Recompute fresh-connection settings after dispatch overrides rebuild the destination.

providerFetch computes fresh from the initial input and applies Connection and keepalive before calling an override (src/server/responses/fetch-helpers.ts:112-125). The OAuth override can then replace destination with rebuilt.url while retaining dispatchInit (src/server/responses/request-transport.ts:445-446). A host change can therefore retain stale settings or omit settings required by the rebuilt host.
