Android Custom Commands: POC cleanup and premium gating

1. Clean up POC code, add AreHostsConnectedToFleetMDM test
2. Premium gating for LOCK / RESET_PASSWORD
3. Update LockAndroidHost, WipeAndroidHost, and ClearAndroidPasscode to populate raw_command when inserting
4. Unit + integration tests for the full api flow
   POST /api/v1/fleet/commands/run → AMAPI → pub/sub callback → DB status update
