Summary

Closes #6288.

On Windows, ocx update could stop the proxy and then fail in the transactional staging install with ENOTDIR on mkdir. The reported cause is a %LOCALAPPDATA%\npm-cache junction whose target on another drive was gone. Two gaps let that through:

  • The npm cache preflight returned windows_skip on Windows before inspecting anything. Its ownership and mode walk is Unix-only, but the simple "can npm use the cache root" check was skipped along with it.
  • Staging runs npm install -g --prefix <stage>, and npm takes its global config from <prefix>/etc/npmrc. A cache= setting in the user's global npmrc was therefore dropped during staging, and npm fell back to its default cache root, which no check had looked at.

The fix:

  • A new inspectNpmCacheRoot checks that the cache root, or the nearest existing folder npm would create it under, resolves to a directory. A file in its place fails as cache_root_not_directory. A link or junction whose target is missing fails as cache_root_dangling_link. Both print fixed guidance that contains no path and no npm output. POSIX runs this check before its existing ownership walk. Windows runs only the root check, in a --root-only worker mode.
  • On Windows, only those two root reasons can block an update. Windows skipped this gate entirely before, so an unresolvable npm config get cache, a worker timeout, or any other inconclusive result still returns windows_skip and the update proceeds unpinned as it did before. POSIX keeps failing closed on those results.
  • bin/ocx.mjs resolves npm config get cache once, before stopping anything, using the environment staging uses. It checks that exact path and passes it to staging as --cache, so the stage installs into the root the preflight inspected.
  • The troubleshooting page update-failed.md replaces its "What is not known yet" section with the confirmed cause and what to do for each reason. structure/ops/service-and-sidecars.md records the contract.

Verification

  • I did not run local test suites or typecheck. That is an explicit instruction from the project owner, not the AGENTS.md resource exception. Hosted CI at the exact head is the only execution evidence. It includes a dispatched lane=all run for the Windows shards, where the real directory-junction case runs. The run will be linked before merge.
  • New and updated tests: tests/update/update-npm-cache-preflight.test.ts covers usable and missing roots, a file at or above the root (also through the real worker), a simulated dangling junction, a real dangling symlink where the host allows it, a real junction on Windows only, the Windows skip for inconclusive results against the POSIX fail-closed path, the cache-path resolver, and its error cases. tests/update/update-transactional.test.ts checks that staging receives --cache only when a path was resolved. tests/update/update-stop-first.test.ts checks that the launcher's gate still runs before every tray and proxy stop and that the pinned path reaches staging.
  • git diff --check, bun run structure:check, and bun run scripts/privacy-scan.ts pass. No changed file is in the file-size baseline.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. This changes the self-update install invocation (--cache), so a MAINTAINERS.md security review is requested. An independent verdict bound to the exact head will be posted before merge. Failure messages never include the cache path or npm output.

Summary by CodeRabbit

  • Bug Fixes
    • The updater now checks the npm cache before stopping the proxy and reports specific guidance when the cache root is a dangling link or not a directory.
    • Cache checks now account for platform-specific behavior, and the verified cache location is used during staging when it can be safely resolved.
  • Documentation
    • Added troubleshooting steps for cache-check failures, including when to retry and what details to share if the error persists.

📝 Walkthrough

Walkthrough

The updater now resolves npm’s global cache path, checks the cache root before stopping the proxy, and uses the resolved path for staging. Windows blocks updates for dangling cache links and non-directory roots, while other inconclusive Windows checks remain non-blocking.

Changes

Npm cache update flow

Layer / File(s) Summary
Resolve and inspect the npm cache
src/update/npm-cache-preflight.d.mts, src/update/npm-cache-preflight.mjs, tests/update/update-npm-cache-preflight.test.ts
The preflight resolves and validates npm’s global cache path, checks the cache root, and returns distinct results for dangling links and non-directory roots. Windows performs a root-only check and skips other inconclusive results; POSIX retains its existing inspection behavior. Tests cover path resolution, root checks, platform handling, and failure guidance.
Check before stopping the proxy and pin the staging cache
bin/ocx.mjs, src/update/transactional-install.d.mts, src/update/transactional-install.mjs, tests/update/update-stop-first.test.ts, tests/update/update-transactional.test.ts, structure/ops/service-and-sidecars.md, docs-site/src/content/docs/troubleshooting/update-failed.md
The launcher checks the resolved cache before stopping the proxy and passes the path to transactional staging. Staging adds --cache when a path is supplied. Tests verify the order and npm arguments; the documentation describes cache-root failures and recovery steps.

Change: Bug fix · Severity of issue fixed: Medium

Merge Risk: 🔵 Low · up to 73490

The update flow itself looks sound. Before or soon after merge, fix the type declaration and the cache diagnostics/docs so they match the actual behavior and operators look at the right cache folder.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 73490

The change reduces avoidable update failures while preserving staged verification and rollback. No introduced security attack path was established. Remaining uncertainty concerns concurrent cache changes, inherited worker authority, and comparison with the complete prior implementation.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The directly evidenced scope is the invoking user's npm cache and the local package replacement lifecycle. Cache configuration influences where npm reads and writes during staging, while installation continues under the invoking process's operating-system authority. Broader deployment or tenant exposure is not established by the inspected evidence.

Trust Boundaries and Controls

  • observed — Global cache resolution avoids a repository-local npmrc selecting the pinned cache. Resolution and installation remove the ownership-mutation token. The inspection worker instead inherits the ambient environment and, with a supplied path, performs filesystem inspection without invoking npm. Its call precedes acquisition of the updater's own mutation lease.

Resilience and Maintainability Implications

  • inferred — Pinning preserves cache-path selection, not directory or junction identity across time. Concurrent filesystem changes can invalidate the preflight observation and cause staging to fail after shutdown. The inspected flow contains expected staging failures without replacing the live package; no introduced cache-race exploit was established.

Hardening Proposals

  • proposed — Consider passing the token-stripped environment explicitly to the inspection worker, matching the least-authority convention already used for npm resolution and installation. This is defense in depth, not an observed exploitation path.

P1 Badge Classify ENOTDIR as a broken cache root

When a missing cache suffix is beneath a regular-file ancestor (for example, <file>/nested/cache), lstat throws ENOTDIR, not ENOENT. This branch therefore reports cache_entry_inaccessible; on Windows, runNpmCachePreflight converts that reason to windows_skip, allowing the updater to stop the proxy and attempt the doomed staging install instead of aborting beforehand. The added assertion in tests/update/update-npm-cache-preflight.test.ts:277 currently fails for exactly this case, so handle ENOTDIR here as cache_root_not_directory.

P2 Badge Restore windows_skip in the result reason type

This Windows fallback still returns { ok: true, reason: "windows_skip" }, but this commit removes "windows_skip" from NpmCachePreflightReason in npm-cache-preflight.d.mts. TypeScript consumers consequently receive a return type that excludes a real runtime result, causing valid checks for this outcome to be rejected and making exhaustive handling unsound; add the reason back to the declared union.

Security review (MAINTAINERS.md, self-update install path)

An independent read-only review at 71a5e7cc6c found one Medium issue. npm config get cache ran in non-global mode from the user's current directory, so a project .npmrc in whatever repository the user ran ocx update from could choose the value pinned into the staged global install as --cache. On Windows, a value containing cmd.exe metacharacters could also survive npm.cmd's re-parse of %*.

7ece0f13bc fixes both. The resolver now runs npm config get cache --global from the home directory, which matches the npm install -g stage that never reads project config. On Windows, a resolved path containing any of " % ! ^ & | < > is refused rather than escaped, and the update proceeds unpinned. Regression tests cover the resolver arguments and working directory, plus Windows refusal with the POSIX literal-path contrast. The re-review returned VERDICT: PASS — HEAD 7ece0f1.

Restore "windows_skip" to NpmCachePreflightReason.

src/update/npm-cache-preflight.mjs, Line 265, still returns { ok: true, reason: "windows_skip" } for inconclusive Windows results. The declaration now excludes that reachable result.

A typed caller that checks result.reason === "windows_skip" receives a non-overlapping comparison error, although the branch remains valid at runtime. TypeScript documents this behavior for literal unions. (typescriptlang.org)

Restore the literal to the union.

Match cache diagnostics to the updater’s global query.

The updater resolves npm config get cache --global. The failure messages and troubleshooting page recommend an unqualified query. Inside a project with cache= in .npmrc, that query can report a different folder because global mode ignores project configuration. Operators can therefore repair or report the wrong cache. (docs.npmjs.com)

Distinguish inspection skips from resolution failures.

When cache resolution succeeds but the worker times out, runNpmCachePreflight returns windows_skip. However, bin/ocx.mjs, Line 289, still assigns the resolved path to npmCachePath. Line 750 passes that path to staging, which adds --cache.

The update therefore remains pinned after an inconclusive inspection. It proceeds unpinned only when cache resolution fails, including unsafe Windows paths.

Replace “the update proceeds unpinned” with this distinction.

As per coding guidelines, a structure document states “the contract that holds right now, in the present tense.”

The npm cache pre-flight returned windows_skip on Windows, so an unusable
cache root (a %LOCALAPPDATA%\npm-cache junction whose target was removed)
was only discovered by the staging install, after the proxy had stopped,
as a bare ENOTDIR on mkdir. Staging also ran npm with --prefix <stage>,
which moves npm's globalconfig into the stage, so a cache= from the
operator's global npmrc was dropped and staging used npm's default root.

- add inspectNpmCacheRoot: the root, or the nearest existing ancestor npm
would mkdir under, must resolve to a directory; classify a file in the
way as cache_root_not_directory and a dangling link or junction as
  cache_root_dangling_link, with fixed path-free guidance
- run the worker on Windows in root-only mode instead of skipping; POSIX
keeps the ownership/mode walk after the root check
- the npm launcher resolves npm config get cache once, pre-flights that
path, and pins it into the transactional stage with --cache
