docs(spikes): cover P3 no-gate mutation scopes in the governance audit - #202

The #182 audit analysed P3 only through the responder's auto-accepted proposal. It did not cover the policy.yaml scopes with requiresApproval: null, the undecided incident-resolve scope, or the incident mutations the policy checkpoint allows. This adds that inventory and records three findings (G12-G14) as owner decisions outside the #201 graph. D5 and #201 are unchanged. G12 and G13 are security-sensitive and are stated at contract level only; the existing P3 text is reworded to the same level. G14 is a plain correctness bug.
