fix(codex): retire a pool account's carried monthly window when WHAM shows none - #8120
A stored pool account that moved from a monthly-primary plan (Free) to a weekly plan (Pro Lite) kept its old monthlyPercent: 100 (with monthlyIsPrimaryWindow: true) forever. Every later WHAM probe reports only a weekly window and secondary_window: null, but mergeAccountQuota carries an omitted monthly tuple forward, so isCodexQuotaExhausted() kept classifying the healthy account as exhausted and pool selection never picked it.
A complete WHAM topology already proves short-window absence for pool accounts (#7961). The same check now also marks monthly-window absence when no window declares a monthly duration and no tertiary window is present, and the merge stops carrying a monthly tuple when that absence is proven. Retirement uses exactly the same proof as the short-window retirement: a live captured pool writer for that account and credential generation. Partial WHAM, an explicit allowed: false, invalid or unparseable readings, response headers and a missing, stale or other-account writer all keep the carried tuple. A present tertiary window replaces the reading as before; real monthly-primary plans (Team, Go/Free) are unchanged. The marker is derived only in poolWhamPolicyObservation, which already rejects an explicit allowed: false and malformed readings, including additional_rate_limits. Main-account observations keep their existing monthly retention rules. The marker is consumed by the merge and never persisted.
Before: weeklyPercent: 0, monthlyPercent: 100, monthlyIsPrimaryWindow: true after every refresh, and 0 requests routed to the account.
After: weeklyPercent: 0 with no monthly fields, and the account is eligible again.
bun test tests/codex-integration/pool-monthly-window-retirement.test.ts: 6 pass, 0 fail. On the dev source, the pool plan-change retirement case and the policy-evidence marker case fail. Earlier PR heads fail the main-account cases: a denied snapshot and a malformed additional window (review findings). Deriving the marker only on the validated pool path fixes both.

bun test tests/ci-workflows/file-size-ratchet.test.ts plus the layout tests after rebasing on dev: pass. The new test is registered on an existing line of layout.json, so the file stays at 1999 lines.
bun run typecheck, bun run privacy:scan, bun run structure:check: pass.
NOT RUN: bun run test:changed and the full bun run test. quota.ts sits on the core request path, so the import-graph selection is close to the full suite. The local run was stopped after about 20 minutes without a result, on a host shared with other full-suite lanes. Left to this PR's test changed CI job.

Summary by CodeRabbit
Bug Fixes
Monthly quota details are cleared when a complete, verified pool update confirms that no monthly quota window exists, including during Free-to-Pro Lite changes.
Incomplete or unverified pool updates continue to preserve existing monthly details, while a newly observed monthly window replaces the cached reading.
Main-account updates retain monthly details when the observation is denied or contains malformed additional quota windows.
Tests
Added coverage for monthly quota updates and retention across complete, partial, denied, malformed, and unverified observations.
No actionable comments were generated in the recent review.
Walkthrough
Pool quota observations can now retire a carried monthly quota tuple when complete topology proves that no monthly window exists and the live-writer proof permits retirement. Main-account observations retain existing monthly data when absence is unproven.
The observation adds a monthlyWindowAbsent marker. Pool topology sets it only when short-window absence is proven, neither primary nor secondary is monthly, and tertiary is absent. Tests check monthly and incomplete topologies.
Apply absence to quota carry src/codex/quota.ts, tests/codex-integration/pool-monthly-window-retirement.test.ts, structure/providers/openai-tiers.md, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json
The parser forwards absence markers only when the writer proof permits retirement. The merge skips carrying the monthly tuple when absence is proven. Integration tests cover replacement monthly data and cases where monthly data must remain, including main-account observations. The provider contract and test-layout mapping reflect the behavior.
The change retires stale pool monthly quotas while preserving main-account monthly data. No actionable merge-blocking risk remains, subject to normal checks.

Full details: Linked Issues check
Explanation
Directly linked issue #8067 asks that a healthy pool account without a monthly window stop carrying the synthesized monthlyPercent: 100. Its suggested option 2 is to track monthly-window absence explicitly, as shortWindowAbsent does. The reviewed head implements that option. In src/codex/quota.ts, poolWhamPolicyObservation returns monthlyWindowAbsent: true only when all of these hold: the observation is not denied or malformed, shortWindowAbsent is already proven, neither primary nor secondary declares a monthly duration, and no tertiary window is present. mergeAccountQuota skips the monthly carry when that marker is set. setAccountQuotaFromParsed forwards the marker only when the same writer proof that gates shortWindowAbsent passes. The marker is typed as transient and is not written to stored snapshots. The main-account path no longer receives the marker. The test file tests/codex-integration/pool-monthly-window-retirement.test.ts covers the Free-to-Pro Lite retirement case and the retention cases. Evidence limit: the file read was truncated before the livePoolEvidence definition, so the writer gate was not re-read in this review. This review relies on the shared #7961 proof. Whether a blocked account in the reported scenario produces a live captured pool writer remains an end-to-end question that this review did not establish.

Explanation
All changed files relate to #8067. src/codex/quota.ts implements the monthly-absence marker, its pool-only derivation, and the merge change. The incremental diff from the previous reviewed head only moves the marker derivation from withWhamShortWindowAbsence into poolWhamPolicyObservation, which is the scope the PR describes. tests/codex-integration/pool-monthly-window-retirement.test.ts provides regression coverage. scripts/test-layout/layout.json and tests/fixtures/test-layout-expected.json register that test and add no behavior. structure/providers/openai-tiers.md documents the new retirement rule. No unrelated production behavior change was found in the inspected diff.

Add a same-account stale-generation writer case to the monthly retirement tests.
The file covers a missing writer and a writer for another account. It does not cover a writer for the same account whose credentialGeneration no longer matches the live credential generation. isPoolQuotaWriterLive rejects that writer, so the carried monthly tuple stays in place. If that generation comparison is removed, these tests still pass. The helper-level test in codex-account-store.test.ts would still catch the removal, but the monthly-retirement boundary would have no case for it.

Add a same-account stale-generation case to the monthly retirement tests using
commitPoolQuotaResponse: increment the writer’s credentialGeneration while
preserving its accountId, then assert monthlyPercent remains 100. Keep the
existing missing-writer and other-account cases unchanged.
