fix(combos): a spent pool account must not cool the whole target - #6234

Conversation
`advanceComboAfterFailure` cooled the combo target for any target-scoped failure, and `cooldownMapKey` is `comboId + provider/model` with no account in it. A provider with an account pool therefore shared one cooldown entry across every account, so a 429 from a single spent account removed the whole target from selection and the remaining accounts were never tried. On 2026-09-28, 01:17-05:18 WITA, one Anthropic account answered `Retry-After: 318747` (88.5 hours, its weekly window gone). `anthropic/claude-opus-5-5` was cooled outright and the gateway returned 503 with zero upstream sends for four hours, while a direct probe showed four of five accounts returning HTTP 200. A 429 names the ACCOUNT that is spent, not the target. Every account pool already records that account's own cooldown and routes past it (`rotateAnthropicAccountOn429`, `rotateKeyOn429`, the Codex health store), so the combo layer needs to stop duplicating that decision at target scope rather than to grow a second cooldown map beside the pool's. The account is read from the provider label the dispatch stamped (`anthropic-p2e22d0`), which a pool rewrites and a configured provider name never matches. `accountLogLabel` is deliberately not used: it is stamped for a lone configured API key too, so it cannot tell a pooled account apart from a provider with no pool to rotate within. Unknown account still cools the target, which is the half that keeps this from trading a blackhole for a hammer. It also lands exactly where it is needed: when every account is cooled, the pool refuses locally with a 429 BEFORE it picks one, so no account is named, the target cools on the pool's own Retry-After, and nothing is sent upstream. Target-level failures (provider down, model gone) and provider-scoped credential failures are untouched. Co-Authored-By: Claude Code <noreply@anthropic.com>

CodeRabbit review on lidge-jun#6234. `cooled` is a filtered subset of `ids`, so its positions are not account indexes. Mapping it with `cooled.map((_, index) => credential(index))` built tokens for accounts 0 and 1 when accounts 0 and 2 were the cooled pair, and the assertion would then pass even if the retry left through cooled account 2 -- the exact failure the test exists to catch. Each cooled id now resolves to its own token, and the same claim is asserted from the other direction: the token actually sent is mapped back to its account and that account must be one the first request never cooled. Correct, and fixed in
I also asserted the same claim from the other direction, since the negative form alone is easy to satisfy by accident: the token actually sent is resolved back to its account, and that account must be one the first request never cooled. #6234) (#6255) Carries #6234 by @vadymhimself: a 429 attributed to a pooled account that was itself cooled no longer cools the whole combo target, so healthy sibling accounts stay usable; unknown-account 429s still cool the target. Co-authored-by: Vadym O <bolein95@gmail.com>

Landed on dev via the credited carry #6255 (Co-authored-by trailer retained in the squash commit). Thank you, @vadymhimself. |
Summary
When a pooled provider returns a 429 for one spent account, the combo layer cools the whole target —
comboId+ provider/model — removing it from selection for every account behind it.The pool has already handled that failure.
rotateAnthropicAccountOn429(src/oauth/anthropic-routing.ts) writesupstreamHealth[accountId].cooldownUntilfrom the sameRetry-After, drops the affinity and rotates;rotateKeyOn429(src/providers/key-failover.ts) and the Codex health store do the same. So the combo layer is not supplying a missing account cooldown — it is duplicating the pool's decision at target scope, and blacking out the accounts that still answer. What this costs in production
On a fork of this repo, 2026-09-28 01:17–05:18 WITA: one Anthropic account answered
Retry-After: 318747(88.5h, weekly limit exhausted).anthropic/claude-opus-5-5left selection and the gateway returned 503 with zero upstream sends for four hours, while a direct probe taken during the incident showed four of the five accounts returning HTTP 200.The change
On a 429 that a pool attributes to a single account, the combo layer records nothing; the pool's own entry is the account cooldown.
Three source files, 42 lines. All three conditions are required, so every other failure cools the target exactly as today.
The unknown-account fallback is load-bearing, not decoration. When every account is cooled,
resolveAnthropicAccountForSessionreturnsreason: "all-cooled"andrequest-transport.tsanswers 429 locally before any account is selected. No account is named, failedAccountisundefined, and the target cools on the pool's ownRetry-Afterwith nothing sent upstream. The blackhole case and the hammering case are separated by whether the pool got far enough to pick an account — which is exactly the right line.poolAccountProviderLabelderives the account from the rewritten provider label (anthropic-p2e22d0), which only a pool dispatch produces. It deliberately does not useaccountLogLabel: that is stamped for a lone configured API key too (stampApiKeyAccountLabel), so it cannot tell a pooled account from a provider with no pool to rotate within — using it would skip the cooldown for single-key providers and hammer a target nothing else is holding back.Deliberately unchanged: API-key pools and the generic OAuth failover providers (xai, cursor, kiro) do not rewrite the provider label, so they keep today's target-level cooling.
Verification
Proven red before green. Forcing
accountScopedtofalse:Restored:
2 pass / 0 fail. The second test — all accounts spent, target still cools — passes in the red state too, which is the point: the fallback is not what this changes. The end-to-end case seeds 5 accounts and a real 429 carrying
Retry-After: 318747. Request 1 burns 4 of 5 (the pool rotates three times, then returns the refusal); request 2 leaves through the untouched fifth and returns 200.Not run: the full
