Do not pace nonterminal 401/403 recovery responses as quota-query failures.
If WHAM returns a nonterminal 401 or 403 while the main account remains hard-locked, fetchMainAccountInfoAttempt returns http_error without marking reauthentication. fetchCodexUsage clears its pacing entry for those statuses. This branch still records a five-minute recovery delay, so subsequent recovery sweeps cannot prepare the token or retry during that interval. Exclude nonterminal 401/403 responses from this recovery-local delay. Keep the existing reauthentication guard for terminal responses.

Clear an older shared deadline after usable usage is published.
If two post-reset epoch reads start before either settles, the first can fail and write sharedDeadlines[pacingKey] here. The second can then publish valid usage and call settle(true). That success deletes only its entry in attempts; ordinary forced reads and hard-lock recovery still see the failed read’s deadline, potentially for the full bounded Retry-After period. Track the order of shared deadlines and accepted reads. When a newer read settles with usable usage, clear the older shared deadline without clearing a later failure. Add a test with a failing epoch followed by a successful in-flight epoch.

Summary
Codex's quota recovery code sent authenticated WHAM usage reads more often than the evidence justified (#6153). The hard-lock sweep queried every minute even when the blocking reset was known to be in the future. Failed ordinary usage reads could also be retried at once by several owners (main probe, pool probe, the 401 replay), with no shared pacing.
This carries #6154 by @terrytan95 and fixes the gaps found while reviewing it:
src/codex/quota-query-backoff.ts) keyed by config home, config generation and credential generation. The main probe, the pool probe and the post-401 replay all go through it. Failures back off 5/10/20/40/60 minutes, and a valid longerRetry-Afterextends the wait (capped at 24 h). A credential replacement gets a new key, so a fresh login is never delayed by the old credential's backoff. Failure deadlines are credential-wide: a post-reset read keeps its own in-flight proof identity, but itsRetry-Afteralso paces ordinary reads and hard-lock recovery. A later, shorter failure never shortens an existing longer deadline.Retry-Afterstops the minute sweep from repeating profile and token preparation before it is due. Unlocking still requires a fresh valid usage reading; time passing alone never releases the lock. The post-reset read uses its own proof epoch, so an older response cannot prove recovery.structure/providers/openai-tiers.md. The CLI provider-accounts docs (en/ko) no longer promise a fresh read every minute while blocked. The how-it-works pages (en/zh-cn) carry the corrected explanation.Closes #6153
Supersedes #6154.
