fix(chat): stop refusal bookkeeping from failing refusal-free turns - #4468
Conversation
A Devin turn streamed ~1000 tokens through /v1/chat/completions and then died with "upstream refusal representations are inconsistent". The turn contained no refusal at all. snapshotRefusalItem exited early only when outputIndex was undefined, so any message item that carried an index was enrolled in the refusal ledger even with zero refusal parts. refusalItem() then registered its id, and the id/index consistency checks began governing ordinary assistant text. That made a benign shape fatal. src/bridge.ts flushes a hidden reasoning envelope at the output_index of a still-open message, so one index legitimately carries two item ids. For a refusal that is a real contradiction; for plain text it is nothing. Devin emits reasoning items, and a chat inbound without a summary takes the hidden-flush path, which is why this combination surfaced it. Enrol an item only when it actually carries refusal content, or when the ledger already tracks that index because real refusal evidence opened it. The non-streaming collector in the same file has always scoped itself this way; the streaming path now matches. The event-level item_id binding at output_item.added/done is likewise limited to an index the ledger already tracks. It tests membership directly rather than through position(), because position() throws on a malformed index and would add a failure to refusal-free streams instead of removing one. Every genuine contradiction still fails closed: a refusal id that moves to another index, a second refusal id on one index, a snapshot that contradicts the accumulated delta, and a non-refusal part replacing a retained refusal part. Not fixed here: the bridge reusing one output_index for an open message and a reasoning envelope. It is untidy against the Responses protocol, but changing it affects reasoning ordering and rendering, so it is recorded as a follow-up. Local product tests, typecheck, build and install: NOT RUN. Hosted exact-head CI on this PR is the merge proof.

Retain message IDs until refusal scope is known
When a normal response.output_item.added announces item.id = A with empty content and a later response.refusal.delta for that index carries item_id = B, this early return forgets A, so refusalItem() seeds B and the stream emits the inconsistent refusal successfully if the terminal snapshot is sparse or omitted. Before this change A was retained and the mismatch was rejected; keep non-refusal message IDs provisionally and reconcile them when actual refusal evidence appears instead of discarding them.

Update every structure document that owns src/chat
This changes the refusal-state semantics of src/chat/, but none of its mapped owners—structure/runtime.md, structure/transports/inventory.md, and structure/data-planes/inbound-compat.md—is updated. The last document still states that present item IDs are correlation constraints, which no longer describes IDs observed before refusal evidence; update all three owned documents in the same change and make the intended ID-retention invariant explicit.

Mark the intermediate position() guard as superseded.
Lines 35-37 label refusalItems.has(position(data.output_index)) as the after implementation, and line 40 applies the same form at :624. position() throws for a malformed index before refusalItems.has(...) can check ledger membership. Copying this snippet can therefore throw outside refusal scope.
Replace it with the final form at lines 81-85, or label lines 35-37 as superseded.

Run focused validation for the test-layout mapping and typecheck.
The scripts/AGENTS.md requirement applies to a changed script and requires focused tests or probes plus bun run typecheck. This mapping is consumed by the test-layout tooling, so run the focused test-layout validation and typecheck before merge.
The privacy:scan requirement applies only when the changed script handles configuration, credentials, requests, logs, or account data. This static test-domain mapping does not meet that condition, so bun run privacy:scan is not required for this change.

The fix enrols an item only when it actually carries refusal content, or when the ledger already tracks that index because real refusal evidence opened it. The non-streaming collector in the same file has always scoped itself this way — the streaming path now matches, which is the asymmetry that identified the bug.
The event-level
item_idbinding atoutput_item.added/doneis likewise limited to an index the ledger already tracks. It tests membership directly rather than throughposition(), becauseposition()throws on a malformed index and would have added a failure to refusal-free streams instead of removing one.Verification
tests/responses/chat-refusal-scope.test.tshas two halves. Four cases assert refusal-free streams now succeed, including the exact reproduction above. Four cases assert genuine contradictions still fail closed: a refusal id that moves to another index, a second refusal id on one index, a snapshot contradicting the accumulated delta, and a non-refusal part replacing a retained refusal part.tests/responses/chat-refusal.test.tsis untouched. Its 18 post-refusalDeltacontract cases still throw, because each has preceding refusal evidence soexistingis true and the new early return never fires — verified case by case against:248,:315and:249.scripts/test-layout/layout.jsonandtests/fixtures/test-layout-expected.json.position()mistake in the first draft of the guard. Not fixed here
The bridge reusing one
output_indexfor an open message and a reasoning envelope is untidy against the Responses protocol, but changing it affects reasoning ordering and rendering. It is recorded as a follow-up in the devlog unit; this change alone removes the user-visible error.Checklist
