feat(doctor): warn about opencodex-derived Codex role TOMLs with no model pin - #4966
Conversation
…odel pin The "Codex agent role files" section only looked for model_fallback, so a role file that pins no model at all reported "ok" while every spawn silently ran on the parent model. opencodex does not write these files. They arrive when the Codex desktop external-agent import converts ~/.claude/agents/ocx-*.md into $CODEX_HOME/agents/ocx-*.toml, dropping the model: frontmatter because a claude-ocx-native-- id is not a Codex model and keeping only the instructions. The surviving ocx-route directive cannot make up the difference: it is honoured only on the Claude /v1/messages path and is inert on /v1/responses. The role file therefore names one model while the turn runs another, which is invisible until someone diffs session_meta.agent_role against turn_context.model. Doctor now names those files and says how to fix them. The check is observe-only and WARN, so it does not fail the command, and it never writes to, repairs, or removes a file opencodex does not own. The pin is read through the TOML-aware scanner already used for model_fallback rather than a line match. That matters in both directions: an imported role keeps its instructions in a multiline string that contains the very words the scan looks for, and a strict line matcher would both read a key out of prose and miss valid TOML such as a literal-quoted value, an indented key, or a trailing comment.

Restrict the pin match to the TOML root
When a derived role contains a later TOML table with a model member—for example, [metadata] followed by model = "x"—this match treats that nested value as the root pin and suppresses the warning even though the required root model is absent. This contradicts the root/table-aware diagnostic invariant in structure/config.md:144-147; track table headers, accept model only in the root document, and add a focused nested-table regression case.

Document the Codex Desktop import remediation
This adds user-facing diagnosis and new configuration guidance, but the commit leaves docs-site/ unchanged. The existing Claude Code guide says ocx-route pins the real route without documenting that a Codex Desktop import drops the model pin and makes that directive inert on /v1/responses; add the import hazard, manual pin/removal steps, and SUBAGENTS = false remediation to the relevant guide while keeping translations consistent.

Add focused runDoctor coverage for the new role diagnostic.
The runDoctor tests use temporary CODEX_HOME fixtures but do not create derived role files or assert either model-pin output branch. The scanner tests call the scanner directly, so they do not cover rendered runDoctor output.
Add doctor integration tests that create pinned and unpinned derived roles, then assert the OK and WARN output.

Do not treat a table-scoped model as a root model pin.
parseTomlModelPin does not track TOML table scope. After [metadata], it still matches model = "gpt-5.6-sol" and returns a pin. The scanner then suppresses the warning, although Codex has no root model pin and runs the role on the parent model.
Stop scanning at the first table header, or track TOML table scope. Add a regression test for a derived role with a table-scoped model.

Summary
ocx doctorreportedokfor a$CODEX_HOME/agentsrole file that pins no model at all, so a role that silently ran on the parent model looked healthy. The "Codex agent role files" section only ever asked one question:opencodex does not write these files, which is why nothing owned the check. They arrive when the Codex desktop external-agent import converts
~/.claude/agents/ocx-*.mdinto$CODEX_HOME/agents/ocx-*.toml. The import drops themodel:frontmatter — aclaude-ocx-native--gpt-5.5id is not a Codex model — and keeps only the instructions, including theocx-routedirective. That directive cannot cover the gap: it is honoured only on the Claude/v1/messagespath and is inert on/v1/responses. The role file ends up naming one model while every turn runs another, which the reporter could only see by diffingsession_meta.agent_roleagainstturn_context.model.Doctor now names those files:
A role counts as derived when its name starts with
ocx-or its body carriesgenerated-by: opencodexorocx-route:, and it is reported only when no rootmodelpin is present.ocx-route:is matched without its<!--prefix because the same import text-replaces "Claude Code" with "Codex" inside that body, so anything around the directive should be assumed rewritten. This is a heuristic for a warning, not an ownership claim. It authorizes nothing: the check is observe-only, stays at
WARNso it does not fail the command, and the marker-based ownership rules that govern files opencodex actually writes are untouched. Prefix alone still never grants ownership anywhere. The pin is read through the TOML-aware scanner already used for
model_fallbackrather thanreadCodexAgentModel, whose single-line matcher is right for reading a chain opencodex wrote and wrong for judging a file somebody else wrote. Reusing the scanner matters in both directions, and the tests pin both.Closes #4790
Verification
Local verification was not run, because this lane forbids it. No test, focused test, typecheck, build, install, or
ocxinvocation was executed in this worktree; a past local run in this repository deleted the user's real~/.opencodexdirectory. Hosted CI on this PR is the executable verification for this change.Regression coverage in
tests/routing/subagent-model-fallback.test.ts:ocx-routedirective, no pin — is reported; a role derived by body marker rather than by name is reported; a derived role that is pinned is not; and a user's own unrelated role is never named.model = "..."quoted inside a multilinedeveloper_instructionsblock is prose rather than a key; an empty pin is not a pin; a non-string value is not a pin; andmodel_reasoning_effortis not the pin.Honest gaps
I could not run the Codex desktop external-agent import, so the fixtures reproduce the file shape the reporter documented rather than one I observed being generated. The detection targets exactly the three signals the issue names.
Table context is not tracked, matching the existing
model_fallbackparse: amodelkey under a later table header would be read as the root pin. Codex role files are flat in practice, and for a warning the conservative direction is to stay quiet rather than to accuse. The issue's optional follow-ups — documenting the import in the Claude Code guide, and offering to write matching role TOMLs with real pins — are deliberately out of scope here.
No
structure/doc describes this doctor section; the siblingmodel_fallbackcheck is not documented there either, so no owned contract changes.Checklist
On docs: no user-facing behavior changes beyond one additional diagnostic line, and no
docs-site/page states that doctor ignores unpinned role files. On security: no authentication, credential, token, workflow, or release path is touched. The scan is read-only, reads no value out of the files into any output other than role names, and doctor remains incapable of writing to
