fix(conversation): a timestamp-only touch of a transcript is not an in-place rewrite - #3227
transcriptGeneration() treated any same-size change of mtime/ctime on the same inode as an in-place rewrite and gave the transcript a fresh generation (randomUUID() suffix on history_id). A running Claude Code sets its open session file's times roughly once an hour without writing a byte (same size, same content, same inode). Each touch therefore changed history_id for every open Claude pane, which invalidates the response cache and settled turns, makes page cursors fail with HistoryChanged (409) and a reload, and can reject a completion proof taken against the earlier id. After a server restart the in-memory suffix is gone, so the id changes again.
Synthetic reproduction: write a JSONL transcript, read its history_id, then utimesSync it to a later time without changing its bytes; the next read returns a different history_id.
Fix: keep a running sha256 of the bytes read so far, fed only what each growth appends. When the size is unchanged but the timestamps changed, digest the whole file and compare: equal bytes are a touch and keep the generation; different bytes are still a rewrite. Bytes that cannot be read still count as a rewrite. To stay within the transcript read budget (bounds total OMO output reads to the transcript tail), only a log within TRANSCRIPT_WINDOW_BYTES keeps a digest; a larger log's touch counts as a rewrite, as before. Shrinking, inode changes and the Muse window check are unchanged.
Validation
New test in server/conversation.test.ts: keeps a transcript's history_id across a touch that leaves its bytes alone (touch, growth, touch keep the id; a same-size one-byte edit is a rewrite; a later touch keeps the rewritten id; a cold read drops the in-memory generation). It fails without the fix and passes with it.
bun test server/conversation.test.ts: 204 pass, 7 skip, 0 fail.
bun run generate:types --check, bun run generate:themes --check, bun run typecheck, bun run build: pass.
bun run test:unit on macOS: 44 failures across 18 files, none in conversation/transcript code. The same 18 files give the same 44 failures on main without this change (environment-dependent: self-hosted runner, CI browser, Codex/Kimi store, bridge and similar), so they are not caused by this PR.
Not run: bun run check full (integration and browser lanes).
…n-place rewrite
Claude Code sets its open session's times about hourly without writing a
byte. A same-size change of mtime/ctime alone counted as a rewrite, so every
Claude pane got a fresh generation (history_id) hourly, invalidating cursors,
caches and read proofs. Keep a running sha256 of the bytes read so far (fed
only what each growth appends) and compare the whole file's digest when the
size stays and the timestamps change: equal bytes are a touch. Unreadable
bytes still count as a rewrite. Only a log within TRANSCRIPT_WINDOW_BYTES
keeps a digest, so reads stay within the transcript budget; a larger log's
touch still counts as a rewrite, as before.

A null result does not show that the image lookup created a leased stream. The final history_id assertion can also pass if the leased stream reads the digest synchronously.
Use an image lookup that succeeds, and assert that it does not extend the digest synchronously. Keep the page assertion to check that the later read extends it.

Bug Fixes
Fixed an issue where open Claude Code chats with transcripts up to 16 MB could reload their history about once an hour when the transcript’s timestamp changed but its contents remained unchanged. History now stays intact in this situation, while actual transcript content changes continue to be detected.
Transcript generation now compares digests when timestamps change but file size stays the same. It tracks appended bytes for eligible files and avoids synchronous hashing for leased transcript reads. Tests cover history IDs across touches, appends, rewrites, and cleared state.
The revision logic hashes transcript bytes and extends the digest for appended bytes. When timestamps change without a size change, it compares content to identify rewrites. Tests cover history IDs across touches, appends, rewrites, and cleared state. The changelog describes the reload scenario.
transcriptStream accepts a leased-read flag and skips synchronous generation hashing for leased reads. The initial and post-append Claude image streams pass their lease state.
devswha/ovis#2604: Adds cooperative Claude image scans; this change passes leased-read state through those scans.
devswha/ovis#2027: Checks the previous Muse transcript window for rewrites during growth, which is retained in the generation calculation.

Test the large-log touch fallback.
For a transcript larger than TRANSCRIPT_WINDOW_BYTES, assert that a timestamp-only touch changes history_id. The current touch test uses only a small transcript, so it does not detect a regression that preserves the ID when no digest is retained.

…n of the file stat saw, never on a leased image read
Review follow-up for devswha#3227: hashRange opens with O_NONBLOCK and requires the regular file stat saw (identity, size, times) before and after reading, so a FIFO swapped in is not waited on and a replaced path is not digested. A leased (async) image read no longer reads the file synchronously for the digest; it drops it and the next page read rebuilds it.
Thanks @bonecool999 for the clear write-up and the failing-first test. Maintainer follow-up pushed to this branch (merge of main plus 48ee5e0):
hashRange now opens with O_NONBLOCK and requires the regular file stat saw (dev, ino, size, mtime, ctime) both before and after reading. A FIFO swapped in at the path is not waited on, and a replaced path is never digested (CodeRabbit's identity thread).
A leased image read (transcriptImageAsync, whose bytes come by async reads) no longer reads the file synchronously for the digest. It drops a digest it would have to extend, and the next page read rebuilds it.
The changelog fragment states the 16 MB limit: a larger transcript's touch still counts as a rewrite, as before.
Verification: the new test fails on main (the first touch adds a generation suffix) and passes here; bun test on conversation, claude-images, chat-history, copilot, conversation-search and claude-host-tools gives 391 pass, 0 fail; bun run check fast passes (12873 pass, 0 fail).
Known limitation, reproduced: when a transcript has earlier bytes rewritten in place while it also grows (same inode), and later a same-size write restores the earlier bytes, the running digest matches and the generation is kept, so a page cached from the intermediate bytes can still be served. main already misses the first step (a rewrite that grows) for every source except Muse, which keeps its window check; it only recovered on the later timestamp change. Claude Code only appends to its session file, so no supported agent writes this pattern. Closing it fully means re-digesting the whole prefix on every growth. Measured on a 15 MiB transcript, that raised a growing page read from about 1-2 ms (same as main and this PR) to 23-28 ms, so it is left out.
…of restarting it
Review follow-up for devswha#3227: hashRange only needs the same regular file with at least the bytes it digests, so an append during the read no longer drops the digest and restarts it from byte 0 on the next poll. The digest records how many bytes it covers, so a leased image read leaves it for the next page read to extend instead of dropping it.
Note: CodeRabbit is an incremental review system and does not re-review already reviewed commits. This command is applicable only when automatic reviews are paused.
Second maintainer follow-up, 405686d: the digest now tracks how many bytes it covers, and hashRange only needs the same regular file holding at least the bytes it digests. An append during the read no longer drops the digest and restarts it from byte 0 on the next poll, and a leased image read leaves the digest for the next page read to extend. A new test (keeps a transcript's history_id across a touch after an image lookup saw it grow) fails at 48ee5e0 and passes here.
Validation at 405686d: bun run check fast passes (12874 pass, 0 fail); related server tests give 392 pass, 0 fail. CodeRabbit did not review 48ee5e0 or 405686d: it answered "Review failed" to @coderabbitai review, so its last review covers 52c1ec4 only. The rewrite-while-growing limitation from the previous comment still applies.

@devswha Thanks for the update. Based on your report, the last completed review covered 52c1ec44a, not the two follow-up commits. The rewrite-while-growing limitation remains an accepted trade-off.

Learning: Within repository "devswha/ovis", only for files matching one of ["server/conversation.ts"]: When reviewing transcript generation and cached pages in server/conversation.ts, treat a same-inode rewrite during growth followed by a same-size restoration as a known limitation: it can retain the history_id and serve a page cached from intermediate bytes. Do not require a full-prefix digest on every growth solely for this pattern while no supported agent writes it; reassess if a supported agent can write this pattern.

Learning: In server/conversation.ts, readCommandTranscript intentionally reuses parsed records only for verified same-file appends. Changed snapshots still read the bounded 16 MiB tail and hash its retained overlap to detect rewrites; inode changes and overlap mismatches trigger reparsing. Incremental parsing does not eliminate the synchronous bounded read and hashing cost.
Thanks again @bonecool999 for finding this and for the clear write-up. We are parking this PR until after the next release; it is not abandoned.
What is left before it can merge: the rewrite-while-growing case described above (earlier bytes rewritten in place while the file grows, then a same-size write that restores them) still keeps the old generation, and our rule is that a reproduced HIGH blocks a merge. The idea to try next is cheaper than re-hashing the whole prefix on every growth: keep a hash of the first 64 KiB plus the size from the last read, and on a growth of the same inode re-hash only that 64 KiB. If it changed, bump the generation and read the file fully; if not, keep the fast path. That needs a measurement showing growth reads stay around 1-2 ms, a failing-first test for that case, and another adversarial review.
No action is needed from you; we will pick it up from here.
