fix(codex): isolate Pool installation identity per account - #8065
Codex stores one installation ID per CODEX_HOME. Pool accounts sharing that home must not send the same caller x-codex-installation-id alongside different ChatGPT credentials. Derive a stable account-specific UUIDv4 from a persistent private local secret and the selected account identity, independent of access-token refresh.
Rewrite only existing installation headers and client_metadata fields for added Pool accounts. Preserve caller values for main and main-pool; absent surfaces remain absent. Keep HTTP, WebSocket and compact/failover identities synchronized without mutating the incoming raw metadata. Required HTTP identity failures return a sanitized 503 before dispatch; live WebSocket frame failures close safely without forwarding. Optional helpers disable themselves.
Defer native profile installation_id switching: the existing vault, journal and rollback transact auth.json only. A second file needs explicit snapshots and crash-recovery semantics. This PR isolates Pool wire identity without expanding that transaction.
Verification
Round 3 fix head: 502173bb9b7957b57ee9208d662e9135257a380f, based directly on 472e0ba966b6fd97235a0d2b7c34d788110774c9. Local evidence is from Windows with Bun 1.4.4 on the committed code. Final targeted validation: 646 pass / 0 fail / 0 skip across 20 files. Full and changed suites were expressly prohibited by the user and were not run.
Live projects recognized JSON content independently of Content-Type before the physical Pool send. A real local listener covers application/json, text/plain, application/octet-stream, vendor +json and application/sdp hints with both valid and corrupt secret storage. Root/session metadata is isolated with an independent HMAC oracle; identity failures return the exact sanitized 503 with zero upstream sends. Legitimate SDP, main/main-pool bytes, absent surfaces and original request bytes are preserved.
Credential admission rejects blank/whitespace canonical account IDs in both legacy and generation-wrapped records. Real fixed-account selection refuses them. Independent shared header/turn/client-metadata and WS-frame projection checks fail closed for invalid selected Pool contexts, including turn-only and metadata-free inputs. A separate 32-case real HTTP matrix covers Responses, compact, search and Live with turn-only headers/bodies and valid/corrupt secrets; malformed credentials produce admission refusals with zero sends. Four real Live WS upgrade cases refuse before socket creation. These admission refusals are separate from the sanitized 503 contract for a projection/derivation failure.
The existing empty-ID refresh regression now asserts the stronger admission contract: neither unidentified record can refresh or propagate; zero sends occur and both stored credentials and generations remain byte-identical. Its initial run was 69 pass / 1 fail because it assumed empty-ID credentials were still admitted; the updated contract passes all 70 cases.
Source audit rechecked shared projection early returns and final sends in Responses HTTP/WS, compact/failover, search, images, native Live, audio-live, context history, sidecar header materialization and Live WS immediate/queued frames. Search/images/compact/Responses project parsed bodies without a media-type gate. audio-live rejects unsupported body hints before sending; accepted session metadata always passes Pool projection. No source modules were added, and the existing responses-core module inventory and core-to-Lab import boundary remain green.
All external upstream sends in these tests are intercepted; local listeners and real account selection establish wire/refusal behavior, not actual ChatGPT acceptance or non-Windows runtime behavior.

Both new test files are registered in the explicit domain layout shard and expected-layout fixture. No size baseline was raised; no responses-core module list change was needed.
Independent fresh security/correctness review: PASS, no actionable finding in the bounded runtime projection/admission paths and new regressions. Source-only review; main separately checked the diff and executed the tests above. The malformed-credential route cases establish no-send admission refusal; exact sanitized 503 is established by the separate identity-projection failure cases.
The round-2 final review and its completion claim are superseded by this round's two reproduced findings and their fixes. Hosted checks for this pushed exact head are separate evidence.
NOT RUN: full suite (bun run test / bare bun test), changed suite, actual external-provider traffic, macOS/Linux runtime or permissions checks, docs-site build, local CodeQL and cross-platform workflow dispatch. No merge, rebase, force-push or Git configuration change was performed.

New Features
Pool-selected Codex accounts use a stable installation identity tied to the account and OpenCodex home. It persists across token refreshes and changes when the selected account changes. Existing identity values are replaced, while absent values remain absent; Direct mode and native main accounts retain caller-provided values.
Bug Fixes
Requests return a retryable 503 if the identity cannot be safely resolved. Pool turn metadata over 32 KiB is refused before sending; oversized WebSocket metadata closes the connection.
Identity handling is consistent across supported request paths, including Responses, compact, live sessions, search, vision, and context history.
Documentation
Added guidance on identity behavior and failure handling.

Walkthrough
Pool requests now use an installation identity derived from the selected account and OpenCodex home. The change applies it to existing header and client-metadata fields, adds secret validation and failure handling, and covers refresh, failover, live sessions, WebSocket frames, and related request paths.

Secret storage and identity derivation src/codex/installation-id.ts, src/codex/account-store.ts, src/lib/windows-secret-acl.ts, tests/codex-integration/codex-installation-{acl,id,invalid-account}.test.ts
Auth contexts and request builders apply account identity to existing Pool header and client-metadata fields. Missing fields remain absent. Header merging removes configured identity, turn-metadata, and attestation values before applying selected values.
Live sessions and WebSocket frames src/server/audio-dictation.ts, src/server/audio-live.ts, src/server/index/{live-sideband,serve-options,websocket-handler}.ts, src/server/live.ts, tests/codex-integration/codex-installation-live-{http,ws}.test.ts, tests/helpers/codex-installation-live.ts
Live requests and WebSocket frames carry authentication context for Pool metadata isolation. Tests cover immediate and queued frames, metadata size limits, and identity failures.
Relays and adapter-build paths handle identity-resolution failures. Context-history requests isolate Pool metadata in prepared bodies. Tests check refusal responses and the optional Claude-sidecar fallback.
Responses retries and failover src/server/responses/{compact,core-codex-account,passthrough-dispatch}.ts, tests/responses/codex-installation-{failover,compact-empty,turn-limit}.test.ts
Responses and compact retries preserve identity on same-account refreshes and replace it when failover selects another account. Identity failures during retry return transport responses after cleanup. Tests cover failover and turn-metadata size limits.

lidge-jun/opencodex#7568 — The issue concerns Pool traffic attribution to the caller-owned main account. This change applies account-specific identity to Pool metadata, but does not establish that the billing coupling is fixed.
lidge-jun/opencodex#8050 — The issue identifies a failure when turn metadata contains non-ASCII workspace paths. The changed rewriting path still uses JSON serialization, and the summaries do not indicate that this symptom is fixed.
lidge-jun/opencodex#7815 — The issue concerns overlapping config-root ACL hardening. This change adds secret-file hardening and handling for pending Windows ACL work, but the issue also describes writers outside this change.

Apply the selected-header merge to search requests.
If upstream.provider.headers contains x-codex-installation-id and the caller omits that header, Line 201 does not replace it. The search relay then sends the same configured installation ID for every selected Pool account. This defeats the account-isolation contract.
Use mergeCodexSelectedHeaders from src/codex/forward-transport-headers.ts when assembling search headers. It removes configured identity headers that are absent from the selected headers. (raw.githubusercontent.com)

Centralize the installation-ID 503 response.
The same formatErrorResponse(503, "server_busy", error.message) mapping now appears in codex-auth-error.ts, audio-upstream.ts, context-history.ts, images.ts, live.ts and search.ts. The docs describe this failure as a "retryable 503". None of the sites sets Retry-After, while nativeMainRefreshFailureResponse in this file does. Add one exported helper here, for example codexInstallationIdUnavailableResponse(error), and call it from each site. A later policy change, such as a Retry-After header, then happens in one place.

Reviewed against the current code. The sites already produce the same sanitized 503/server_busy mapping, and no installation-identity response contract specifies a Retry-After value. Failures include corrupt persistent material and actual ACL denial, so there is no established recovery interval to advertise. Centralizing identical formatting is optional refactoring rather than a demonstrated behavior defect; keeping this fix round focused avoids introducing additional cross-surface coupling. Leaving this discussion open for the author's preference.

@lidge-jun Agreed. The mapping in src/server/responses/codex-auth-error.ts, Lines 82–84, does not show a behavior defect. A retryable 503 does not require a fixed Retry-After value, and these failures have no established recovery interval.
The shared helper is optional refactoring. I withdraw the change request and will leave the discussion open for the author’s preference.

Preserve an empty installation header during routed compact handoff.
The truthy check at src/server/responses/compact.ts:1749 drops an existing empty x-codex-installation-id before internalReq is built. Pool header isolation only derives the account installation ID when that header is present. Preserve non-null installation and turn-metadata values here, and add a routed-compact regression for an empty installation header.

…d account
Replays the ID-isolation parts of three upstream commits:
- e04aae0 (lidge-jun#7838): Anthropic metadata device_id is derived per account
UUID, and the Claude Code session id gains a per-conversation key, so
pooled accounts no longer share one device and every conversation on a
token no longer shares one session.
- ba880df (lidge-jun#7840): x-codex-turn-state (header and client_metadata) is
scrubbed when a request moves to another account.
- 007dbfd (lidge-jun#8065): pool accounts send an installation ID derived as
HMAC(secret, account) from a private 32-byte codex-installation-secret
in the config home, across Responses HTTP/WS, compact, context history,
search, images, live, audio and the vision/web-search sidecars. Direct
and main accounts keep the caller's values. If the secret cannot be
read or created safely, pool sends return 503 instead of falling back
to the caller's ID. A blank chatgptAccountId is no longer a credential.
Deferred parts (Claude Code 2.1.280 profile, JSON refresh body, login
originator, version header) are recorded in docs/fork/UPSTREAM.md.
