feat: add Claude Code memory integration

Summary

• Add a Claude Code memory integration under memind-integrations/claude-code with prompt retrieval, conversation ingestion, PreCompact ingestion, SessionEnd commit, retry spooling, local state management, and unit tests.
• Add an open health endpoint for integration readiness checks and register a local Claude plugin marketplace manifest.
• Fix startup/runtime robustness by lazily wiring audio/image raw data parsers, hardening insight evidence normalization, and renaming the server DDL bootstrap migration to V1.
