Implement "Milestone-Driven" prompting strategy

Description:
Update System Instructions to adopt the "Topic -> Action -> Summary" pattern. Inhibit narration of routine tool calls (reads/lists) and focus on high-level task summaries. The goal is to make the agent "quiet by default" unless a significant milestone is reached.
