What & why

A hosted Muse participant never ran a second flow round: the daemon steered the input into a turn that was still winding down, Muse acked the steer and then rejected the submission in its own log (turn_submit.rejected, already_terminal) without telling the client, so the runtime took the ack for delivery and never fell back to turn/start. The steer now counts as delivered only once a user-message item carrying its command id appears; if the turn ends first, the same text starts a new turn.

Where to look

MuseMspHostedAgentRuntime.SteerAsync: an admitted steer that is neither taken nor outlived within SteerFallbackCap is reported delivered, as an unanswered request already was, so a host that never emits user-message items cannot run an input twice.

Verification

dotnet run --project test/Capacitor.Cli.Daemon.Tests.Unit -- --treenode-filter "/*/*/MuseMspHostedAgentRuntimeTests/*": 67 passed. The two new dropped-steer tests fail against the previous runtime at Assert.That(second.IsCompleted).IsFalse() (the input completed on the ack). Reproduced live on kcap 1.3.0-beta.9 / Muse 1.4.10: flow 139175dfe27f402abe09afffa298ac34 round 2 timed out with no second prompt in the participant's session.

Summary by CodeRabbit

  • Bug Fixes
    • Improved handling of input sent while a turn is in progress. If the turn ends without confirming receipt, the input can be retried in a new turn.
    • Avoided starting an unnecessary new turn when the active turn confirms it received the input.
    • Improved delivery handling when a host does not echo user messages or a steer request times out; these cases are treated as delivered.
    • When a steer is refused, the runtime waits briefly for the active turn to become idle before retrying the input.

Muse acks a steer into a turn that is winding down and then rejects it in its own log
(turn_submit.rejected, already_terminal) without telling the client, so the ack cannot stand
for delivery. Only the user-message item carrying the steer's command id does; a turn that
ends first dropped the input.

📝 Walkthrough

The runtime tracks active-turn steer requests and matches them to user-message notifications by command ID. It waits for confirmation or turn completion, then either starts a fresh turn or reports the input delivered. Tests cover matching, missing, and mismatched notifications and turn completion ordering.

Merge Risk: ⚪ Minimal · up to ff2cd

Dropped steers now start a new turn, and hosts that never echo user messages keep the earlier behavior of reporting the input delivered. No concrete merge-blocking risk remains in the supplied evidence. Behavior against a real Muse host was not exercised in the review.

Security architecture risk: 🔵 Low · up to ff2cd

The change adds recovery without adding permissions or a new external entrypoint. The main unresolved risk is repeated execution if delivery confirmation can arrive after fallback begins. Existing serialization and conservative timeout behavior limit exposure to the existing session.

Retained concerns

  • Medium · reliability · inferred: The new admitted-steer fallback depends on turn replacement or completion proving that an unconfirmed input was not consumed. WaitForTurnEndAsync also completes when another turn becomes current; fallback then submits the same text under a fresh command ID and clears the original correlation state. If the original matching item can arrive later, the instruction could execute twice with the session's existing authority. The base did not retry an acknowledged steer. Exact-ID matching and conservative timeout handling reduce this risk, but do not establish the missing producer-ordering guarantee. Replacement-turn tests demonstrate the new path, not production host safety.
Security review details

Security Blast Radius

  • inferred — The supported exposure is repeated or conflicting work within the existing hosted session, using its already-established permissions. Pending confirmation is per runtime and local deliveries are serialized. The inspected change does not establish a new cross-tenant route or grant additional tool authority.

Trust Boundaries and Controls

  • observed — Host notifications remain inputs from the existing process stream, and matching an item only completes local delivery-confirmation state. Factory-side reviewer sandbox availability checks remain separate from this mechanism; steer confirmation does not grant approvals or change session permissions.

Hardening Proposals

  • proposed — Establish an explicit host contract for consumption-event ordering and turn/start behavior during replacement turns. If ordering cannot prove non-consumption, use an authoritative delivery-status or logical-operation deduplication mechanism before resubmitting potentially side-effecting instructions.

AI Description

• Treat a matching user-message item, not a steer acknowledgment, as proof Muse took the input.
• Start a new turn if the steered turn ends without taking that input.
• Cover dropped, accepted, and unrelated-message steers in unit tests.

High-Level Assessment

Correlating the command ID with Muse's user-message notification is the appropriate delivery signal because an acknowledgment alone does not prove the turn took the input. Retaining bounded, no-retry behavior when delivery remains uncertain avoids running the same input twice.

Confirm steered input before treating it as delivered

• Tracks a pending steer's command ID and recognizes its matching user-message notification. If the turn ends without that notification, the runtime starts a new turn with the same text; unanswered and unconfirmed steers retain bounded, duplicate-averse behavior.

Remediation recommended

1. A later turn can hide a dropped steer ✓ Resolved
Description
SteerAsync calls WaitForTurnIdleAsync after the acknowledgement, so its completion tracks
whichever turn is current then rather than the turn identified by turnId. If the steered turn
completes and another turn starts before a delayed acknowledgement arrives, the input waits on that
other turn and can be reported delivered after the fallback cap without starting the needed new
turn.
Code

+            var ended = WaitForTurnIdleAsync(_ownerToken);

Recent runtime precedents accept lifecycle races where waits can observe stale or subsequent turn
state.

PR-#582
PR-#588

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The pump handles responses and lifecycle notifications as separate frames. A completion clears the
matching running turn, and a subsequent start installs a new one; the new wait checks only that
current running-turn state, so it can attach to the later turn. The test suite also models a user
message from another client's command in the same session.

## Recommended Fix
Register a completion signal for the specific steered turn before sending the request, and race that signal against the matching user-message item. Do not use the current turn's generic idle waiter as evidence that the steered turn ended.

3. The merge subject lacks an issue reference ✗ Dismissed
Description
The commit subject Start a new Muse turn when an admitted steer is dropped does not end with a
GitHub issue reference in the required (#digits) form. The subject is therefore not suitable for
the repository's enforced merge-subject format even though it is concise and imperative.
Code

4. Timed-out steers retain idle waiters ✓ Resolved
Description
SteerAsync times out the outer Task.WhenAny wait but never cancels the WaitForTurnIdleAsync
task registered in _idleWaiters. When an admitted steer has no matching item and its turn remains
active past the fallback cap, each later input can leave another waiter retained until the turn
eventually becomes idle or the session ends.
Code

src/Capacitor.Cli.Daemon/Harness/Muse/MuseMspHostedAgentRuntime.cs[R708-711]

+            var ended = WaitForTurnIdleAsync(_ownerToken);
+
+            try {
+                await Task.WhenAny(taken.Task, ended).WaitAsync(SteerFallbackCap, _time).ConfigureAwait(false);
Relevance

●● Moderate

The waiter-retention concern is plausible, but no close precedent confirms this exact timeout
cleanup pattern.

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The idle-wait method adds a waiter to _idleWaiters and removes it on cancellation, while clearing
the turn releases and removes all waiters. The added timeout applies only to the outer race and does
not cancel the owner token supplied to the idle wait, leaving that registration in place for an
active turn.

- src/Capacitor.Cli.Daemon/Harness/Muse/MuseMspHostedAgentRuntime.cs[614-640]

The wait tracks the steered turn rather than session idleness, so a next turn already running
cannot hold the input; the idle waiter is cancelled when the wait ends; and the fallback to a
new turn needs a host that has echoed a user-message item before, since a host that never shows
one leaves the ack as the only evidence and a second turn could run the input twice.

Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>
A next turn announced before the steered turn's completion would otherwise leave the wait on
the old turn unreleased, since completion only clears the turn that is still current.
