feat(hitl): edit-on-approve and the decision audit contract v2 - #1228
A human reviewer in a HITL pause could only approve or reject. If the agent's draft was nearly right, the reviewer had to reject and wait for a rewrite, and the audit trail recorded only a verdict and a free-text note, with no machine-readable reason, no record of whether the reviewer changed anything, and no way for other code to react to a decision.
What changed
Backend
A resume can now carry editedAnswer, reasonCode and per-call taskDecisions.
A RULE pause can be approved with an edited answer. The draft is captured before the pause (IAnswerEditingTask, LlmAnswerEditor) and the edit replaces it as the turn's answer.
Group task results and the synthesis can be edited on approval (GroupReviewEditor, SharedTaskList.reviewerEdits).
MCP resume_conversation and approve_group_phase gain optional edit, reason and per-call arguments.
Slack: "Edit & approve" and "Reject with reason" modals. After an edit is submitted the card reads "Approved (edit submitted)", not "Approved with edits": the resume is asynchronous and the edit can still be refused (screened out, config drift), in which case the turn pauses again on the unchanged draft. Posting a fresh Slack card for that re-pause is not built; the conversation stays actionable in the Manager and over MCP.
Audit contract v2: decisionOutcome, approvalCategory plus its source, reasonCode, edit metrics (digests, never the original text), channel, latencyMs. Each recorded decision is also published as the CDI event HitlDecisionRecorded (@ObservesAsync) for later features to consume.
Manager: edit-and-approve UI and reason codes in the approval banner/card.
Docs: docs/hitl-review-and-audit.md, docs/hitl.md, metrics, the full metrics dashboard (two new panels, ids 300 and 301, with the Kubernetes copy regenerated by scripts/sync-monitoring-assets.py).
Config surface
Off by default: with no review block nothing changes.
on an agent, and "hitlConfig": { "review": { "allowResultEdits": true } } on a group. A decision that carries an edit while the matching flag is off is rejected with a validation error. review is edited as JSON only in the Manager.
What this changes
Every Manager decision now sends X-EDDI-Client: manager, which becomes the audit channel.
Existing audit entries keep their shape; the new fields are additive.
REST/MCP schemas gain optional fields only. openapi-operations.json is unaffected.
Rolling upgrade:SharedTaskList gains reviewerEdits, and revision candidates gain answerFormat and editable. Check that older pods tolerate the unknown field before enabling allowResultEdits on a mixed-version cluster.
Where to look hardest
ConversationHitlService and LlmTask: the draft capture before the pause and the edited-answer resume path.
GroupHitlCoordinator: the branch merges origin/main and has one resolved conflict in cancelStored.
HitlApprovalAudit: that edits are recorded as digests only.
Verification
Unit tests for each new class (validator, audit, edit metrics, Slack modal, LLM edit, group review, MCP args) and Manager vitest suites. Repo guards run locally: ImportStyleTest, BuildQualityGatesTest, ChangelogFragmentTest, DeploymentManifestsTest, MetricsDashboardCoverageTest. Not run locally: Docker-backed integration tests.
Out of scope
A fresh Slack card when a refused edit re-pauses the turn (see above).
Manager UI for group task-result/synthesis edits and a per-call reason select, a hitlConfig.review editor, and driving onRejection: REVISE from the Slack note (Slack tool edits are a JSON textarea). ToolLoopResumer is untouched.
MCP/Platform Operator updates are tracked in planning/operator-followups/hitl-edit-on-approve.md for the combined follow-up.
Merge order and conflicts
Base for approval analytics (F4) and approval cards (F7), which will stack on this. This PR is over 100 files, so the review bots may skip it. Sibling: cost accounting #1222 touches LlmTask, the locale JSONs, docs/metrics.md and the dashboard JSON, so expect textual conflicts there; the dashboard panel ids (300/301 here) were chosen not to collide with the other branches.
Summary by CodeRabbit
New Features
Approvers can edit an agent’s answer before approval, add decision reasons, and see when edits are refused by validation or output screening.
Group reviews support approved edits to task results and synthesis outputs, subject to configuration and validation.
Slack approval cards offer edit-and-approve and reject-with-reason modals when eligible.
HITL decisions include structured audit records and edit summaries, with metrics and dashboard panels for outcomes and edit activity.
Existing approve/reject-only behavior remains available when review settings are not configured.
Documentation
Added guidance for review settings, editing limits, approval flows, and audit records.
hitlConfig.review on agents (allowAnswerEdits, screenEdits, maxEditChars,
approvalCategory, reasonCodes) and groups (allowResultEdits, maxEditChars,
phaseCategories, reasonCodes), plus toolApprovals.rules[].approvalCategory.
All optional; validated at save time.
Every hitl.approval entry is written through HitlApprovalAudit (schema 2):
legacy fields kept, plus decisionId, pauseId, latency, channel, automation
reason, approval category, reason code and units[] with a decisionOutcome
and edit metrics per answer, tool call, phase or task. parse() reads schema
1 and 2 into HitlDecisionUnit; HitlDecisionRecorded is fired async after
each write. Regular and group cancels now name the agent, user, decider
and pause; group decisions capture the phase before it is cleared.
reasonCode on decisions and per tool call, validated before the CAS by
HitlDecisionValidator; approval-status pauseDetails list the vocabulary
and the approval category. REST records the X-EDDI-Client channel.
HitlRevisionPolicy gains captureDraft (review.allowAnswerEdits), keeping
its two-argument constructor. The LLM task keeps its answer as the
revision candidate when either REVISE or edits are on (the request is
frozen only for REVISE), records its answer format (TEXT or JSON), and
the conversation persists the candidate with a rule pause in both cases.
editedAnswer on the resume body replaces the model's draft when the
agent's pinned review config allows it. Validated before the CAS; applied
by the LLM task in place (IAnswerEditingTask, LifecycleManager
.replaceAnswerAt, LlmAnswerEditor): the draft's output item and data
keys only, nothing re-runs. Optionally screened by the task's output
guardrails; a refused edit re-pauses on the unchanged draft and records
hitl.edit.refused. approval-status serves the editable draft, and
IConversationService.validateHitlDecision offers a side-effect-free dry
run.
taskDecisions (verdict, note, editedResult, reasonCode per task) as the
richer, mutually exclusive form of taskApprovals, and editedAnswer on a
PHASE pause after a SYNTHESIS phase, for groups with
review.allowResultEdits. Validated before any mutation by
GroupReviewEditor. An edited result replaces TaskItem.result and is noted
in SharedTaskList.reviewerEdits; a synthesis edit is appended as an
unsigned reviewer SYNTHESIS entry, never rewriting signed entries. Per-task
notes feed RETRY/FAIL; the audit records per-task edits.
resume_conversation takes optional editedAnswer, reasonCode and
toolDecisionsJson (per-call verdicts and amendedArguments, so far only on
REST); approve_group_phase takes editedAnswer, reasonCode and
taskDecisionsJson. Existing overloads delegate; the service validation is
the REST body's, and its message is returned as BAD_REQUEST. The plugin
skill tells assistants to send only text the user wrote or approved.
Regular-conversation approval cards gain Edit & approve (a single tool
call whose redacted arguments fit 3000 characters; a rule draft only with
platformConfig.hitlShareRuleDrafts and edits allowed) and Reject with
reason. Modals open via views.open, bound by private_metadata to card,
pause and call. Submissions re-run approver, workspace, card and pause
checks, merge edited arguments against the raw ones server-side (redacted
leaves stay raw and cannot be edited), and are validated synchronously so
Slack shows errors in the modal. buildApprovalBlocks keeps its signature;
the new overload keeps the actions block last.
The approval banner offers Edit the answer before approving on a rule
pause with pauseDetails.editableAnswer: the draft in a textarea, a
counter against maxChars, JSON validation for a JSON answer and the
original one click away; an unchanged edit is a plain approval. A
reason-code select travels with rejections and edits only. A notice shows
when the engine refused the last edit; reviewer SYNTHESIS entries are
marked in the transcript. Manager decisions send X-EDDI-Client: manager.
hitlReview.* keys in all 11 locales; MSW fixtures and tests.

surface, the Slack modals, the full hitl.approval schema-2 contract with
the schema-1 mapping and the HitlDecisionRecorded event, metrics, and a

for the decision audit entry.

Security architecture risk: Moderate · up to 94ae8
The feature changes what reviewers can approve and how their actions are recorded. A failed group resume can retain task edits despite returning to the approval state, weakening rollback and traceability. Access checks and configuration gates limit exposure, but recovery and trust-boundary coverage remain incomplete.
Retained concerns
Medium · reliability · observed: An executor-rejected group resume can persist an edited task result and reviewer metadata while restoring AWAITING_APPROVAL, without recording the approval decision. Task editing marks the task COMPLETED before submission; rollback reverses synthesis edits but not task edits. This leaves replacement content committed after an operation reported as failed and weakens the correspondence between persisted reviewer changes, retryable approval state, and the decision audit trail.
Security review details
Security Blast Radius
inferred — The demonstrated recovery issue is scoped to an authorized edited approval of a group task followed by resume-submission failure. Its immediate impact is the affected conversation's shared result, approval state, and audit completeness; wider tenant or infrastructure compromise is not established.
Trust Boundaries and Controls
observed — Slack modal handling checks the owning integration, approver membership, configured workspace identity, persisted approval-card binding, and current pause identity. Answer-draft sharing requires an explicit sharing flag and refuses drafts changed by secret redaction. Submitted tool arguments are merged server-side with raw values so unchanged redacted secrets need not reach Slack.
observed — Reviewer synthesis entries use a hitl: speaker prefix. The PR exempts every transcript speaker with that prefix from peer verification, without checking the reviewer-edit marker. Existing verification failures are diagnostic rather than execution-blocking. Production reachability of colliding executable member identities remains unresolved, so this is not treated as a verified new execution bypass.
Resilience and Maintainability Implications
observed — Group approval and rejection transitions use state-and-pause identity comparison, limiting stale or concurrent decisions. This protects pause ownership but does not make task edits, asynchronous execution, and audit recording one atomic transition.
Hardening Proposals
proposed — Define one durable outcome for task edits and approval recording: either restore task results, statuses, and reviewer metadata when submission fails, or explicitly commit and audit the accepted decision independently of execution failure. Keep retries consistent with that choice.
proposed — Represent trusted reviewer provenance separately from an ordinary speaker name, and enforce any reserved identity namespace at executable-member boundaries. Coordinate edit enablement with writer compatibility so older writers cannot silently discard provenance.

Review validation errors return a misleading 400 body.
GroupReviewEditor.prepare now throws IllegalArgumentException for new cases:
allowResultEdits is off.
editedResult is too long.
The reason code is unknown.
taskDecisions and taskApprovals are both present.
These exceptions reach this catch block (Lines 608-615). The block returns a fixed body that says an unknown task id was referenced or a task is not awaiting approval. A reviewer who sends an invalid reasonCode gets a hint about task ids. The streaming twin at Lines 865-871 has the same problem.
The generic body exists so caller-supplied ids are not reflected. The editor's messages put entry.getKey() (a task id) into the field name, so they cannot be returned as they are.
Throw a dedicated subtype from the editor with a curated message that contains no caller-supplied ids. Catch that subtype before the generic IllegalArgumentException, and return its message in both endpoints.
// sketch: in GroupReviewEditor, use a curated exception type
final class HitlReviewValidationException extends IllegalArgumentException {
HitlReviewValidationException(String curatedMessage) { super(curatedMessage); }
}
// sketch: in RestGroupConversation.approveGroupPhase, before the generic catch
} catch (HitlReviewValidationException e) {
return Response.status(Response.Status.BAD_REQUEST).type(TEXT_PLAIN).entity(e.getMessage()).build();
}

lines 608 - 615:
Update GroupReviewEditor to distinguish review-validation failures with a
dedicated IllegalArgumentException subtype carrying a curated message that never
includes caller-supplied task IDs. In both approveGroupPhase and its streaming
counterpart, catch this subtype before the generic IllegalArgumentException and
return its message in the 400 response; preserve the existing generic response
for invalid task approvals.
Outside-diff finding from review 5745571519 (RestGroupConversation, approve path): fixed in 246191c. The 400 body for an IllegalArgumentException now carries the actual message (capped at 300 chars, plain text) so a rejected review field names itself instead of the generic unknown-task-id text.
Also in 246191c (found while stacking on this PR): a failed resume now removes the reviewer's synthesis edit from the transcript before restoring the pause; the pause-to-cancel conversion audits with the captured pausedAt; ReviewerEditStep lower-cases with Locale.ROOT.

Scope the original synthesis to the paused phase and current round.
238-241: 🗄️ Data Integrity & Integration | 🟠 Major | ⚡ Quick win
Scope the original synthesis to the paused phase and current round.
If a continuation’s synthesis phase produces no entry, latestSynthesis can return a prior round’s answer. If the reviewer submits that same text, appendSynthesisEdit treats the edit as unchanged and adds no entry. The final-answer extraction reads only the current round, so the approved edit is not applied. Limit this lookup to the paused phase in the current round. (raw.githubusercontent.com)

Update latestSynthesis’s reverse transcript lookup to consider only synthesis
entries from the paused phase of the current round, using the existing phase and
round metadata rather than returning an earlier round’s answer. Preserve the
existing behavior when a matching current-round synthesis entry exists.
Restrict the hitl: signing exemption to reviewer edits.
Restrict the hitl: signing exemption to reviewer edits.
The current member validation does not reserve the hitl: prefix, and write-time validation would not change existing group documents. A reachable member named hitl:agent can therefore produce an ordinary entry that GroupSigningGuard skips before checking its envelope.
Keep the reviewer exemption only when the entry carries the engine-created REVIEWER_EDIT_MARKER.

Restrict the `GroupSigningGuard` signing exemption for speakers with the `hitl:`
prefix to engine-created reviewer edits. Require
`GroupReviewEditor.REVIEWER_EDIT_MARKER` in `entry.errorReason()` alongside the
existing prefix check, while preserving the exemptions for user entries and null
content.
Outside-diff findings of review 5746244548, fixed in a31bf51:
GroupReviewEditor.latestSynthesis is now scoped to the paused phase of the current round (round start index and phase index), so an earlier round's identical text is no longer taken as the original; test added.
GroupSigningGuard: the unsigned reviewer-edit exemption now also requires the engine-set reviewer-edit marker, so a member merely named with the hitl: prefix is still verified; test added.
Both tests were mutation-checked (they fail with the fix reverted).
