ngx_http_proxy_create_request can be more friendly to the case about removal of the headers in rewrite handlers #1

Currently ngx_http_proxy_create_request doesn't handle header.hash == 0 case which is often used in removal of response headers. This causes rewrite handlers need more work to remove request headers. We can enhance it by modifying two lines of code.
