codex unload requests thread unsubscribe; OpenCode2 unload returns without cleanup for active or background work. Equivalent terminalization, especially before native thread identity exists, remains unproven. The manager's shared unload branch predates this PR.

Use the new worktree for queued continuation checkpoints.

When runtimePolicy.resolve returns no cwd, dispatchMessage selects selectedProviderSession?.cwd before projection.thread.worktreePath. The queued run stores that checkpoint scope, so checkpoint operations can target the previous worktree.

Signal the detach to the affected run in shared sessions.

When a workspace-change detach reaches a multi-thread-capable session after the run subscribes but before its first provider turn is recorded, activeTurns is empty, so no turn is interrupted. The shared-session branch does not fail the subscriber; only the non-shared final-release branch supplies expectedDetach. RunExecutionService needs a matching ProviderAdapterSessionDetached to synthesize an interrupted terminal, so the run can remain subscribed while it waits for provider events. Route the detach only to that thread’s subscriber, without failing subscribers for other attached threads.
