Bug

Unreadable files are skipped with no warning and unparseable ones only warn, so deny rules in them are lost instead of falling back to Ask.

Verification

Checked against fork develop 9af2b0f by code inspection (no live repro unless stated): src/hooks/permissions.rs:152-161.
