Expanded folders in the Explorer now keep themselves up to date: files and folders created, deleted or renamed outside TuiCode (another terminal, git switch) show up in the tree within about a second, with no keypress. Expansion and selection survive the refresh the same way they do for Refresh explorer.

Closes #334

Choices worth a look:

  • An automatic refresh only scrolls to keep the selection in view if it was already in view, so a tree you've scrolled away from the selection doesn't jump back. Refresh explorer keeps its existing behaviour.
  • Changes inside collapsed folders are ignored until they're expanded, as the task says. In-app create/rename/delete also trigger one redundant refresh 250 ms later, which is harmless.

Each expanded folder gets a non-recursive watcher (capped at 64, most
recently expanded first) and refreshes itself 250 ms after entries appear,
go or are renamed, keeping expansion and selection as Refresh explorer does.
TG's TreeView raises no expand/collapse events, so the view re-derives its
expanded folders after its own refreshes and on each draw.

Closes #334
