Summary

Fixes #2999.

Codex owns CODEX_HOME/auth.json: it rewrites the file in place and serializes refreshes only inside its own process. When OpenCodex refreshed the native main login itself, it could race Codex, overwrite a newer login, or retire the grant. This PR lets Codex own that refresh whenever a supported Codex binary is installed.

New setting nativeMainRefresh: "codex" | "opencodex":

  • "codex" (the default when a supported binary is found): when the access token is stale, or upstream rejected it, OpenCodex starts a short-lived codex -c model_provider="openai" app-server under the same CODEX_HOME and calls getAuthStatus { includeToken: true, refreshToken: true }. It uses the returned authToken for the request and writes nothing. It never rereads auth.json afterwards, because Codex in keyring mode deletes that file. authMethod: "chatgpt" with a null authToken is the only permanent-failure signal on the wire, so it maps to reauth (the grant is marked rejected). An unreachable binary, a non-ChatGPT method, or the same/expired bearer is transient.
  • "opencodex", a missing binary, or a binary older than 0.130.0: the existing guarded writer, plus the in-flight-token behavior from fix(codex): serve the in-flight token when a native main save is refused #7035.

The binary is found by the existing runtime resolver (CODEX_CLI_PATH, the pinned runtime, real binaries behind the shim, PATH, and the ChatGPT.app bundle), and that resolver's --version probe is the version check. The 0.130.0 floor is conservative: from that release on, the handler withholds the token after a permanent refresh failure, and model_provider="openai" keeps authless Desktop injection (requires_openai_auth = false) from hiding the credential. The exchange runs only on the 30s refresh timeout, so caller cancellation cannot kill app-server mid-exchange. On timeout the call settles right away, and a child that ignores SIGTERM gets SIGKILL after 2s.

ocx doctor prints a Native main refresh section with the active mode, binary and version, or the reason it fell back. Auto resolution never spawns Codex while the test home guard is armed.

Known limits, unchanged by this PR: the spawned app-server is one more Codex refresher, and its in-process semaphore does not cover a Codex CLI or Desktop process that is already running, so a Codex-vs-Codex race is still possible (upstream AuthManager). Native main still requires auth.json to be present before any refresh; keyring-only logins were unsupported before and still are.

Verification

  • New tests/codex-integration/native-main-refresh-mode.test.ts (16 cases): mode resolution (floor, prerelease, missing, explicit opt-out never probes, explicit codex fallback, test guard); the JSON-RPC client against a fake codex executable (argv, CODEX_HOME, initialize → initialized → getAuthStatus order, error/early exit/hang/SIGTERM-ignoring child, missing binary); main account in codex mode (no write, no epoch bump, token used after auth.json is deleted mid-call, null token → reauth + rejected grant, transient classes, fresh token skips the RPC, opencodex mode never calls Codex); doctor lines.
  • Manually against the real binaries (codex-cli 0.162.1 and ChatGPT.app 0.155.0-alpha.9) with a throwaway CODEX_HOME: a fresh token is returned; a fake refresh grant comes back as authMethod: "chatgpt" with no token. A real login was not refreshed.
  • bun run src/cli/index.ts doctor on macOS prints ok codex: Codex refreshes the main login through app-server (…/codex (0.162.1); auto).
  • After rebasing onto dev with fix(codex): serve the in-flight token when a native main save is refused #7035 merged, focused: native-main-refresh-mode, codex-main-account-refresh, codex-main-grant-refusal, main-device-reauth, codex-model-entitlement-admission, lab/core-lab-boundary, doctor, repo-import-resolution, structure-ssot, test-layout guards, file-size ratchet: all pass (289 tests). The new test file is placed by the codex-integration regex seed. It is not in the explicit layout map because one more line pushes scripts/test-layout/layout.json to 2000 lines and trips the file-size ratchet.
  • bun run typecheck, bun run structure:check, bun run privacy:scan: pass.
  • I couldn't run the full local suite reliably: the disk is nearly full (an earlier attempt hit ENOSPC) and it contended with another worktree's suite. Exact-head CI is the gate.
  • Independent review (gpt-6.1-sol): its subprocess-timeout finding is fixed here. Its other two findings, the Codex-vs-Codex race and keyring follow-up calls, are the known limits above.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Summary by CodeRabbit

  • New Features
    • Added the nativeMainRefresh setting to choose whether Codex or OpenCodex handles refreshes for the native ChatGPT login. By default, a supported Codex version is selected; otherwise, OpenCodex is used.
    • ocx doctor now reports the active refresh mode and why it was selected.
  • Documentation
    • Added guidance on refresh modes, fallback behavior, and login recovery.

Review in Change Stack →

📝 Walkthrough

The change adds a nativeMainRefresh setting to select the native-main refresh owner. Supported Codex binaries can provide refreshed auth status without an auth.json write. The change also adds mode diagnostics, configuration and provider documentation, and integration tests.

Changes: Native-main refresh ownership

Layer / File(s) Summary
Refresh-mode configuration and diagnostics
src/types/config.ts, src/config/schema/config-schema.ts, src/codex/native-main-refresh-mode.ts, src/cli/doctor.ts, docs-site/src/content/docs/reference/configuration/server.md, structure/providers/openai-accounts.md, tests/codex-integration/native-main-refresh-mode.test.ts
Adds the nativeMainRefresh setting and resolves the refresh owner from the configured mode and Codex runtime. ocx doctor reports the selected owner and reason. Documentation and tests describe and verify the selection rules.
Codex app-server auth-status request
src/codex/native-main-refresh-mode.ts, tests/codex-integration/native-main-refresh-mode.test.ts
Adds an abort-aware getAuthStatus request, response parsing, and child-process shutdown. Tests cover request parameters, response handling, and request failures.
Main-account refresh integration
src/codex/main-account.ts, tests/codex-integration/native-main-refresh-mode.test.ts
Routes Codex-mode refreshes through the app-server and uses a fresh returned token without rereading or writing auth.json. Tests cover token and failure handling, fresh on-disk tokens, and the OpenCodex guarded-writer path.

Full details: Linked Issues check

Explanation

Active directly linked issue [#2999] requires native-main refresh to coordinate by canonical CODEX_HOME, preserve an external Codex writer atomically, recover deterministically after process death, and never remove the canonical auth.json target during publication. The PR does not implement those requirements. src/codex/native-main-refresh-mode.ts delegates to a separate short-lived Codex process, but its module comment and the PR description state that Codex serializes refreshes only inside one process and that Codex-vs-Codex races remain. Two OpenCodex instances can therefore start separate Codex refreshers for the same CODEX_HOME. The opencodex path in src/codex/main-account.ts still uses the existing atomicWriteFile guarded writer, whose final validation remains vulnerable to a replacement after the check and before rename. The added tests in tests/codex-integration/native-main-refresh-mode.test.ts cover mode selection and the app-server exchange, but they do not establish the linked issue's cross-process single-refresh, external-writer preservation, process-death recovery, or canonical-target guarantees.

Resolution

Implement a cross-process lock or claim keyed by canonical CODEX_HOME, and use it for every native-main refresh path. Make publication revalidate and replace the canonical file through a protocol that cannot overwrite an external writer after the final check. Ensure crash recovery removes only stale coordination state and does not remove CODEX_HOME/auth.json. Add regression tests for two different OPENCODEX_HOME values sharing one CODEX_HOME, an external replacement at the final publication point, process death and recovery, and stable auth.json target presence.

* auth.json is not read again after the call. In keyring mode Codex deletes the file once the
* keyring save succeeds, so the RPC answer is the only place the new token exists for us.

P1 Badge Preserve keyring credentials beyond the delegated refresh

When Codex stores the login in the OS keyring, auth.json may already be absent or may be deleted during this call as described here. The returned bearer serves only the current request; the next resolveMainAccountToken invocation returns at if (!initial) before resolving Codex mode, so it never queries app-server and the native main account becomes unavailable. Retain enough delegated credential state or query Codex when the file is absent, and cover a second request after the keyring migration.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Doctor resolves the Codex runtime a second time with a full loadConfig().

Line 1436 calls loadConfig(). Everywhere else, doctor reads config through readConfigDiagnostics(), which has no side effects. loadConfig() hardens the config directory and the secret files, and it can back up an invalid config. ocx doctor is documented as read-only, so this call must not mutate state. Pass doctorConfig (Line 1368) to resolveNativeMainRefreshMode instead.

Proposed fix
-    const { loadConfig } = await import("../config");
     const { formatNativeMainRefreshDoctorLines, resolveNativeMainRefreshMode } = await import("../codex/native-main-refresh-mode");
-    const resolution = await resolveNativeMainRefreshMode(loadConfig());
+    const resolution = await resolveNativeMainRefreshMode(doctorConfig);

As per path instructions, ocx doctor "is read-only; it prints diagnostics and repair hints but does not apply repairs."

…nd a reliable watcher (lidge-jun#2999)

All OpenCodex code reads the native-main ChatGPT credentials from one
in-memory store instead of CODEX_HOME/auth.json. One module writes the
file (refresh, device reauth, profile switch, rollback). Only the store
observes the file: a directory watcher, a short stat timer and a forced
check before every refresh and write.

- Adopt only complete credentials; empty, partial or malformed content
  is a hiccup that keeps memory and blocks writes.
- A missing file is a logout only after a grace period; no refresh or
  write during it.
- Writes use temp file + rename and re-observe before the rename; an
  external change wins over the pending write.
- A rotated refresh token is written right away; refresh and write stay
  under the existing CODEX_HOME claim.
- Consumers that rejected a symlinked auth.json on base still do.

Accepted risk: a Codex write that lands within the same split second as
OpenCodex's rename can still be lost (needs a lock Codex also honors).

Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>

This builds on dev 4ae4936, which already contains lidge-jun#7048 and lidge-jun#7035. "codex" mode is unchanged and starts from the in-memory copy. In "opencodex" mode, a save that the store refuses because another writer changed auth.json still serves the in-flight token to that request, while memory follows the file. That keeps the behavior of lidge-jun#7035. A confirmed deletion still gives no token. The desktop-compatibility identity reader (lidge-jun#6079) now reads through the store observe step.
