Summary

This fixes two of the runner-load CI flakes tracked in #6912. Both were test-design problems, not product defects. Neither fix raises a timeout or weakens an assertion.

Drip-feed probe test. plaintext-v2-agent-messages-server.test.ts asserted that a drip-fed, headerless upstream body fails closed in under 3000 ms of wall-clock time. On loaded runners it took 3279 ms (Windows 2/9) and 5397 ms (macOS control). It also failed on the 2.82.0 candidate.

The test now uses Bun fake timers and an upstream that sends a byte only after its read is acknowledged:

  • Time is anchored to the probe's first read.
  • A byte arrives at 300, 600 and 900 ms, and each one renews the inactivity window.
  • At 999 ms the response is still pending and the upstream has not been cancelled.
  • At 1000 ms the request fails with 502 "unsupported content type", and the upstream has been cancelled.

The old check only bounded elapsed time. The new one shows that the fixed total budget fires exactly while inactivity keeps renewing. With the total-budget timer removed, the test fails.

Plugin-loader lifecycle tests. On macOS, the loader spawns /bin/ls -lebd for the plugin directory, each ancestor and each file, with a 2000 ms probe timeout. When a loaded runner made one probe time out with partial output, the loader correctly failed closed with ancestor_untrusted at about 2036 ms. That happened before the setup-deadline test reached the behavior it checks.

  • src/plugins/loader.ts gains setPluginAclInspectorForTests(inspector?). It is a shipped mutable seam with no config or env path, and undefined restores the real inspector.
  • Only the six lifecycle-focused tests install a no-ACL inspector, through a helper that restores it in finally and again in afterEach.
  • Every real /bin/ls, fake-getfacl and setfacl test is unchanged and still runs the real inspector.
  • New injected-refusal cases prove that the directory, ancestor and file checks each consult the inspector. They fail if the override lookup is removed.
  • The probe timeout and fail-closed rules are unchanged.
  • structure/ops/plugins.md documents the seam and its residual risk.

The third #6912 flake, the desktop journal lock, is fixed in a separate PR.

Verification

  • bun scripts/test-with-pinned-bun.ts tests/server/plaintext-v2-agent-messages-server.test.ts tests/lib/plugin-loader.test.ts: 62 pass, 4 skip (Linux-only ACL cases on macOS), 0 fail.
  • The drip-feed test passed 3 times in a row.
  • Negative controls:
    • With the total-budget timer removed, the drip-feed test fails (0 pass, 1 fail).
    • With the override lookup removed, the 3 injected-refusal tests fail.
    • Both mutations were restored byte for byte.
  • bun run typecheck, bun run structure:check and bun run privacy:scan pass.
  • bun test tests/ci-workflows/file-size-ratchet.test.ts tests/test-layout.test.ts tests/test-layout-tooling.test.ts: 28 pass. No new test files.
  • The full suite was not run locally. Hosted CI covers it.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Summary by CodeRabbit

📝 Walkthrough

Walkthrough

The plugin loader now supports a replaceable ACL inspector for tests, with updated loader tests and documentation. A plaintext-probe test now uses fake timers to check the inactivity deadline and upstream cancellation.

Changes

Plugin ACL inspector

Layer / File(s) Summary
ACL inspector override
src/plugins/loader.ts, structure/ops/plugins.md
The loader adds setPluginAclInspectorForTests(inspector?). aclTrustError uses the injected inspector when set and otherwise retains the existing ACL checks. The documentation describes its scope, reset behavior, and mutable export.
Plugin loader ACL tests
tests/lib/plugin-loader.test.ts
The tests reset the inspector after each test and use a helper to bypass real ACL checks in selected cases. Parameterized cases verify the loader errors for refusals at a plugin directory, an ancestor, or a plugin file.

Plaintext probe inactivity timeout

Layer / File(s) Summary
Deterministic inactivity timeout test
tests/server/plaintext-v2-agent-messages-server.test.ts
The byte-drip test uses fake timers to check that each byte renews the inactivity deadline. It verifies the timeout response and upstream cancellation, then cleans up the request, stream, and timers.

This change only adds a test seam and updates tests. The seam lives in shipped code, so it is worth tightening, but it is not reachable from the package's public exports today.

Keep the ACL override out of production code.

setPluginAclInspectorForTests is exported from the shipped src/plugins/loader.ts module. A caller that installs () => null makes aclTrustError return before the Linux or macOS ACL checks. Owner and mode checks still run, but the documented ACL guarantee is bypassed.

The package root does not export src/plugins/loader.ts, and no current non-test source imports this setter. However, the source is shipped, and the documentation states that this boundary is enforced only by convention. Move the override into a test-only seam or otherwise prevent production callers from installing it.

Test inactivity renewal independently of the total budget.

In tests/server/plaintext-v2-agent-messages-server.test.ts, a probe that never renews its inactivity timer can still pass this test: the response remains pending at 999 ms and times out at 1000 ms. The total-budget timer masks that regression. Keep this boundary test, but add a case that advances past the original inactivity deadline while remaining before the total-budget deadline. Assert that the probe is still pending in that case.
