Summary

  • A drain past its deadline calls abortAndReleaseAllTurns, which aborts every controller bound to an admitted turn. trackStreamLifetime only stopped on a consumer cancel, and several callers pass a fresh AbortController that nothing else listens to: the adapter bridge (adapter-delivery.ts, run-turn-execution.ts), client-encoder delivery, policy refusals and the local SSE path in adapter-dispatch.ts. For those turns the abort did nothing: the tracked stream kept relaying and its source stayed open.
  • trackStreamLifetime now listens for its controller's abort and, once tracking has started, finishes the turn, cancels the source and errors the stream with the abort reason. The listener is attached after registerTurn, so the lease's own abort handling (release the turn, abort sibling controllers) still runs first. A controller that registerTurn aborts synchronously (a lease that has already settled) is not listened to, so that case keeps its current behaviour.
  • Only src/server/lifecycle.ts changes (+17/-1).

Verification

  • New test a drain abort ends the stream and cancels its source in tests/service/shutdown-drain.test.ts, run before the src change: 1 fail (Expected: "errored", Received: "still open" 200 ms after abortAndReleaseAllTurns). After: bun test tests/service/shutdown-drain.test.ts tests/responses/passthrough-abort.test.ts tests/codex-integration/active-registry-admission.test.ts -> 59 pass, 0 fail. The test also asserts the source is cancelled and onDone runs once.
  • bun x tsc --noEmit -> exit 0. bun scripts/file-size-ratchet.ts -> passed.
  • bun run test:changed (Bun 1.4.3 via OCX_TEST_RUNNER_BUN, OCX_TEST_MAIN_TIMEOUT_MS=2400000 because the 1480-file import graph of lifecycle.ts exceeds the default 900 s cap on this loaded machine) -> 33218 pass, 42 skip, 3 fail across 1480 files. tests/responses/chat-native-spend.test.ts failed after a 921 s stall with ECONNRESET and passes alone (14 pass, 0 fail). tests/codex-integration/codex-runtime.test.ts "treats missing persisted and resolved versions as the same selection" and tests/cli/cli-connect-readiness.test.ts "a rejected preferred runtime falls back without rewriting the saved selection" also fail on checkouts without this change on this machine (local-environment dependent) and are unrelated to stream tracking.
  • Full bun run test was not run locally: test:changed already covered 1480 files through the changed module's import graph; the remainder is left to CI.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed. (No docs change needed: shutdown-drain internals only.)
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. (No secrets or auth behaviour touched.)

Source: carried from the vadymhimself fork build (trackStreamLifetime in commit 8fb1bbb), re-authored against upstream's current lease model: the fork's tracksAdmission change and pre-aborted-controller handling are not carried.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Summary by CodeRabbit

  • Bug Fixes
    • Streams now respond to turn cancellation during shutdown: pending reads reject with the cancellation reason, the underlying stream is canceled, and completion is reported once. This ensures an open stream does not remain pending after its turn is aborted, and the stream’s completion callback is not invoked more than once.

A drain past its deadline calls abortAndReleaseAllTurns, which aborts every
controller bound to an admitted turn. trackStreamLifetime only stopped on a
consumer cancel, and several callers (the adapter bridge, run-turn execution,
client-encoder delivery, policy refusals, local SSE) pass a fresh
AbortController that nothing else listens to. For those turns the abort was a
no-op: the stream kept relaying upstream output and its source stayed open.

trackStreamLifetime now listens for the controller's abort and, once
tracking has started, finishes the turn, cancels the source and errors the
stream. The listener is attached after registerTurn so the lease's own abort
handling still runs first, and a controller registerTurn aborted
synchronously (a settled lease) keeps the previous behaviour.

📝 Walkthrough

trackStreamLifetime now handles turn-signal aborts by cancelling the source reader and erroring the tracked stream with the signal reason. A shutdown test verifies that the pending read rejects, the source is cancelled, and the completion callback runs once.

Changes: Stream lifetime

Layer / File(s) Summary
Abort handling and validation
src/server/lifecycle.ts, tests/service/shutdown-drain.test.ts
trackStreamLifetime stores the stream controller and adds a one-shot abort listener after turn registration. On abort, it cancels the source reader, errors the tracked stream with the signal reason, and finishes tracking. Finishing removes the listener. The test checks pending-read rejection, source cancellation, and one completion callback.

`onDone` callback throws. Add a regression case verifying that a throwing
`onDone` still cancels the reader and errors the stream.

Intake/scoped review at 4141315e5df8e1aa023ec8705eaff0f3c3b3b0f7.

The abort listener now cancels the source and errors the tracked reader before invoking completion cleanup. Please preserve exactly-once cleanup, pending-read cancellation and settled-lease behavior in the drain fixtures. src/server/lifecycle.ts is an owner-only CODEOWNERS path, so final owner approval remains required in addition to CI.

Current-head functional workflows are awaiting fork approval; that is pending execution, not a passing test result. No local suite was run for this branch in this intake. No workflow approval, new scan, live configuration change or merge was performed. Required review/readiness gates remain in force.
