The comment // ref keyword can also appear before the this keyword is obsolete as this keyword is not used in this example.

The comment should be updated to reflect that account is the receiver (analogous to the parameter with the this modifier) in the old-style extension method. And, because the extension block declares ref Account account, the receiver is a reference parameter.
