Failed request rows can include bounded `upstreamErrorType`, `upstreamErrorCode`, and

Request logs record these as 429 for `rate_limit_exceeded` / `rate_limit_error`, or 503 for

a genuine completed, failed, or incomplete terminal replaces it before account-health recording.
Refusal precedence is unchanged.
This diagnostic mapping adds no automatic replay or retry.

P2 Badge Preserve recognized codes when the error message is absent

When an upstream emits a valid bare error containing only a recognized type/code, such as {"type":"error","error":{"type":"rate_limit_error","code":"rate_limit_exceeded"}}, these new arguments never carry that classification: boundedBareUpstreamError returns undefined whenever both the message and refusal code are absent, so upstreamError() remains undefined and EOF takes the adapterEofIncompleteFrame branch instead. Both relay modes consequently expose a generic incomplete terminal rather than the intended 429/503 failed terminal; treat a recognized type/code as sufficient evidence and synthesize a bounded fallback message.

Preserve explicit http_status before provisional classification.

When a bare error contains code: "rate_limit_exceeded" and http_status: 503, captureTerminalHttpStatus records provisional 429 before combo preflight reads the explicit field. The projection then uses 429, so the synthesized failure and final failed request-log row can report the wrong status.

Only record the recognized provisional status when the bare event has no valid explicit status.

In captureTerminalHttpStatus, check for a valid explicit HTTP status on the bare
error or its nested error before calling noteStatus with
recognizedUpstreamError’s provisional status. Only record the provisional status
when no explicit status in the 400–599 range is present.
