Summary

Heal lost routed catalog rows only from the idle owner. Establish observation immediately after successful Codex startup sync, before unrelated startup awaits. Use filesystem-equivalent path comparisons for baseline, target, publication, and restore fences.

Manual dependent catalog series: #6553 → #6558 → #6560 → #6563. Carries #6530 (451bcd43e078d811b2bef0455000d48964bb06cd) once in A1 and #6537 (8e50608319dcd5180e43fa2b9377ed4b3702127b) across A2–A4. Each layer's diff is relative to its named parent.

The lane's planning documents were moved to ignored scratch and removed from rewritten reachable branch history. Prior public exposure is not erased by this cleanup. Public outcome and attribution remain here; source disposition and integration belong to the coordinator.

Verification

Path red10 failures and startup red evidence captured; final focused80/80 and adjacent179/179 pass. Typecheck, structure and ratchet pass. Final privacy/layout gates pass. Windows-specific casing cases remain for native execution. Independent security/code review PASS. Fresh current-head hosted CI is running; earlier passing checks are historical. Full local/changed suites are omitted under the explicit concurrent-lane resource exception; focused behavior tests plus typecheck/structure/privacy/layout/ratchet are required. Native installed Desktop/live-provider acceptance remains separate. No merge or review-thread resolution performed.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Final repair head 75f9a28947ab5945bc387655249ef9d4b12633a4: Cross-platform CI 37179255239 completed success (pull_request, attempt1). Earlier-head/cancelled runs are historical. All reviewed findings have verified repairs; review-thread resolution and integration remain with coordinator.

Hosted follow-up repaired the stale readiness source-oracle expectation; bun test tests/cli/cli-ready.test.ts tests/cli/cli-start-routing-heal-wiring.test.ts tests/codex-integration/codex-catalog-self-heal.test.ts tests/claude-integration/claude-agent-startup-sync.test.ts passes125/125. Independent review passed.

Current-dev integration refresh: rebased unchanged onto 33125e62d267345d8aaa2e32b993d5bbe8050ff3. Range-diff reports all10 catalog commits identical. All103 non-overlapping landed paths remain byte-identical and both test registries preserve the complete union. The six verified repairs, original source credit, absent security plans and explicit first-event-only Windows audit deferral are preserved. Independent integration review PASS.

Fresh combined local verification:316catalog tests,138landed-behavior tests,43layout/core-boundary tests passed; typecheck/structure/privacy/ratchet passed. Docs build:561pages and77,953 internal links passed. Full/changed local suites omitted under the recorded resource exception. New exact-head per-PR CI is pending; previous-head results are historical. No merge, retarget or settings changes performed.

Integration gate: this parent-based PR does not select the base-filtered Service lifecycle workflow. After A4 targets dev and its integration head/base are final, an actual successful Service lifecycle run is required in addition to Cross-platform CI. Absence or skip is not passing evidence. No premature dispatch was performed.

Post-A1 integration refresh: coordinator merged #6553 as 9289396e5400db1d42012ef01b2444e4e29b8e13; A2 automatically targets dev and A3/A4 retain manual parents. The remaining eight commits rebased unchanged onto that actual merge. New head df653d9d35d4218dae9338271fec61f028f5ac18; all interdiffs equal, source tree unchanged, original credit and all fixes/Windows deferral preserved. Independent fresh interdiff review PASS. No prior CI is reused: new exact-head per-PR checks are required. No merge, review resolution, repository setting change, or deleted-parent recreation performed by this lane.

New A2 review follow-up carried into head 4d6032a2bc7e8256feba3cb73a0cb847b2191d13: direct journal writers now enforce ownership; both foreign binding and exact recovery bytes are preserved. Catalog comparison/count reuses one Buffer while preserving read-error/no-op/clear behavior. RED4fail→GREEN34; adjacent145/layout18 and cascade140 tests pass, typecheck/structure/privacy/ratchet pass. Independent security and cascade review PASS. Fresh current-head CI is required; old results are historical.

Post-A2 integration refresh: coordinator merged #6558 as f74a3db92650bfd40fc6496b92a634f2265c7f93. A3 automatically targets dev; A4 remains on its manual A3 base. All six remaining commits rebased unchanged, with actual merge parents/ancestry checked. New head e7efbc84e4d5fb206f1c2ffcdaed5c3eda94a5d3; independent fresh interdiff review PASS. Journal guards, single-buffer handling, earlier fixes, Windows audit deferral/public docs and original credit remain intact. New exact-head per-PR CI is required; no previous CI is reused. A4 still needs actual Service lifecycle verification after its later final dev-base integration refresh. No merge or review resolution performed by this lane.

Final A4 integration refresh: coordinator merged #6560 as 8aff79ed2942be778e6cdba8237f902122cb37e1; GitHub automatically retargeted #6563 to dev. The three A4 commits rebased unchanged onto that actual merge, yielding 5bd1d554f1838a34fcde1381c1b52a3dd7462f73. Fresh independent interdiff review PASS; the 16-file healer/startup layer and final tree exactly match prior reviewed content. All fixes, credit and explicit Windows audit deferral remain preserved; no public security plans restored.

Both new current-head/dev-base Cross-platform CI and Service lifecycle must actually complete successfully. Earlier parent-based CI is historical and not a substitute. No merge, release workflow, installed runtime change, review resolution, repository setting change or merged-parent recreation was performed by this lane.

Final review follow-up at 89f5ef841dcece00c82da0b30af2bc55b5fb9965: matching owner publications now update the read-only baseline while write eligibility gates are closed, retaining stopped/running/path/release fences and both write-gate checks. Four regressions failed before and pass after; focused159tests, typecheck/structure/privacy/ratchet pass. Independent review PASS. The test-layout selection assumption was rebutted against the physical-directory schema and Bun import-graph runner. Both authorized review threads were answered with evidence and resolved. Fresh Cross-platform CI and Service lifecycle on this repaired head/dev base are required; previous results are pre-repair only.

Final repaired-head evidence: 89f5ef841dcece00c82da0b30af2bc55b5fb9965 on dev base 8aff79ed2942be778e6cdba8237f902122cb37e1 has successful Cross-platform CI 37188595240 and Service lifecycle 37188595216, both pull_request attempt1. All three Service jobs actually passed. Both new review threads were answered with evidence and resolved; no unresolved threads remain at verification. No merge performed by this lane.

📝 Walkthrough

Walkthrough

This change adds owner-lifecycle Codex catalog observation and guarded self-healing. It adds publication events, path and file checks, startup coordination, and integration tests for convergence, retries, ownership gates, and restore behavior.

Changes

Codex catalog self-healing

Layer / File(s) Summary
Catalog observation and publication signals
src/codex/catalog/heal-observation.ts, src/codex/catalog/publication-observer.ts, src/codex/internal/catalog-writer.ts, src/codex/catalog/restore.ts, src/codex/inject/restore.ts, tests/codex-integration/codex-catalog-heal-integration.test.ts
Catalog paths are selected from journal state and files are inspected with size and identity checks. Subscribers receive notifications for catalog publication, restore, and native release. Tests cover invalid or unavailable targets, event delivery, and path aliases.
Guarded catalog convergence
src/codex/convergence.ts, src/codex/management-convergence.ts, tests/codex-integration/codex-catalog-heal-integration.test.ts
Convergence accepts an expected catalog path and a synchronous pre-commit check. Tests verify stale commits do not publish or write catalog and cache data, and verify accepted commits preserve unrelated config edits.
Owner lifecycle and healing
src/codex/catalog-self-heal.ts, structure/codex-home.md, tests/codex-integration/codex-catalog-self-heal.test.ts
The healer tracks catalog baselines, detects missing enabled routed namespaces, checks lifecycle and ownership gates, and applies retry and hourly limits. The documentation and tests describe and cover these behaviors.
Startup wiring and validation
src/cli/claude-agent-startup-sync.ts, src/cli/index.ts, tests/cli/cli-ready.test.ts, tests/codex-integration/codex-catalog-self-heal.test.ts, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json
Startup begins catalog observation after Codex sync resolves ready. Cleanup stops the healer. Tests cover readiness, failed sync, and OFF startup; the test layout includes the new integration tests.

Merge Risk: ⚪ Minimal · up to 89f5e

The catalog tests’ placement does not leave CLI startup ordering untested by CLI verification. No actionable merge-blocking risk remains after normal checks.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 89f5e

Automatic repair remains constrained by ownership, inactivity, configuration and target-path checks immediately before writing. No introduced security weakness was established. Recovery across partially completed writes and platform-specific path behavior remain incompletely verified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The traced mutation scope is the selected shared Codex home's catalog, cache, backups and associated discovery configuration. Other consumers of that home can observe these changes; the traced path does not establish new remote authority or cross-service access.

Security Findings and Attack Paths

  • inferred — Influencing observed catalog rows can trigger a repair attempt, but observed paths and namespace strings are not sufficient write authority. Enabled-namespace filtering, admitted candidate construction and final ownership and target checks constrain the path to publication. No bypass was established in the traced flow.

Trust Boundaries and Controls

  • observed — Healing requires no active turn, no connected or journal-owning client, eligible runtime and home ownership, file-backed configuration and enabled Codex synchronization. Foreign or unknown home ownership is also refused by the serialized convergence funnel.

Resilience and Maintainability Implications

  • inferred — Retries gather fresh evidence rather than replaying a consumed candidate, and catalog/cache replacements skip identical bytes. These controls reduce stale-write and repetition risk, but do not prove atomic recovery across all persisted files after interruption or a configuration-save failure.

Scoped startup correctness review at 185c2c61abf37171fe34233471098a21b6456f72: discussion4176133621 identifies a real baseline gap.

src/cli/index.ts:705-737 publishes through startup Codex sync before awaiting Raycast refresh and dynamic imports, but creates the catalog healer only afterward. startCodexCatalogSelfHeal then reads the current file as its initial baseline. If Codex removes the routed rows during that interval, the first observed baseline is already empty; subsequent unchanged ticks see no lost namespace and never request a heal.

There is an earlier version of the same gap inside reconcileClientStartupBeforeReady (src/cli/claude-agent-startup-sync.ts:61-64): it awaits the Claude roster and Desktop registry AFTER the Codex sync finishes. Merely moving healer creation immediately below the outer startupSync await still leaves those awaits between publication and baseline capture.

Please retain the successful startup catalog publication's namespace baseline before subsequent asynchronous client reconciliation, without enabling writes during startup or weakening the existing ownership/idle/cleanup gates. Capture publication evidence or arrange the observer at the actual Codex publication boundary, rather than adopting an arbitrary later disk snapshot. Add an injected startup-order regression that holds the roster/registry or Raycast continuation, replaces the just-published catalog during that hold, then verifies the missing namespace is detected after startup completes. Existing 'baseline is immediate' tests instantiate the helper directly and do not cover this CLI ordering.

This blocker is established by the current control flow and helper tests, not a live Desktop reproduction. No local tests, runtime mutation, provider traffic, or security/privacy scan was run for this PR. The separate equivalent-path review finding still needs its own validation; this review neither resolves it nor gives full-PR approval.

Withdrawing only my delayed-startup-baseline objection at current head d303a46. The preloaded module and syncCodexBeforeCatalogObservation establish observation before deferred Claude/Desktop reconciliation; owning docs reflect this. My exact-head self-heal run at c572e5c passed 56 tests, 0 failures, 131 assertions under CPU75%/MemoryMax1536M/swap0/TasksMax64/120s with protected files unchanged. Direct complete interdiff c572→d303 changes ONLY tests/cli/cli-ready.test.ts: it adjusts the source-order oracle for the new forwarding helper; runtime, self-heal tests and docs are unchanged. I inspected that delta but did not rerun the unchanged 56-test suite or execute the modified CLI oracle. Current-head CI, the failed earlier-head checks, independent full-PR/boundary review and Windows/native acceptance remain separate gates. No scan, live operation or full-PR approval is implied.

Require saved configuration authority before removing routed provider or combo aliases. Bare and slashed OpenCodex-authored combo aliases use combo authority; foreign and trusted account-native rows retain their existing treatment.

Record bounded, private catalog-write diagnostics. On Windows, a newly created blank audit file receives data only after required ACL hardening; existing files are skipped without ACL mutation. At most the successful new-file creation event is recorded; later events skip the existing file in the same process and after restart. Hardening failure may leave an empty file, and later events also skip it. This is an explicit deferral of continuous Windows auditing, not complete cross-platform audit-stream coverage; no native NTFS validation is claimed.

The publication handler now accepts a matching read-only baseline while healing is gated, retaining its stopped/running/path/release fences. Both write eligibility checks remain in the tick and precommit paths; the owning Codex-home documentation describes the distinction.

Independent Linux/Bun 1.4.0 validation: bun test tests/codex-integration/codex-catalog-self-heal.test.ts --test-name-pattern 'matching owner publication rearms release|legitimate owner emptiness observed while busy|external owner empty publication during failed heal|during gather prevents publication and late revival' — 18 pass, 0 fail, 31 assertions, 42 filtered out, exit 0. This includes the four new cases and related in-flight/write-refusal fences. Convergence, publication, scheduling and authority were injected; no real client/provider/service was exercised.

Execution was sequential under verified CPUQuota=75%, MemoryHigh=1280M, MemoryMax=1536M, swap=0, TasksMax=64, IOWeight=20, nice=10 and a 120-second deadline, with fresh private homes. Peak memory was 112.1 MiB; the test unit settled and all protected configuration/catalog file snapshots remained unchanged.

Current-head Service lifecycle run 37188595216 completed successfully on Linux, Windows and macOS. Cross-platform CI 37188595240 remains in progress at this check. This is not full-suite/typecheck execution, native Windows healer evidence, independent boundary approval or final PR approval. No security/privacy scan was run.

Owner-authorized maintainer integration into dev after the verified 2.77.0 release. Release workflow 37170949169, the stable tag, published receipt and npm metadata agree at 06841165f884a9176d701310638b2112aca7a514; downloaded package integrity was verified before integration.

Reviewed head: 89f5ef841dcece00c82da0b30af2bc55b5fb9965. Accepted dev base: 8aff79ed2942be778e6cdba8237f902122cb37e1. Cross-platform CI run 37188595240 attempt 1 passed. The actual tested checkout cb42a5ee2de7dbe0d5d4390b14e559de46be786f has those exact base/head parents. All four test shards, required gates, structure and aggregate passed.

Independent technical/security review, source coverage and original contributor credit were checked separately. The current-head automated review completed successfully, no unresolved review threads or maintainer objections remained, and the live maintainer-integration helper passed. Native/live limitations remain as documented in this PR; this does not establish installed-runtime acceptance.

This records the explicit maintainer-integration decision under MAINTAINERS.md. It is not self-approval or a substitute for independent review.

Service lifecycle run 37188595216 attempt 1 also passed Linux systemd, macOS launchd and Windows scheduled tasks. Its actual tested checkout matches the accepted base/head and candidate source tree.

The tree is upstream dev plus the current heads of
feat/chatgpt-desktop-pac-fallback (which includes the ready-marker
watcher and the send-unblock intercept), feat/dsh-desktop-profile-patch
and fix/codex-home-test-guard. The earlier local commits on this branch
are replaced: the catalog safety work landed upstream through lidge-jun#6553,
lidge-jun#6558, lidge-jun#6560 and lidge-jun#6563, and the ChatGPT desktop work now follows the
maintainer's app-server shim structure. The previous dev stays as the
first parent so this is a fast-forward; it is also kept as
backup/dev-before-upstream-rebuild-20261005.
