Retire only one identical prompt per completed call
When multiple no-ID Claude permission requests have the same tool name and input, a single transcript result satisfies this predicate for every request raised before it, so WithdrawSettled withdraws them all. This can occur with identical parallel calls when the direct hook notice is missed, causing the still-running call's prompt to be withdrawn and effectively denied; the daemon-side matcher in PermissionPromptBroker.TryWithdrawTool avoids this by consuming only the oldest matching prompt. Pair each settled call with at most one pending invocation rather than treating it as reusable evidence.

Add tests for the reset and switch paths and for multi-match behavior.
The test covers the main match cases. It does not cover _settledCalls.Clear() on a feed switch or reset. A missing clear would leave an old call that withdraws a card for a new transcript. The test also has no case for two identical cards and one settled call.

Claude's PermissionRequest hook carries no tool_use_id, so the id both withdraw paths keyed on was never on the prompt. PostToolUse carries the same tool_input object, the one key present on both sides. Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>

) A completion notice carries no per-call link to the prompt it answered, so an allowed call's late report could pass for the terminal answer to a newer identical prompt and deny a live hook. The ledger of owed completions and the subagent scope keep the match to the call the user answered; the app's own name-and-input match had the same ambiguity with less to go on, so the daemon decides alone. so identical tool name and input do not let FindIndex assign an unordered

and avoid consuming either, or use a per-call correlation value shared by the

is unambiguous.

match and withdraw an identical prompt registered in a later turn; retain

Stop is not proof that every completion report has landed: the reporting hook is asynchronous, and the app withdraws on transcript evidence ahead of it. Forgetting an owed report would let it pass for a terminal answer and deny a live hook, so the ledger lapses on a grace period, an overflowing scope suspends the fallback, and only Claude's allows are recorded. …1603) A prompt still pending at Stop may have been allowed in the terminal with its report overtaken by Stop, and a call allowed while its scope is suspended reports on its own clock, not the earlier turn's. Both are completions the ledger must not forget while the fallback can still act on them. Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>
The terminal can answer a prompt the daemon still shows, so a deny or a withdrawal settled here does not prove the tool never ran; only the completion report itself settles without owing one. Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>
A gone agent reports nothing more and ends no more turns, so an entry owed on its behalf could neither be collected nor lapse and would outlive the agent in the broker. Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>
Closes #1603 — AI-3941
What & why
In the desktop app, a Claude permission prompt answered in the agent's own terminal keeps its NEEDS YOU card until the agent's whole turn ends. Claude Code's PermissionRequest hook input has no
tool_use_id(its hooks reference says so, and a dump of the hook's stdin on 2.1.302 confirms it), while the daemon's PostToolUse tool-settled notice keys on that id. PostToolUse does carry the sametool_nameandtool_inputobject plus the subagent'sagent_id, so the notice carries all three and the broker matches an id-less prompt by name and deep-equal input within the subagent scope the hook ran in. A completion has no per-call link to the prompt it answered, so the broker also keeps a ledger of completions each scope is owed: every settlement of a Claude prompt other than by its own report (an allow, a deny or a withdrawal here does not prove the terminal had not already let the tool run), every policy-allowed call, and every prompt still pending when the turn ends adds one; a matching notice consumes it instead of retiring anything, and only an unowed notice falls back to the oldest identical pending prompt. Owed entries lapse 30 s after their turn ends (the reporting hook times out at 5 s). A scope whose ledger overflows loses the fallback instead of forgetting an entry, and stays suspended until a turn ends with nothing discarded since, plus the same grace.Where to look
PermissionPromptBroker.TryWithdrawTool,SettleLockedandOweLocked: a withdrawal answers a live hook with a deny, so the fallback must never take a prompt the user has not answered. Every way the ledger can lose count errs toward leaving a card until the turn's end, never toward a deny.Verification
