Summary

An otherwise healthy model can remain skipped by a combo after its saved target cooldown outlives the actual upstream outage or quota window. When the other targets also fail, requests keep returning combo_unavailable without checking whether that target recovered.

This adds an opt-in cooldownReprobe setting per combo. It is disabled by default. When every otherwise eligible target is cooling, an initial non-preview request can reserve one new failover pass. The minimum interval between passes is configurable in milliseconds (default 300000; accepted range 1000–86400000).

ocx combo set review --cooldown-reprobe on --cooldown-reprobe-interval 300000
ocx combo set review --cooldown-reprobe off

Equivalent combo config:

{ "cooldownReprobe": { "enabled": true, "intervalMs": 300000 } }

The reservation is synchronous and shared per combo, including the wait-based selection path. Preview operations and in-flight failover exclusions cannot trigger another reset. New failures record cooldowns normally. Provider/account quota admission, compatibility checks, metered-spend consent, cancellation and existing dispatch budgets remain authoritative. There is no timer or background request. This deliberately permits an early retry of a server-advertised target cooldown only after explicit operator opt-in; it does not clear account-level quota limits. State resets on proxy restart or combo selection-state reset.

CLI receipt validation accepts the new field, and the management API preserves it when clients omit it during unrelated edits. User and architecture documentation are included. This PR adds configuration and CLI controls, not dashboard controls.

Verification

Latest follow-up at commit 8c849ed: recovery admission is checked before the encrypted-task fallback can reserve a cooldown reprobe. Non-spawn requests and disabled recovery leave target cooldowns and the interval reservation unchanged; the next admitted request can still recover immediately. Added both end-to-end regressions and reset selection state between test cases.

  • bun test --isolate --timeout 60000 tests/server/agent-task-recovery-combo.test.ts tests/server/agent-task-recovery-fallback.test.ts tests/codex-integration/combo-cooldown-reprobe.test.ts tests/server/server-combo-cooldown-recording.test.ts: 43 passed, 0 failed, 205 assertions.
  • TypeScript tsc --noEmit, structure SSOT, file-size ratchet and privacy scan: passed. Broader prior-commit coverage remains documented below.

Validated on Windows with pinned Bun 1.4.4 at commit 8770b73:

  • bun test --isolate --timeout 60000 over combo-cooldown-reprobe, combos, cli-combo-parity, file-size-ratchet, both layout/tooling guards, skill-ocx, agent-task-recovery-combo, agent-task-recovery-fallback, server-combo-reasoning-replay-eligibility, combo-metered-consent, and server-combo-cooldown-recording: 253 passed, 0 failed, 2152 assertions across 12 files. The initial run's sole failure was a repository file scan exceeding the default 5-second timeout; the 60-second run passed.
  • node node_modules/typescript/bin/tsc --noEmit, structure SSOT, file-size ratchet and privacy scan: passed.
  • Generated ocx skill surface check previously passed; these follow-up fixes do not change CLI capabilities.
  • Documentation build previously passed (593 pages and internal-link validation); subsequent edits are architecture documentation only.

Regression coverage includes throwing eligibility callbacks on cooled targets, preserving available siblings, scoped cooldown clearing, interval reservations, preview and in-flight guards, route-level reroutes, encrypted-task recovery with mandatory reasoning replay, and CLI/API persistence. Recovery bypasses only encrypted-payload readability; metering, JEV, reasoning-replay and protocol-lane constraints remain enforced in every encrypted-task recovery selection.

The full suite was not run under the documented resource exception. Changed-mode test selection previously stalled before starting tests and was stopped; no passing result is claimed for that run. Existing server failover E2E coverage passed 176 tests on the preceding commit; current related regression coverage is listed above.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs were updated for the new opt-in behavior.
  • No credential, authentication or workflow changes; the default stays off.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Summary by CodeRabbit

  • New Features
    • Combos can optionally retry eligible targets when all are cooling down. Recovery is off by default and runs on demand, with an interval from 1 second to 24 hours (5 minutes by default).
    • Configure recovery with the combo command-line options. Setting an interval alone does not enable it. Recovery creates no background traffic and remains subject to eligibility checks and retry limits.
  • Documentation
    • Clarified how cooldown recovery interacts with fallback policies, provider limits, and existing cooldown behavior.

Independent validation of 52a9328ebf7d8c0ab82f8edd154956221c6ac141 with official Bun 1.4.4, matching frozen dependencies, private homes and hard cgroups:

bun test --isolate tests/codex-integration/combo-cooldown-reprobe.test.ts tests/cli/cli-combo-parity.test.ts tests/ci-workflows/file-size-ratchet.test.ts -> 75 pass, 1 fail, 326 assertions.

Blocker — scripts/test-layout/layout.json:744: the new inline registration grows this map to 2000 lines, so the repository file-size ratchet rejects it as NEW_OVERSIZED. Move this new registration into an existing appropriate explicitShards map (or a properly registered new shard); keep the independent expected fixture registration. Do not add/increase a file-size baseline to clear it. Re-run the file-size and layout/tooling gates after the move.

The opt-in and persistence regressions passed; this finding is the actual gate failure, not a request to alter runtime cooldown/retry semantics. I approved the existing exact-head Cross-platform CI 40193151607 and Catalog async contracts 40193151732 after full diff/execution-surface inspection; their execution permission is not a CI pass or review approval. No live provider traffic, config mutation or Codex Security scan was run.

Hosted CI follow-up on the same head: job 121201042860 reproduces the 2000-line ratchet failure. A second independent blocker is confirmed by jobs 121201042839 and 121201042856: the capability registry changes leave the committed generated ocx skill surface stale. Run bun run skill:surface and commit its generated result, then bun run skill:surface:check plus tests/ci-workflows/skill-ocx.test.ts. These are assertion/generation failures; do not rerun them unchanged as flakiness.

Scope cooldown clearing to the reprobe targets.

reopenAllCooledTargets excludes disabled, provider-missing, quota-exhausted, and request-ineligible targets from usable, but clearComboTargetCooldowns(comboId) clears every target in the combo. An excluded target can lose its cooldown without participating in the reprobe pass. If it becomes eligible later, dispatch can occur before its configured cooldown expires.

Pass only the reprobe target keys to the clear helper. This is separate from route-level request-eligibility handling.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Disable cooldown reprobe for retryAfterCooldown.

After a failed target receives a cooldown, retryAfterCooldown calls pickWithWait without exclude. This reaches pickComboTargetWithWait with an empty exclusion set, so the enabled reprobe path can call clearComboTargetCooldowns before waitForCooldownMs expires. The single-target retry can therefore dispatch the failed target immediately and bypass its configured cooldown.

Add an explicit reprobe opt-out and use it only for this post-failure retry.

src/combos/resolve.ts:687-693
🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Disable cooldown reprobe for the route-level selector.

tryPickComboModel calls pickComboTarget(config, comboId, { preview }) with no request-specific eligible predicate. Because preview defaults to false and no exclusions are passed, this call can reach reopenAllCooledTargets. That function then checks only provider availability and clears the entire combo's cooldowns.

The core-combo path supplies metered, JEV, payload, reasoning-replay, and protocol eligibility. The route-level path does not. A route-level reprobe can therefore reopen targets that the current request will reject. Those targets are no longer protected by their configured cooldown when a later request can select them.

Scoping the clear to usable alone does not fix this call because its usable set still lacks request-specific eligibility. Add an explicit reprobe opt-out and disable it for tryPickComboModel, while preserving reprobe for callers that provide the required request context.

Maintainer integration decision for v2.93.0 release prep: MERGED into dev (squash; dev maintainer-integration path under MAINTAINERS.md).

Outstanding CHANGES_REQUESTED (Ingwannu, on 52a9328e): checked as addressed on head 8c849ed335e232a6d2d674925a0848c4bb536fa5.

  1. layout.json grew to 2000 lines (ratchet NEW_OVERSIZED). The registration moved into a new explicitShards map, scripts/test-layout/layout-combos.json (73a3cfc). The independent registration in tests/fixtures/test-layout-expected.json is kept, layout.json stays at 1999 lines, and no baseline was raised. The file-size ratchet and both layout guards pass.
  2. Stale generated ocx skill surface. skills/ocx/references/01_surface_agents-routing.md was regenerated (73a3cfc), and bun run skill:surface:check reports all nine generated files current.

The later CodeRabbit threads are fixed and resolved: scoped cooldown clearing to reprobe targets (62aa653), throwing eligibility for cooled targets, preserving an available sibling, route-level reroutes with reprobe disabled (8770b73), and admitting encrypted-task recovery before a cooldown-clearing reprobe (8c849ed). The review stated it was not a request to change runtime cooldown/retry semantics, and the default stays off.

  • Combined tree (this head merged with current origin/dev 8dd330a3f), Bun 1.4.4: combo-cooldown-reprobe, cli-combo-parity, agent-task-recovery-combo/-fallback, server-combo-cooldown-recording, combo-metered-consent, file-size-ratchet, both layout guards and skill-ocx: 156 pass / 0 fail. tsc --noEmit, structure:check, skill:surface:check and privacy:scan pass.
  • Diff review: reprobe is gated on cooldownReprobe.enabled; preview, in-flight, wait-path and metered-reroute picks pass allowCooldownReprobe: false; the management API keeps the field on unrelated edits. The one change for all users is the encrypted-task recovery pick, which now applies the metered/JEV/replay/protocol eligibility checks (a tightening).

NOT RUN: full local suite (resource exception; parallel release lanes), Windows/macOS jobs (not requested by the PR filter), live provider traffic.
