When Claude creates or completes several tasks in one parallel tool batch, each PostToolUse launches a separate kcap process, and Claude documents that these events fire concurrently. Every process can therefore pass this File.Exists check before any reaches RecordNudge at line 151, causing the supposedly once-per-moment reminder to be injected multiple times. Use an atomic create/claim operation in the ledger before writing the response, while removing the claim if emission fails.

Fixed in 4a35885: the reminder is claimed by creating its marker with FileMode.CreateNew before it is emitted, and released if the write fails. A test runs 8 concurrent claims and expects exactly one to win.

Follow-up of #1312. Claude Code on current models has no task tools by default, and Superpowers keeps progress in a
progress.mdledger appended from Bash, so plan tasks never reach kcap-plans, or reach it and then go stale. A synchronous PostToolUse hook (kcap hook --claude --task-list) watches the native task tools, ledger writes, implementer dispatches and kcap-plans writes:set_plan_tasks/update_plan_tasklast returned, and aTask N: dispatched|completeline orImplement Task N:dispatch the plan lags gets one reminder naming thetask_idand status.Local only, no subagents, same gate and opt-out as the other plans nudges.
