Summary

  • Carry Harsha Vardhan's provider cache-affinity feature and retain its owner-approved decisions: default auto applies to key-auth pools with 2+ entries; manually choosing an active key leaves existing bindings intact. Eligibility, cooldown and pacing still outrank affinity; OAuth and forward routes keep their existing policy.
  • Fix the two CodeRabbit findings from 2026-10-28T21:59Z in 41a9967514ae0603d4baba094e3ad9a9dbf3757f: an HTTP-200 structured failed result cannot create/refresh affinity, and native compact pacing/budget plus both adapter-dispatch budget refusals carry synthetic attribution so they preserve a healthy binding. Upstream matching-provider/entry refusal release remains intact.
  • Preserve contributor commits and earlier recovery/release fix ceb49025fdc09af276632a280ebb621d3283c8da.
  • Add seven regression rows and synchronize the transport contract. 85c43019f20f1e7bf5371fe60ac571e78ff98148 applies the repository's named real-server budget to the key-failover suite after observed five-second timeouts. All assertions, cases and iteration counts are retained.
  • Delta source review scope: only src/server/request-log-cache-affinity.ts, src/server/responses/compact.ts, and src/server/responses/adapter-dispatch.ts (five changed source lines). No new credential, log field, persistence, configuration default or manual-selection behavior.
  • 2026-10-31 fix rounds: all five CodeRabbit threads raised across the 6e8f1efe0 and 569daec8f heads are fixed, replied to with SHAs, and resolved — 0 unresolved threads. 0438ee8a5: observedTerminalStatus cleared in finishFailedPolicyAttempt, and the four lane derivations aligned on the x-opencode-session fallback (5+/4−). 569daec8f: the e2e afterEach finishes its cleanup when a tracked server stop rejects, collecting failures and throwing an AggregateError (3+/1−). 7f98a2455: terminalSource also cleared on a policy hop so a synthetic 429 cannot suppress a genuine upstream release, and the four lane derivations unified into one affinityLaneFromRequest helper in request-log-conversation.ts so the fallback order and Hermes guard exist in exactly one place (29+/8−, 6 files). Branch merged onto dev tip 5c45797e6+ (0 behind).

Verification

Final head: 7f98a2455 (0 behind dev).

2026-10-31 validation (head 7f98a2455, macOS arm64):

  • bun run typecheck: pass (2,261 files).
  • Focused cone (cache-affinity, cache-affinity-e2e, key-failover, routing-policy-fallback, routing-policy-dispatch-scope, compact-key-selection): 189 pass / 0 fail, 985 assertions.
  • bun scripts/file-size-ratchet.ts: pass. bun run structure:check: pass. bun run privacy:scan: pass.
  • All review-fix diffs are minimal by construction (5+/4−, 3+/1−, 29+/8−); formatter churn was reverted and re-applied as isolated edits, keeping every touched file under its ratchet threshold.
  • NOT RUN on this update: full bun run test, GUI suite (no GUI change), docs-site build (no docs delta). Broader coverage left to CI.

Prior head records (569daec8f, 6e8f1efe0, 85c43019f) are retained in PR history: e2e 37/37 and cone 123/123 at 569daec8f; 336/336 focused cone at 6e8f1efe0; 337 unique focused cases with every failed case revalidated at 85c43019f; red proofs for each carried fix.

  • Security: original security review PASS, sponsorship, and owner decisions are retained. The review-fix commits touch candidate-scoped log-field cleanup, lane-derivation consolidation, and a test teardown — no new credential, log field, persistence or configuration surface. The terminalSource fix restores the release semantics the security review already covered.
  • Full-suite exception: shared-host resource exception per AGENTS.md; focused regression/selection/failover/policy/sweeper/layout coverage replaces a local full backend run. Broader/platform coverage is left to CI/coordinator.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. (Original PASS and sponsorship recorded; final-head delta recheck remains with the coordinator.)

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Walkthrough

The PR adds configurable, process-local cache affinity for API-key pools. It connects conversation lanes to successful serving credentials, applies eligibility and retention rules during selection, and records outcomes in request logs. It also adds tests and reference material, including a separate account auto-pause reference page.

Changes: Provider Cache Affinity

Layer / File(s) Summary
Affinity contract and store
src/types/provider.ts, src/config/schema/leaf-validators.ts, src/providers/cache-affinity.ts, src/lib/state-store-*, src/providers/model-rename-fields.ts, src/server/auth-cors.ts, src/types.ts
Adds four provider modes and a process-local binding store. Bindings use provider, model, and key-entry identity. The store enforces cache thresholds, retention, capacity, expiry, and configuration reconciliation. The provider editor accepts the setting.
Request selection and lane wiring
src/providers/api-key-selection.ts, src/providers/key-failover.ts, src/providers/label.ts, src/server/chat-native.ts, src/server/messages-native.ts, src/server/responses/*, src/server/management/provider-routes.ts
Request paths derive affinity lanes and pass them into proactive key selection. Eligible bound entries can serve request-locally; cooldown, pacing, and pool-entry eligibility remain selection constraints. Provider updates preserve an omitted affinity mode.
Finalization and persisted outcomes
src/server/request-log-cache-affinity.ts, src/server/request-log.ts, src/usage/log.ts, src/server/responses/policy-fallback.ts, src/server/responses/adapter-dispatch.ts
Finalization records eligible successful turns and releases bindings for specified upstream failures. Validated outcomes and reasons are added to request logs and persisted usage entries. Local send-budget refusals are marked synthetic, and failed policy attempts clear affinity metadata.
Behavior validation and reference material
tests/providers/cache-affinity.test.ts, tests/server/cache-affinity-e2e.test.ts, tests/routing/routing-policy-fallback.test.ts, tests/oauth/state-store-sweeper.test.ts, tests/fixtures/test-layout-expected.json, scripts/test-layout/layout.json, docs-site/src/content/docs/reference/configuration/providers.md, structure/*, readme/*, readme/i18n-manifest.json, devlog/_plan/261019_cache_affinity/000_spec.md
Adds unit and end-to-end coverage for binding rules, selection, request paths, failure handling, and configuration updates. Updates reference material and the draft specification, registers tests, and changes the documented state-store count from 31 to 32.

Status from the author side: the readiness checklist is complete (4/4, as the gate comment above now shows), local validation is documented in the description, and the branch is on the current dev tip.

The only remaining blocker is unsponsored_surface: this PR touches src/server/auth-cors.ts with exactly one line — registering the new cacheAffinity field in the provider field-policy map (cacheAffinity: "editor", alongside the existing promptCacheKey entry; the map is exhaustive over provider fields, so the field cannot land without it). Per MAINTAINERS.md that needs a maintainer security review and the maintainer-sponsored label; I can't apply that myself. The credential-selection changes themselves store and log pool entry ids only — no key material — and are described in the Summary's security note.

The test workflows are also still action_required (fork PR), so no suite has run in CI yet. Happy to adjust anything on review — the design record is in devlog/_plan/261019_cache_affinity/000_spec.md if the security reviewer wants the reasoning first.

Planning record for keeping a conversation on the key-pool credential
that served its last turn, so that credential's upstream prompt cache is
read again instead of rebuilt on another key. Verified viable against
the current seams (session lane, proactive credential selection,
attempt finalization, state-store sweeper) before implementation.
… server seams

Store semantics (worthiness, cold windows, silent-cache and reads-only
rules, modes, detected continuations only, reconcile, bounded
eviction), the proactive-selection preference (eligible bound entry
returned, cooling or pacing-limited entry falls through, removed
entry releases, pin survives other conversations' rotations, a 429 on
the pinned key still rotates), and end-to-end conversations across the
Responses, Chat, Messages, and Compact paths that stay on the
credential that warmed their cache. Red until the implementation lands.
…gistration

New provider cache-affinity store: per-lane bindings to the key-pool
entry that served the last turn, with measured worthiness (1024 cached
tokens), warm windows (5m, 1h after long-retention writes), the
silent-cache rule for vendors that report no write counter, and
session/turn/auto/off modes. Process-local and bounded (512 lanes,
per-entry and total byte caps, 24h hard expiry), registered with the
state-store sweeper; generation reconciliation drops bindings for
removed or disabled providers and removed credentials (the generation
context now carries both). The mode is validated at config load and
exposed through the provider field policy. README store counts follow.
selectProactiveApiKeyTransport — the one funnel the Responses, Chat,
Messages, and Compact paths share — first consults affinity: an
eligible bound entry is preferred as a request-local pick, rebuilt
through the same transport seam as rotation and marked cacheAffinity
on its captured selection. Selection re-reads keep such a pick while
the entry stays live, deliberately unfenced on the selection revision
(that revision bumps on every committed change, automatic rotation
included, so it cannot distinguish an operator re-selection from
another conversation's rotation — and a 429 on the pinned key still
rotates to the spare). A manual re-selection does not supersede a
live binding either; cacheAffinity off, or disabling the entry, is
the operator override. A removed or disabled entry releases the
binding; a cooling or pacing-limited entry (per-key concurrency full,
or its next permitted start beyond the send queue's tolerated wait)
falls through to ordinary selection. The committed provider.apiKey
is never written.

Call sites pass the session lane (with the normalized Messages
conversation identity as fallback) and whether the request is a
detected tool-result continuation; combo children inherit the lane so
affinity operates inside the chosen target.
Attempt finalization is the one seam every request passes with its
terminal status, the credential that ultimately served (entry id
captured beside the account label at each stamp, rotations included),
and cache usage normalized from whichever adapter answered. Recording
is gated on affinity actually applying to the provider, reuses the
consult-time model identity so virtual models match, and binds on
success; a quota/auth terminal — or any failure that left the serving
entry cooling, which catches classified credit refusals — releases it.
The logged outcome attributes rotations honestly (kept only when the
kept entry is the one that served). The outcome persists on the
request-log row as cacheAffinity/cacheAffinityReason with bounded
vocabularies.
Provider reference row (modes, eligibility precedence, process-local
bindings, manual-selection semantics, log fields) and the structure
SSOT for API-key pools: the affinity consult at the shared selection
seam, the request-local pick contract, and recording at attempt
finalization.

Pushed the review-fix commit and rebased onto current dev:

  • Head: a49ed47257690991208841f4e2bd078cd2fc3597 (base 8a3be7716, 0 behind at push time)
  • Marked this PR ready for review; reviewers are requested.
  • Post-rebase local validation: bun run typecheck pass; focused affinity/key-failover/policy tests 192 pass / 0 fail; bun run structure:check pass; bun run privacy:scan pass.
  • The PR description now documents the review fixes and the local full-suite exception in detail.

Remaining external gates are unchanged: unsponsored_surface still needs maintainer security review / maintainer-sponsored for the one-line auth-cors.ts field registration, and fork test workflows remain action_required until a maintainer approves them.

Maintainer triage (2026-10-28, against dev 4bf5728):

General API-key-pool cache affinity is not on dev yet. The dispatch/fallback follow-up, both test registrations, the field-map entries, and the 32-store documentation count are present.

  1. Address the two open functional review threads: src/server/request-log-cache-affinity.ts (lines 41-50) should keep bindings on proxy-generated refusals, and src/providers/cache-affinity.ts (lines 180-181) should scope release to the serving provider and entry. Add a focused regression for each.
  2. The new cacheAffinity row in docs-site/src/content/docs/reference/configuration/providers.md makes the merged document exactly 2,000 lines, which fails the file-size gate. Move a subsection into a linked sibling page (as docs: keep the provider configuration reference under the file-size cap #7856 did); baselines cannot be raised.
  3. Mark devlog/_plan/261019_cache_affinity/000_spec.md as historical and link the current structure contract, closing the remaining documentation thread.
  4. Update onto current dev, refresh validation, resolve the valid review threads, and re-attest readiness (the description currently has 0/4).

Because the PR changes src/server/auth-cors.ts (a restricted surface), it stays in draft until a maintainer completes the security review and decides on sponsorship. Thank you.

Carried the 2026-10-28 triage items onto this branch as two ordinary commits on top of your a49ed4725 (your history unchanged, Co-authored-by credit kept):

  • 1b2257f90 fixes the two open functional threads, each with a regression that failed first (44 pass / 7 fail before the fix):
    • src/server/request-log-cache-affinity.ts: a proxy-generated refusal no longer releases a healthy binding; only an upstream outcome can.
    • src/providers/cache-affinity.ts: release is scoped to the serving provider and entry, so a failure on one provider or key no longer drops another's binding (with the matching call sites in key-failover.ts, chat-native.ts and messages-native.ts).
    • The cache-affinity subsection moved out of reference/configuration/providers.md (now 1,965 lines, under the 2,000-line gate) into the linked sibling page reference/configuration/account-auto-pause.md, with the locale tables linking to it; caps are unchanged.
    • devlog/_plan/261019_cache_affinity/000_spec.md is marked historical and links the current structure contract (structure/transports/inventory.md, structure/transports/responses.md).
  • 9f593b2f5 merges current dev.

Local, Bun 1.4.4: cache-affinity, cache-affinity-e2e, routing-policy-fallback, server-key-failover-e2e, state-store-sweeper, both test-layout tests and the file-size ratchet: 232 pass, 0 fail. bun run typecheck, structure:check, privacy:scan and the docs-site build pass. bun run test:changed exceeded the local 900 s limit on a shared host, so that coverage is left to PR CI, which I approved for this head after checking the diff touches no workflow, dependency or release file. The carry does not change src/server/auth-cors.ts.

Still needed: the maintainer security review and sponsorship decision for the auth-cors.ts change, and your readiness re-attestation (currently 0/4).

Independent review (round 1) of the carry at 9f593b2f5. Both carried fixes hold (each regression fails against the earlier implementation and passes now; 172 focused tests pass), the merge keeps dev's wording, and providers.md is at 1,965 lines. 79962885b adds the moved Account Auto-pause page to the configuration sidebar, which the review noted was missing. Two items remain in the original affinity design, so I am leaving them to you rather than changing the contract:

  1. [P2] src/providers/key-failover.ts:646 — affinity breaks request-local round-robin failover. With requestPacing.keySelection: "round-robin", an affinity hit returns a selection marked only cacheAffinity. A later 429 then enters rotateKeyAfterFailure's persisted-selection branch and moves the saved active key from A to B, whereas without a binding the distributed branch keeps A. The reviewer reproduced it with a scratch regression (unbound control passes, bound case fails). Please keep request-local recovery semantics for affinity selections under round-robin, including the liveness fencing, and add that regression.
  2. [P3] src/server/request-log-cache-affinity.ts:51 — the log can report released when nothing was released. Since release is now scoped to the serving provider and entry, a refusal finalized on entry B leaves a binding on A in place, yet the log still says released. Returning a boolean from releaseProviderCacheAffinity and logging only on an actual release is the obvious fix, but it changes what cache-affinity-e2e.test.ts ("when every key fails, the binding is released…") asserts: in that scenario the binding on key A is cooled, not released, and the next turn still binds to B. Please decide whether a request whose bound entry failed over should release that binding (only for the refusal statuses) or report a different outcome, and align the test name and assertion with it.

src/server/auth-cors.ts only adds the non-secret cacheAffinity editor field; no concrete concern was found there, but it still needs the maintainer's security review.

Maintainer security review at 79962885b (independent review, read-only): PASS, 0 blocking findings.

  • Credential selection keeps live pool/key/destination checks before dispatch; affinity state is process-local and bounded; new persisted fields are allowlisted enums.
  • Management writes keep authentication; the new setting is validated and editor-safe. No new credential destination, OAuth change, dependency or workflow change.

Applied maintainer-sponsored. Exact-head CI run 40666635842 already succeeded. Remaining before merge: owner decisions on (1) default-on auto affinity for existing pools with 2+ keys, including providers without measured cache writes, and (2) manual active-key selection leaving existing bindings in place.

Exclude structured failed Responses results from the HTTP 2xx affinity
recording branch. Mark native compact pacing/budget refusals and both
adapter-dispatch send-budget fallbacks synthetic before finalization.
Keep upstream refusal release and owner-approved affinity defaults intact.

Add seven regression rows, including native compact URL/send-count checks,
and retain completed-turn controls. Budget real-server fixture setup with
the existing SERVER_BUDGET_MS, matching teardown. Synchronize the transport
contract without increasing the structure or file-size budgets.

F2 carry complete at 85c4301.

  • Ordinary dev merge: 3c272ec58112d254512e156d828d073eb2998631, dev e7b8cdfdaf16e8ec3fdb5333a655207a09dc70c3.
  • Behavior fix: 41a9967514ae0603d4baba094e3ad9a9dbf3757f excludes HTTP-200 structured failed results from affinity recording and marks compact pacing/budget plus both adapter-dispatch budget refusals synthetic. Default-auto and manual active-key semantics are unchanged.
  • Test fixture correction: 85c43019f20f1e7bf5371fe60ac571e78ff98148 uses the existing 30-second SERVER_BUDGET_MS for real key-failover server tests. Their observed >5-second work no longer dies at the implicit five-second deadline. No assertion, iteration or test was removed.
  • Red: bun run test --parallel=1 tests/server/cache-affinity-e2e.test.ts -t 'structured failure|local refusal preserves affinity' was 0/7 before production changes. With the native compact fixture corrected and compact fix removed, bun run test --parallel=1 tests/server/cache-affinity-e2e.test.ts -t 'compact-.*local refusal preserves affinity' was 0/3. The completed-result controls remain.
  • Green: all seven new regression rows passed in the 12-file focused command documented in Verification. That run was 316 pass / 21 fail, with all failures confined to existing server-key-failover five-second timeouts; 272 tests in the other 11 files passed. After the named-budget fix, bun run test --parallel=1 tests/server/server-key-failover-e2e.test.ts was 65 pass / 0 fail, 469 assertions (254.69s). This gives 337 unique passing focused cases, with every failed case revalidated; the earlier combined run itself was not green.
  • bun run typecheck: PASS, 2,230 files. bun run structure:index, bun run structure:check, bun scripts/file-size-ratchet.ts, bun run privacy:scan, and git diff --check: PASS. Lockfiles and generated index unchanged; no cap increased; layout.json is 1,999 lines.
  • Both new threads failed-result finalization and synthetic refusal attribution are replied to and resolved. Final GraphQL inspection: 0 unresolved threads.
  • Coordinator delta security scope is only the five changed source lines in src/server/request-log-cache-affinity.ts, src/server/responses/compact.ts, and src/server/responses/adapter-dispatch.ts. Independent final-head delta security approval and successful exact-head CI remain prerequisites for merge.

NOT RUN: full backend suite (shared Windows host/resource exception; broader coverage left to CI), import-graph suite, GUI suite/build/lint, live provider smoke, final carry docs-site build, workflow dispatch, independent final-head delta security review. No merge, rebase, force-push, label change, CI approval or workflow rerun performed.

Maintainer review (lane L4, account pool and quota bug sweep), against dev 3ea8965 and head 85c4301. This lane is skipping it. The reasons are scope; the design is not rejected.

  • It is a feature without a linked bug, and this lane is limited to pool and quota bug fixes.
  • It changes five src/server/responses/** transport files (adapter-dispatch.ts, compact.ts, core-combo.ts, policy-fallback.ts, request-transport.ts), which another maintenance lane owns.
  • git merge-tree against current dev reports content conflicts in docs-site/astro.config.mjs and readme/i18n-manifest.json. Source files auto-merge.
  • The selection policy change needs an independent security review before merge: affinity survives selection-revision changes and can keep using a previously selected key after a manual selection (src/providers/api-key-selection.ts).

The safeguards look careful: OAuth and forward routes are excluded, and affinity is bounded by mode, provider, model and age, with availability, cooldown and pacing checks. Before review, it needs the conflicts resolved, the docs and translation hashes refreshed, and green required CI on that head.

Resolve the three conflicts lidge-jun's 2026-10-30 pre-review list named:

- docs-site/astro.config.mjs: keep both sidebar rows — our Account
Auto-pause page and dev's new Catalog page.
- structure/gui-and-management-api.md: take dev's evolved Updates row
(dead-PID status reporting) and ten-setting Providers row
(promoteAdditionalTools), re-inserting our cacheAffinity carry sentence.
- readme/i18n-manifest.json: stamp all seven locales with the merged
  README.md LF-normalized hash; the translation-parity guard passes (53/53)
confirming the locale token streams still match.

Addressed the pre-review items from the 2026-10-30 lane review (issuecomment-6521245500):

  • Conflicts resolved — new head 6e8f1efe0 merges dev through tip c618c77b1 (0 behind). docs-site/astro.config.mjs keeps both sidebar rows (our Account Auto-pause page and dev's new Catalog page); structure/gui-and-management-api.md takes dev's evolved Updates/Providers rows with the cacheAffinity carry sentence re-inserted.
  • Docs and translation hashes refreshed — all seven sourceSha256 values re-stamped against the merged README; tests/ci-workflows/docs-readme-translation-parity.test.ts passes 53/53, confirming every locale's token stream still matches.
  • Readiness re-attested — all four boxes ticked in the description; local validation for the update commits is documented there (336/336 focused cone, typecheck, structure, privacy, ratchet, parity guard). The update commits change docs/config files only; the affinity source delta and its recorded security scope are unchanged.

Remaining external: green required CI on 6e8f1efe0 (fork runs still need approval), and the independent selection-policy security review named in the lane comment. Ready for review.

Mark passthrough-local refusals as synthetic before finalization.

preparePassthroughExchange returns local refusals directly to executePassthroughResponse, which returns them before deliverPassthroughResponse runs. The delivery callbacks cannot add synthetic attribution.

The pacing branch returns a 429 without setting logCtx.terminalSource. The ordinary send-budget fallback also returns 429 when unboundPoolSpendRefusalResponse has no unbound-pool history. The request-log finalizer can then release a matching provider/entry affinity binding as if the provider refused the request.

Comment thread src/server/responses/policy-fallback.ts Resolved
…only lanes

Two CodeRabbit findings on head 6e8f1ef:

- policy-fallback: finishFailedPolicyAttempt now clears observedTerminalStatus
alongside the other candidate-specific terminal fields. A buffered
response.failed from a failed candidate otherwise suppressed cache-affinity
recording on the fallback candidate that succeeded (the recorder refuses
status<300 only when observedTerminalStatus is 'failed') and could surface a
stale terminal through the relay's eof finalization.
- request-transport/chat-native/messages-native/compact: the affinity lane
fallback now honors a request carrying only x-opencode-session, matching the
  explicit-lane treatment claude-messages.ts and getOrAllocateRequestSessionLane
already give that header. Previously such requests derived no lane and never
  bound or consulted affinity.

The x-opencode-session check sits between sessionLaneIdFromRequest and the
conversation-id fallback, so the existing Hermes guard and conversation-id
behavior are unchanged.
CodeRabbit finding (PRRT_kwDOS-0Gi86xXeqH): a rejected server.stop(true) in
the afterEach aborted the loop before the remaining stops and before
globalThis.fetch, keychain factory, cooldowns, affinity state, OPENCODEX_HOME
and testDir were restored, so one stop failure leaked shared state into later
tests. Collect stop failures, run every cleanup step, then rethrow them as an
AggregateError.

All three CodeRabbit threads from the 6e8f1efe0 review are now fixed, replied to with SHAs, and resolved — 0 unresolved threads.

  • 0438ee8a5 — the two code findings: observedTerminalStatus cleared in finishFailedPolicyAttempt (a failed candidate's buffered response.failed can no longer suppress affinity recording on the successful fallback candidate); all four lane derivations honor x-opencode-session-only requests (5+/4−).
  • 569daec8f — the e2e teardown finding: afterEach now completes every cleanup step when a tracked server stop rejects, collecting failures and throwing an AggregateError (3+/1−).
  • Branch merged onto dev tip 63de499e1 (0 behind; the v2.95.0 release and 2.96.0 bump landed between pushes).

Local validation on 569daec8f: e2e file 37/37, focused cone 123/123, typecheck (2,255 files), file-size ratchet, structure:check, privacy:scan — all pass. Readiness re-attested (4/4). Remaining external as before: required CI on this head (fork approval) and the independent selection-policy security review.

Mark passthrough budget refusals as synthetic.

A key-auth request can select and attribute an existing affinity entry before a local SendBudgetExhaustedError reaches transportFailureResponse. This branch returns a local HTTP 429 without calling markLocalRequestLogRefusal. The addFinalRequestLog seam then treats the response as a non-synthetic 429, releases the healthy affinity entry, and records the outcome as quota.

Call markLocalRequestLogRefusal(logCtx, "request_send_budget_exhausted") in this branch. Keep genuine upstream 429 handling unchanged; upstream responses do not enter this instanceof SendBudgetExhaustedError branch.

Follow-up to the current-head readiness note (569daec8ff6e5fb67b6b45c32199f6fffc947ef4). The earlier three findings are resolved, but the fresh thread snapshot now contains two new unresolved threads, so the older zero-thread count is no longer current.

I checked the functional one against source: src/server/responses/policy-fallback.ts:154-179 clears observedTerminalStatus and affinity fields but retains logCtx.terminalSource. src/server/request-log-cache-affinity.ts refuses release when that field is synthetic; a local synthetic failure on an earlier candidate can therefore suppress release for the next candidate's genuine upstream 401/402/429. Clear the candidate-owned terminal source when committing to a retry, while preserving the saved failureLog restoration if the next candidate is locally refused. Add a cross-candidate regression for both the dispatched and locally-refused-next cases. The shared lane-helper suggestion is the other thread; address it or record an evidence-based disposition through the normal readiness flow.

I am not treating these source checks, author validation or the review-ready label as completion of the separate selection-policy boundary review. No scan was launched and no sponsorship/approval/merge was granted here.

…ne derivation

Both CodeRabbit findings on head 569daec:

- finishFailedPolicyAttempt now also clears logCtx.terminalSource. A synthetic
local 429 on a failed candidate otherwise survived the hop and made
finalizeProviderCacheAffinity skip releasing a binding on a genuine upstream
401/402/429 from the next candidate. The refusal restoration path is
unchanged: the saved failureLog still carries that response when the next
  candidate is locally refused.
- The four inlined affinity-lane derivations (chat-native, messages-native,
compact, request-transport) move into one affinityLaneFromRequest helper in
  request-log-conversation.ts, beside sessionLaneIdFromRequest. The expression
already drifted once (three of four sites missed x-opencode-session until
PR lidge-jun#7738); the helper keeps the fallback order and the Hermes guard in one
  place.

Both CodeRabbit threads from the 569daec8f review are fixed in 7f98a2455 and resolved — 0 unresolved threads across the PR.

  • terminalSource is now cleared in finishFailedPolicyAttempt, so a synthetic local 429 from a failed candidate cannot suppress the release of a binding on a genuine upstream 401/402/429 from the next candidate. The saved-failureLog restoration path is unchanged.
  • The four affinity-lane derivations are unified into one affinityLaneFromRequest(headers, slot) helper beside sessionLaneIdFromRequest, eliminating the drift that produced the x-opencode-session gap in the first place.

Branch is on the current dev tip (5c45797e6, 0 behind). Local validation on 7f98a2455: focused cone 189 pass / 0 fail (including routing-policy-dispatch-scope and compact-key-selection), typecheck 2,261 files, file-size ratchet, structure:check, privacy:scan — all pass. P2 Badge Mark local passthrough refusals as synthetic before finalizing

For an openai-responses passthrough turn that has already sent once, a subsequent local pacing or send-budget refusal is converted to HTTP 429 by transportFailureResponse in src/server/responses/passthrough-dispatch.ts:926–943 without setting terminalSource. The earlier send has populated the serving credential identity, so this branch deletes the healthy cache-affinity binding and records a quota release even though the credential never refused the request. Mark those passthrough refusals as synthetic, as the native and adapter paths now do, and cover a locally refused passthrough retry.

P2 Badge Preserve long-retention warmth across read-only cache hits

After a lane writes an Anthropic one-hour cache, a successful cache hit can report cache reads but zero new one-hour writes. Replacing the binding here then resets longRetention to false, shortening its warmth to five minutes. For example, a one-hour write followed by a read-only hit one minute later is considered cold at ten minutes, even though the original cache is still warm; ordinary selection can then move the conversation to another credential and rebuild the cache. Preserve the existing long-retention expiry for the same provider, entry, and model instead of deriving it solely from the latest response's writes.
