Port of pingdotgg#14148 while it is still open upstream. Adds claude-sonnet-5-5 (minVersion 2.1.284, default effort medium) with its own profile and moves Sonnet 5 to legacy. Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>

The fork was one upstream commit behind, and Claude Sonnet 5.5 (shipped in Claude Code 2.1.284) is not in the model picker yet because upstream has not merged it.
