What & why

A repo's approval policy and judge now hold for Codex as for Claude, at every seam Codex has:

  • PreToolUse: a deny rule refuses the call before it runs, in every launch mode, apply_patch edits included.
  • PermissionRequest: rules apply first, then the judge on a call no rule decides.
  • Hosted app-server approvals: evaluated before they reach a card.
  • Terminal-hosted agents: evaluated by the daemon's bridge.

The judge sees turns the server has stored:

  • Watched: the hash of each line as kcap watch sent it, bounded by the root watcher's acknowledged count.
  • Hosted: each user message's item id, once the forwarder reports it persisted.

Needs kcap-server #2535; an older server answers windowless.

Where to look

A refusal list is complete only for a session this daemon started itself, so a resumed or adopted run's judge cannot allow. A watched session records no user decline, so its judge can deny or ask but never allow.

Verification

  • A real codex app-server 0.155 fires PreToolUse for both shell and apply_patch, with the input shapes the seams read.
  • Hashes match the server's on a shared prelude fixture and UUIDv5 vectors, including a redacted line.
  • An env-gated live cert drives a real codex through the hook. It skips ungated and has not yet run against a server with #2535.

Summary by CodeRabbit

  • New Features

    • Added Codex policy enforcement for tool calls and permission requests across interactive, hosted, and app-server sessions.
    • Matching deny rules can block actions before execution; approved requests may proceed without prompting.
    • Hosted sessions can use policy-judge decisions, while undecided requests continue to user approval.
    • Improved transcript tracking across reconnects and rewinds, including acknowledgment progress and turn identification.
    • Added file-change association and improved filtering of Codex setup messages.
  • Documentation

    • Documented Codex approval-policy behavior across supported session types.

…start

A resumed, rebound or adopted run may have been refused before this daemon saw it, so its judge cannot allow.

A patch is evaluated by the files its headers name; the PTY lane marks only an agent's own session; a failed card still counts as a decline.

📝 Walkthrough

Codex policy handling now evaluates pre-tool and approval requests in hook, interactive, and hosted app-server sessions. The change adds acknowledgment and persistence tracking, turn declarations for judge consultation, normalized actions, and refusal accounting.

Changes: Codex approval policy

Layer / File(s) Summary
Turn, persistence, and action data
src/Capacitor.Cli.Core/..., src/Capacitor.Cli.Daemon/Services/..., src/Capacitor.Cli/Commands/WatchCommand.cs, src/Capacitor.Cli/Harness/Codex/...
ACP envelopes carry optional turn IDs. Codex watchers persist acknowledgment counts. Transcript components report persisted turns. Codex commands, patches, and preludes use normalized representations.
Hook policy evaluation
src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs, src/Capacitor.Cli/Harness/Codex/..., src/Capacitor.Cli.Core/Policy/PolicyWire.cs, test/Capacitor.Cli.Tests.Unit/Harness/Codex/...
PreToolUse denies matching rules in each launch mode. PermissionRequest applies policy and judge decisions, then preserves Codex handling when no decision is available.
Interactive and hosted approval handling
src/Capacitor.Cli.Daemon/Harness/Codex/..., src/Capacitor.Cli.Daemon/Services/..., src/Capacitor.Cli.Daemon/DaemonRunner.cs, test/Capacitor.Cli.Daemon.Tests.Unit/...
Interactive and hosted Codex requests use policy seams when configured. Hosted requests evaluate canonical commands and file changes, consult persisted turns, and record refusals.
Bridge attribution and validation
src/Capacitor.Cli.Daemon/Acp/..., src/Capacitor.Cli.Daemon/Services/PermissionRefusalLedger.cs, test/..., README.md
Refusal records retain approval and prompt identifiers. Startup bridging distinguishes fresh sessions from resumed or adopted sessions. Tests cover policy, persistence, turn classification, and live Codex flows.

Merge Risk: 🔵 Low · up to b242a

A watcher restart may temporarily prevent the judge from using acknowledged turns. Correct the cursor handoff before merging, or accept this bounded gap.

Security architecture risk: 🟡 Moderate · up to b242a

This change can approve commands and edits without a human decision. Rule precedence, conservative history declarations and human fallback limit exposure. However, safe automatic approval depends on server-side handling of incomplete history and upgrade compatibility that were not verified in this review.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The material exposure is automatic execution of governed interactive sessions' commands and file changes that previously reached a human approval. Effective reach remains bounded by the launched process's configured sandbox, writable roots and available credentials; the inspected runtime still supplies its sandbox policy on turn creation.

Trust Boundaries and Controls

  • observed — Fresh hosted thread starts mark refusal observation as complete-capable; resumed threads do not. Watched consultations explicitly declare incomplete refusals. These declarations communicate the intended restriction on judge authorization, but preventing an allow in those cases depends on the server honoring the completeness contract.
  • observed — Hook policy selection uses the supplied session identifier and repository context to load a frozen per-session snapshot. The inspected code does not itself authenticate that identifier; this remains a trusted hook-protocol assumption, not an established attacker-controlled cross-session path. Hosted launch construction clears inherited app-server markers before applying its own overlay.

Resilience and Maintainability Implications

  • observed — Failure containment differs intentionally by entrypoint: hook policy failures leave native behavior standing, whereas unresolved hosted policy evaluation forwards to a human interaction whose cancellation or unexpected failure returns a decline. Consequently, pre-tool policy is not a fail-closed containment boundary when its own loading or evaluation fails.

Hardening Proposals

  • proposed — Consider locally suppressing judge allow when the submitted refusal declaration is incomplete, as defense in depth against server-version or contract drift. This would supplement the intended server restriction; the review did not establish that the server currently violates it.

High-Level Assessment

Keep policy evaluation at Codex’s distinct hooks and approval bridges while sharing action normalization and the existing policy engine. A single post-approval interception point would miss unasked tool calls; unconditional transcript declarations would risk presenting evidence the server has not stored.

Action required

1. File approvals can grant wider write access ✓ Resolved
Description
CodexHostedPolicySeam.Action evaluates a file-change approval using only the item's changed paths
and ignores the request's grantRoot. When Codex requests write access under that root for the rest
of the session alongside an otherwise allowed edit, the bridge returns accept without sending the
broader grant to the user.
Code

src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[R100-103]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A file-change approval can request session-wide write access through `grantRoot`, but the policy seam evaluates only the changed paths and may auto-accept it.
## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[93-108]
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexApprovalBridge.cs[80-87]
## Recommended Fix
Forward approvals carrying a nonempty `grantRoot` to the human card unless the policy explicitly models and authorizes that broader grant.

2. Hosted network access skips host rules ✗ Dismissed
Description
CodexHostedPolicySeam.Action normalizes a command-execution approval solely from command,
ignoring its networkApprovalContext and approval kind. When Codex requests managed-network
access alongside a command covered by an allow rule, the bridge accepts the approval without
evaluating the requested host; a writeStdin approval with a command value can take the same
command-only path.
Code

src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[R96-98]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A hosted command allow rule can accept a managed-network or stdin approval without evaluating what that approval grants.

## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[93-99]
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexApprovalBridge.cs[82-85]

## Recommended Fix
Inspect the approval kind and network context before command normalization. Normalize and evaluate the requested capability explicitly where supported; otherwise forward that approval to the card rather than accepting it on a command-only allow.

3. Hosted approvals use stale prompts ✗ Dismissed
Description
CodexHostedPolicySeam.DeclaredTurnsAsync filters out unpersisted messages but still declares the
previous stored message as the open turn to kcap-server's judge. If the current Codex message has
not persisted within the two-second wait, the server can validate that older turn as its anchor and
allow the approval without seeing the current user's instructions.
Code

src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[R134-135]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A hosted approval can be judged against the previous user turn when its current turn has not persisted within the wait.
## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[129-139]
## Recommended Fix
If the newest observed user message is absent from the persisted results, send no declared turn set. Add a test where an older message is stored but the current one is not.

4. Renames into unapproved paths get approved ✗ Dismissed
Description
CodexPatchPaths.Observe extracts only each file change's path and omits the rename destination
in kind.move_path. When an allow rule covers the source but not the destination, the hosted seam
sees full path coverage and accepts the file-change approval without a card.
Code

src/Capacitor.Cli.Daemon/Harness/Codex/CodexPatchPaths.cs[R19-22]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
Hosted file-change approvals evaluate a rename's source path but not its destination, allowing a source-only allow rule to approve a move outside its coverage.
## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexPatchPaths.cs[19-24]
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[100-104]
## Recommended Fix
Extract and resolve `kind.move_path` for update changes alongside `path`. Evaluate both paths, and forward the approval to the card if either cannot be placed.

5. Terminal refusals bypass judge history ✗ Dismissed
Description
LocalPermissionBridge now routes terminal Codex approvals through CodexPtyPolicySeam, but its
policy-deny early return never calls RecordRefusal. When the judge denies one approval in a fresh
session, a later request can declare an incomplete refusal history as complete to kcap-server, so
its refusal-based allow clamp does not apply.
Code

src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[R677-679]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A terminal Codex approval denied by its policy judge exits the bridge without entering the refusal ledger, although the next judge request can declare that ledger complete.
## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[677-697]
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexPtyPolicySeam.cs[59-68]
## Recommended Fix
Before returning a Codex policy-deny response, record the refused call in the bridge ledger, or mark its declaration incomplete if the call cannot be described. Test a judge denial followed by a second consultation in the same fresh session.

6. Codex calls can use a later policy ✗ Dismissed
Description
HandlePreToolUse first loads and saves the session policy at the tool call, rather than freezing
it when Codex starts the session. If an approval file changes between session start and that call,
the hook can miss a deny that was present at launch, while hosted approval decisions still use the
launch snapshot.
Code

src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs[R529-531]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The Codex hook builds its persisted policy snapshot on the first tool call, allowing policy edits after session start to change what PreToolUse enforces.
## Fix Focus Areas
- src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs[529-531]
- src/Capacitor.Cli/Harness/Codex/CodexPolicySeam.cs[129-130]
## Recommended Fix
Persist the session snapshot during Codex SessionStart, before tools can run. Ensure hosted sessions' hook snapshot is consistent with the launch snapshot used by their approval bridge.

Remediation recommended

7. Long Codex commands bypass judge ✗ Dismissed
Description
CodexPolicySeam.ConsultJudgeAsync sends the complete analyzed shell action without checking the
server’s 64-segment and 16,000-token-character limits. When an unmatched Codex approval contains an
analyzable command beyond either limit, kcap-server rejects the judge request and the CLI leaves the
decision to Codex’s native prompt.
Code

src/Capacitor.Cli/Harness/Codex/CodexPolicySeam.cs[R112-115]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The new Codex judge call can send analyzed shell actions that kcap-server rejects for exceeding its segment or token limits, so no judge verdict is returned.
## Fix Focus Areas
- src/Capacitor.Cli/Harness/Codex/CodexPolicySeam.cs[110-115]
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexHostedPolicySeam.cs[119-125]
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexPtyPolicySeam.cs[34-38]
- /cross_repos/kcap-server/src/Capacitor.Api.Public/Policy/PolicyJudgeRequestValidation.cs[44-57]
## Recommended Fix
Coordinate CLI and server handling of oversized actions: define a bounded representation the server accepts, preserving enough information to prevent an unsafe allow, and apply it to each new Codex judge lane. Add a cross-repo test with more than 64 shell segments.

ⓘ Copy this prompt and use it to remediate the issue with your preferred AI generation tools

8. Hook tests skip shared process safeguards ✗ Dismissed
Description
The live certification recorder invokes kcap hook --codex directly from a shell script instead of
using Helpers.KcapProcess with a DaemonStore. Whenever real Codex triggers the recorder, the
kcap child bypasses the test wrapper and its process-ownership and lifecycle checks, so the test
does not exercise the required launch path.
Code

test/Capacitor.Cli.Tests.Unit/Harness/Codex/CodexPolicyJudgeLiveCertTests.cs[R119-123]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The live Codex certification test launches `kcap hook --codex` directly inside its shell recorder, bypassing the required `Helpers.KcapProcess` wrapper and its `DaemonStore`-based process lifecycle handling.

## Fix Focus Areas
- test/Capacitor.Cli.Tests.Unit/Harness/Codex/CodexPolicyJudgeLiveCertTests.cs[119-123]

## Recommended Fix
Replace the direct shell-based kcap invocation with the `Helpers.KcapProcess` wrapper constructed with the test's `DaemonStore`. Preserve the hook recorder's stdin, output capture, exit-code propagation, and environment behavior while ensuring the real kcap process is started through the shared helper.

9. Codex policy files accumulate after exit ✗ Dismissed
Description
FreezePolicySnapshot now creates persisted per-session policy state for Codex, but the Codex exit
path never evicts that state. Repeated Codex sessions leave their snapshot files behind, along with
any policy journal and upload markers created by decisions; only Claude's end path performs that
cleanup.
Code

src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs[431]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The new Codex session-start snapshot writes persistent policy files without a corresponding Codex cleanup path.
## Fix Focus Areas
- src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs[429-431]
- src/Capacitor.Cli/Commands/WatchCommand.cs[1243-1248]
## Recommended Fix
Retire the Codex session's snapshot, journal, and upload markers on confirmed session end, reusing the best-effort policy-state cleanup used for Claude.

10. Codex users miss broken-policy warnings ✗ Dismissed
Description
FreezePolicySnapshot discards the snapshot returned by LoadOrBuild, including its degradation
messages, and the Codex session-start output contains no policy notice. If an approvals file is
invalid and its rules are dropped, the user gets no warning at session start while subsequent calls
run without those rules.
Code

src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs[431]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
Codex freezes a degraded policy snapshot without showing the user that an approvals file was ignored.
## Fix Focus Areas
- src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs[429-456]
- src/Capacitor.Cli/Commands/Harness/CodexHookCommand.cs[531-539]
## Recommended Fix
Return the degradation notice from snapshot freezing and include it in the single Codex session-start output, following the Claude session-start notice pattern.

ⓘ Copy this prompt and use it to remediate the issue with your preferred AI generation tools

11. Early terminal approvals bypass the judge ✗ Dismissed
Description
FreshIn requires a fresh agent's discovered SessionId to match the permission request before the
bridge marks its refusal list complete. A new terminal-hosted Codex agent starts with no session ID
and discovers it asynchronously, so approvals arriving before discovery declare an incomplete list
and cannot receive a judge allow.
Code

src/Capacitor.Cli.Daemon/Services/AgentOrchestrator.cs[R1105-1106]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A fresh terminal-hosted Codex session cannot obtain a complete refusal declaration until asynchronous transcript discovery populates the agent's session ID.
## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Services/AgentOrchestrator.cs[1103-1106]
- src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[660-664]
## Recommended Fix
Establish a trustworthy fresh-session association from the launch and hook data before marking completeness, without marking an unrelated or resumed session complete.

12. Refusals grow daemon memory without limit ✓ Resolved
Description
PermissionRefusalLedger.Record adds each unkeyed tool-call ID to Session.Unkeyed, but never
removes IDs from that set. In a long-running session with distinct refused calls, the set keeps
growing after the declaration's 32-entry list has been trimmed and marked incomplete.
Code

src/Capacitor.Cli.Daemon/Services/PermissionRefusalLedger.cs[58]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The refusal ledger retains every unkeyed tool-call ID for the lifetime of a session, even after its bounded declaration becomes incomplete.

## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Services/PermissionRefusalLedger.cs[18-21]
- src/Capacitor.Cli.Daemon/Services/PermissionRefusalLedger.cs[58-72]

## Recommended Fix
Bound or clear duplicate-detection IDs when the refusal declaration becomes incomplete, while preserving the incomplete flag so later judge requests cannot treat omitted refusals as a complete history.

ⓘ Copy this prompt and use it to remediate the issue with your preferred AI generation tools

13. Terminal refusals disable judge approvals ✗ Dismissed
Description
CodexPtyPolicySeam.RecordRefusal passes call.ToolUseId to the refusal ledger, but Codex
permission prompts do not carry a tool id, so the ledger discards each refusal and marks the session
incomplete. After a terminal-hosted user declines a call, later judge consultations cannot allow
even unrelated calls, and the declined call is absent from the refusal list.
Code

src/Capacitor.Cli.Daemon/Harness/Codex/CodexPtyPolicySeam.cs[68]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
Terminal Codex permission prompts lack `tool_use_id`. Recording their refusals with that missing value permanently marks the session's judge refusal declaration incomplete and omits the refusal.

## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Harness/Codex/CodexPtyPolicySeam.cs[59-69]
- src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[883-901]
- src/Capacitor.Cli.Daemon/Services/PermissionRefusalLedger.cs[49-58]

## Recommended Fix
Provide a unique, usable reference for each terminal Codex approval when forwarding it to the bridge, and retain that reference for refusal recording. Ensure the resulting declarations remain complete when all refusals have been captured; add a test using a Codex permission payload without `tool_use_id`.

14. Live certification bypasses managed temp cleanup ✗ Dismissed
Description
RunAsync creates files and directories beneath TempDir.Path with Directory.CreateDirectory,
File.WriteAllTextAsync, and related direct filesystem calls instead of the TempDir helper
methods. Any setup failure or future path change can bypass the helper's path handling and cleanup
guarantees for the temporary repository and home directory.
Code

test/Capacitor.Cli.Tests.Unit/Harness/Codex/CodexPolicyJudgeLiveCertTests.cs[R102-103]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The live certification test performs direct filesystem operations under `TempDir.Path`, bypassing the `TempDir` helper APIs required for managed temporary test state.

## Fix Focus Areas
- test/Capacitor.Cli.Tests.Unit/Harness/Codex/CodexPolicyJudgeLiveCertTests.cs[102-126]

## Recommended Fix
Replace direct `Directory.CreateDirectory`, `File.WriteAllTextAsync`, `File.Copy`, and related operations under `repo.Path` or `home.Path` with the corresponding `TempDir` methods such as `CreateDir`, `CreateFile`, `CopyExecutable`, or `CreateExecutable`, while preserving the generated paths needed by the Codex process.

15. Recent turns vanish while the watcher lags ✗ Dismissed
Description
Scan retains the newest 16 human lines before ReadAsync filters them against the acknowledged
count. If the watcher remains more than 16 human messages behind when the wait expires, the reader
declares no turns even though older messages have been acknowledged, leaving the judge without that
conversation context.
Code

src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[R44-45]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The reader discards older acknowledged turns before it knows which lines the watcher has acknowledged.

## Fix Focus Areas
- src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[27-82]

## Recommended Fix
Select the newest 16 human lines at or below the acknowledged count, rather than bounding the scan's queue before applying that count. Keep the single-scan approach and add a test where more than 16 newer lines remain unacknowledged.

ⓘ Copy this prompt and use it to remediate the issue with your preferred AI generation tools

16. A reconnect can publish an older count ✗ Dismissed
Description
PublishAckedLines compares and writes the acknowledged count without synchronizing its two
callers. A reconnect can read an earlier count after releasing the rewind gate, then write it after
a concurrent drain publishes a newer count; until the next drain, the hook uses the older frontier
to choose turns for the judge.
Code

src/Capacitor.Cli/Commands/WatchCommand.cs[R54-56]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
The reconnect and drain paths can publish acknowledged counts out of order, leaving the cursor temporarily behind the count already acknowledged.

## Fix Focus Areas

- src/Capacitor.Cli/Commands/WatchCommand.cs[729-732]
- src/Capacitor.Cli/Commands/WatchCommand.cs[4709-4725]

## Recommended Fix
Publish the reconnect count while holding the same rewind gate as the drain, or otherwise serialize the count read, disk write, and cached-count update. Preserve the ability to publish a genuinely lower count after a rewind.

17. An oversized reviewer result blocks judge approvals ✓ Resolved
Description
LocalPermissionBridge calls _refusals.MarkAllIncomplete() for an oversized reviewer
result/message submission or a shared-handler exception, but not when an oversized permission
request is denied locally. Because the ledger applies its permanent _lostRequest flag to every
session, unrelated submission failures block subsequent judge approvals until daemon restart, while
the oversized permission request leaves refusal declarations marked complete.
Code

src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[R476-479]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
Reviewer submission failures can permanently mark permission-refusal declarations incomplete across sessions, while an oversized permission request that the hook denies locally is not marked incomplete.

## Fix Focus Areas
- src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[475-482]
- src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[574-581]
- src/Capacitor.Cli.Daemon/Services/LocalPermissionBridge.cs[742-746]
- src/Capacitor.Cli.Daemon/Services/PermissionRefusalLedger.cs[29-33]

## Recommended Fix
Remove `_refusals.MarkAllIncomplete();` from the reviewer submit 413 branch and correct that line's indentation. Add the call to the permission-request route's `if (body is null)` 413 branch after `ReadCappedBodyAsync(..., MaxPermissionRequestBodyBytes, ...)`. Narrow the shared-handler catch-path marker to permission-hook requests denied before their session could be identified. Consider marking the permission route's malformed-JSON 400 path as well, since the hook denies there too.

18. Large turns consume the judge's deadline ✗ Dismissed
Description
CodexJudgeDeclarationReader.ReadAsync redacts up to 16 covered lines sequentially without checking
the consultation's remaining deadline. With several large user messages, those per-line redactions
can consume the hook's two-second judge budget before ConsultAsync runs, so an otherwise available
judge is skipped and the permission falls back to Codex's prompt.
Code

src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[R47-51]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
Redacting multiple large turns can exhaust the entire hook judge budget before the consultation starts.
## Fix Focus Areas
- src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[47-54]
- src/Capacitor.Cli/Harness/Codex/CodexPolicySeam.cs[100-115]
## Recommended Fix
Pass an overall deadline through the declaration read, bound or stop redaction against that deadline, and go windowless in time to leave a useful budget for the judge request.

19. Live tests can oversubscribe subprocesses ✗ Dismissed
Description
CodexPolicyJudgeLiveCertTests launches real codex, git, and kcap subprocesses but only marks
its methods [NotInParallel]. Multiple live-cert classes or suites can therefore run without the
shared SubprocessLimit semaphore and overload the host.
Code

20. AOT builds can reject a test array ✗ Dismissed
Description
Hook returns a JsonArray from collection-expression syntax instead of constructing it with `new
JsonArray(...). AOT-targeted test builds that cannot lower this JsonArray` conversion will fail or
emit the prohibited pattern in the live certification suite.
Code

test/Capacitor.Cli.Tests.Unit/Harness/Codex/CodexPolicyJudgeLiveCertTests.cs[130]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Fix Focus Areas
- test/Capacitor.Cli.Tests.Unit/Harness/Codex/CodexPolicyJudgeLiveCertTests.cs[130-130]

## Recommended Fix
Replace the collection expression returned by `Hook` with an explicit `new JsonArray(...)` construction while preserving the existing nested hook object structure.

21. Codex turns lose judge evidence ✓ Resolved
Description
CodexJudgeDeclarationReader.HumanTurn declares a message with user.* content kinds even when its
text consists entirely of an <environment_context> block, which kcap-server's CodexNormalizer
discards regardless of content kinds. If a person submits that literal text, the CLI declares an ID
the server never stored, and the server makes the judge's evidence windowless.
Code

src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[102]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A literal user-authored environment-context block is declared as a human turn by the CLI but discarded by kcap-server, so its judge evidence cannot verify.
## Fix Focus Areas
- src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[97-102]
- /cross_repos/kcap-server/src/Capacitor.Server.Services/Sessions/Canonical/CodexNormalizer.cs[191-201]
## Recommended Fix
Update the server normalizer to retain environment-context-shaped messages when their content kinds identify them as user-authored, while continuing to discard synthetic context. Add a shared fixture case that checks both storage and CLI declaration.

22. Malformed patch headers skip path deny rules ✗ Dismissed
Description
CodexHookActions.Normalize downgrades the whole apply_patch call to kind Other in two cases:
when PatchPaths returns null because any one header fails LexicalPaths.TryResolve (an empty or
blank path, for example), and when no header matches the exact, untrimmed StartsWith prefixes.
RuleMatch.Restrictive rejects on a kind mismatch, and path rules exist only for
file_edit/file_read. So a patch that also names a denied path (.env, secrets/**) gets
PolicyOutcome.None at PreToolUse and Codex applies it without being asked.
Code

src/Capacitor.Cli.Core/Harness/Codex/CodexHookActions.cs[R16-23]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
When any patch header fails to resolve, or no header is recognised, `CodexHookActions.Normalize` returns kind `Other`. Path deny rules never match `Other`, so the PreToolUse seam lets the patch through even when it also edits a denied path.

## Fix Focus Areas
- src/Capacitor.Cli.Core/Harness/Codex/CodexHookActions.cs[15-44]
- src/Capacitor.Cli/Harness/Codex/CodexPolicySeam.cs[29-57]

## Recommended Fix
Trim leading whitespace before matching headers. When a header is unresolvable, keep the action `FileEdit` with the paths that did resolve, so their deny rules still apply. Also mark the action so that `PreToolUse` denies it, or at least so an allow can never pass it. Treat an `apply_patch` call with no recognised headers the same way rather than as `Other`. Add tests for: a denied path combined with an empty header, and an indented header.

23. Patch path parser keeps direct type checks ✓ Resolved
Description
CodexPatchPaths.Observe compares ValueKind directly before enumerating changes and extracting
object paths. Any file-change notification handled by this parser follows those checks, bypassing
the shared IsArray and IsObject helpers.
Code

24. Large rollouts delay judge decisions ✓ Resolved
Description
CodexJudgeDeclarationReader.ReadAsync rescans and parses the rollout from its first line on every
100 ms poll for the newest acknowledged human message. On a rollout near the permitted 64 MB size,
one consultation can perform repeated full-file scans outside a scan-time bound, consuming the

25. Continued agents lose judge approvals ✗ Dismissed
Description
AgentOrchestrator sets BridgedSinceStart to false whenever resume is non-null, including a
continuation that opens a new vendor session. That session’s refusal declaration remains incomplete,
so its judge cannot allow an otherwise eligible approval even though this daemon has seen the
session from its start.
Code

src/Capacitor.Cli.Daemon/Services/AgentOrchestrator.cs[2998]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
A continuation uses resume launch machinery but opens a fresh vendor session, which the new freshness flag treats as an old session.

- src/Capacitor.Cli.Daemon/Services/AgentOrchestrator.cs[2995-2999]
- src/Capacitor.Cli.Daemon/Services/AgentOrchestrator.Resume.cs[1036-1046]

Set the freshness flag from whether the vendor session is newly started, not merely whether a `ResumeLaunch` object exists. Preserve incomplete declarations for genuine session resumes and adoptions.

26. New comments preserve design-section coordinates ✗ Dismissed
Description
The changed comments retain references such as §2.3 and add §2.5 to explain current code
behavior. These coordinates depend on an external design document and can become misleading when
that document or implementation plan changes.
Code

src/Capacitor.Cli.Daemon/Harness/Codex/CodexAppServerHostedAgentRuntime.cs[R552-553]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

27. Test environment state can leak between tests ✓ Resolved
Description
PreToolUse_answers_a_deny_rule_in_every_mode directly writes KCAP_HOSTED_APPSERVER with
Environment.SetEnvironmentVariable instead of using an environment scope. The write and
restoration span the test body, so failures or concurrent environment readers can observe altered
process state.
Code

test/Capacitor.Cli.Tests.Unit/Commands/Harness/CodexHookCommandTests.cs[R364-373]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Fix Focus Areas
- test/Capacitor.Cli.Tests.Unit/Commands/Harness/CodexHookCommandTests.cs[357-375]

## Recommended Fix
Replace the manual save, `Environment.SetEnvironmentVariable` calls, and `try/finally` restoration with `Helpers.EnvScope.Exclusive("KCAP_HOSTED_APPSERVER", hostedAppServer ? "1" : null)` and keep the test protected by the bare `[NotInParallel]` attribute.

28. Turn reader keeps direct type checks ✓ Resolved
Description
CodexJudgeDeclarationReader uses direct ValueKind comparisons throughout HumanTurn, Text,
and Is for objects, arrays, and strings. Transcript parsing reaches these checks for every
candidate user-message line, so the new reader duplicates the shared JSON shape logic instead of
using the available helpers.
Code

src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[R92-103]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description

- src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[92-103]
- src/Capacitor.Cli/Harness/Codex/CodexJudgeDeclarationReader.cs[114-129]

29. New Codex hook keeps direct type checks ✓ Resolved
Description
Patch compares ValueKind directly for the input object and command string instead of using the

type-checking convention.

30. Bridge failures leave refusals untracked ✓ Resolved
Description
LocalPermissionBridge marks fresh Codex sessions complete for judge declarations, but a bridge

37. Batch size estimate ignores the new turn id ✗ Dismissed
Description
AcpEventEnvelope gains a serialized TurnId string, but AcpTranscriptForwarder.EstimateBytes
still sums only the older string fields, so the new field isn't counted. Every non-ephemeral Codex

38. Saved acknowledged-line count can stay too high after a rewind ✓ Resolved
Description
PublishAckedLines sets _ackCursorWritten = state.LinesProcessed even when WatchAckCursor.Write
swallows an I/O failure and leaves the on-disk count unchanged. If a reconnect rewind fails to
replace a higher count, later publishes at the same value are suppressed, so the hook can declare
turns beyond the server’s acknowledged position and the judge’s evidence read fails instead of going
windowless.
Code

39. Terminal Codex decisions are logged under the wrong seam ✓ Resolved
Description
CodexPtyPolicySeam records both its judge requests and its decision events with
PolicySeams.HostedCodexApproval, the seam this PR defines for app-server approvals. The terminal
lane is the hook's PermissionRequest bounced through LocalPermissionBridge, and its
consultations are windowless with bridge refusals. The server therefore cannot tell these decisions
from app-server ones, and any per-seam handling or audit filter on hosted_codex_approval sees a
mix of both lanes.

🟠 Major · Clamp watched-session judge allows to ask. · CodexPolicySeam.cs:35-100

src/Capacitor.Cli/Harness/Codex/CodexPolicySeam.cs:35-100
🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Clamp watched-session judge allows to ask.

A self-started Codex PermissionRequest with no matching rule can receive a judge Allow, and the seam writes {"behavior":"allow"}. This violates the contract that this lane may deny or ask, but never allow. Map only a judged Allow to Ask; keep explicit rule allows unchanged.

Suggested fix

+ ? PolicyOutcome.Ask

🟡 Minor · Delete the ack cursor before the pid file is retired. · WatchCommand.cs:1248

src/Capacitor.Cli/Commands/WatchCommand.cs:1248
🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Delete the ack cursor before the pid file is retired.

RemoveOwnPidFile runs first (Lines 1246-1247). After it, a hook can see no live watcher. For example, a Stop hook can call EnsureWatcherRunning. The new watcher connects and calls PublishAckedLines.

This watcher then runs RetireAckCursor and deletes the file that the new watcher just wrote. The new watcher caches _ackCursorWritten. It does not rewrite the file until its line count changes. Until then, CodexJudgeDeclarationReader finds no count and returns no_ack_cursor. The judge runs windowless.

Move the delete above RemoveOwnPidFile. The delete then runs while the pid file still blocks a second watcher.

Proposed fix
- watchers.RemoveOwnPidFile(
-            agentId is null ? sessionId : $"{sessionId}-{agentId}", Environment.ProcessId);
-        RetireAckCursor(sessionId, agentId, vendor);
+        RetireAckCursor(sessionId, agentId, vendor);
+ watchers.RemoveOwnPidFile(
+            agentId is null ? sessionId : $"{sessionId}-{agentId}", Environment.ProcessId);

The server hashes Codex lines with its own implementation, so TranscriptIdsTests also pins one redacted line to the literal id kcap-server's normalizer test pins.
