Rationale
When GOAWAY marks an upgrade stream as unaccepted,
severRequestStream()already releases its HTTP/2 session. Upgrade streams also retain
onUpgradeStreamClose, which later attempts the same release after the streamcloses. The second release dereferences a cleared session and escapes as an
