Closes #158.

A count the completed Runs have spent now holds the item even while another Run is queued or in progress. Before, a Run in flight made the item wait, so a refused Run that stayed queued could hide attempts_exhausted or run_ceiling. The rulings are on #158. No new decision record: decision 0003 gains a row.

What changes

  • kernel/brakes.py: the ceilings are asked apart from the deferrals. run_ceilings covers a Run's attempts, its rounds and the Run ceiling; approval_ceilings covers an approval's attempts and the Run ceiling. on_run and on_act call them after the Pause, as before.
  • kernel/decide.py: while a Run is in flight, _decide asks only those ceilings, counted over completed Runs.
    • A Pause, a Pause that can't be read, or an unreadable count no longer hides a spent count.
    • A superseded Run is no attempt.
    • An act, including the act ceiling's Hold, waits for the Run. Taking the act ceiling's Hold there made an undeclared Lifecycle move (held → adjudicating by a Lens).
  • sim/world.py: uncovered reviews run on one seed in three, not on every seed as ruling 6 asked. At spread 1, the first 40 seeds (CI's mutant runs) no longer reach two paths the coverage tests ask for; the comment names them. Filed as Simulator: uncovered reviews on every seed, with the first 40 seeds still reaching every path #168, "Simulator: uncovered reviews on every seed, with the first 40 seeds still reaching every path". Seed 297 is covered and now ends with r4 superseded.
  • Mutants: one for each branch on the in-flight path, and one each for asking on_run or on_act in place of the ceilings. 21 were checked by hand, and all are killed.

Left out

Verified

  • make verify passes (655 tests) on a git archive export of 6ed8a96 with PATH=/usr/bin:/bin, after merging main. Every mutant target appears exactly once.
  • The simulator passes at 300 seeds on 8fea0ce.

Reviewed by: a fresh-context Claude review and a security pass on 64d50d4, and AGY (Gemini 4 Argon) on 64d50d4, then again on the fix round 8fea0ce, where it found nothing ("Approve"). The fix round answered:

6ed8a96 only merges main (#166). The one conflict was the mutant list, where both sides were kept.

Summary by CodeRabbit

  • Bug Fixes
    • Items now honor completed Run and approval limits even while another Run is in progress. Waiting and in-progress Runs do not count as completed attempts, and superseded Runs are excluded.
    • Acts wait while a Run is in flight when the act limit is reached; completed approvals remain enqueued.
  • Tests
    • Added coverage for in-flight Runs and attempt limits, and updated simulator scenarios for uncovered acceptance criteria.

Closes #158. With a Run in flight, `_decide` returned `nothing, working`
before the brakes were asked, so a count already spent was hidden while
the Run waited: seed 297 left `held` for `verifying` with no declared move.

Now the wanted Run is worked out first. With a Run in flight the brakes
are asked about it, or a converged item's act, against the completed
Runs since a writer acted, and only a Hold they give is taken. A Pause's
deferral keeps the item working, as before; a superseded Run is still no
attempt. The act ceiling waits for the Run too: it counts acts, stops
only an act, and holding on it while a Run waits made an undeclared move
(held -> adjudicating by a Lens) in tests/test_transitions.py.

Boundary tests from seed 297's facts, at the Run ceiling, and for the
act ceiling; three mutants. Decision 0003 gains one Boundaries row.
The simulator's uncovered reviews widen from one seed in six to one in
three, which takes in seed 297. On every seed all 300 pass, but the
first 40, which a local run and the mutation check use, no longer
reach an approval under a Pause or acceptance evidence missing.

… waits

The review round on 64d50d4. With a Run in flight, `_decide` asked
`on_run` and `on_act` and took only a Hold, but both read the Pause
first, so a Pause hid a spent count and the item left `held` for
`verifying, working` with no writer acting.

`kernel/brakes.py` now holds the ceilings apart from the deferrals:
`run_ceilings` (attempts, rounds, the Run ceiling) and
`approval_ceilings` (an approval's attempts and the Run ceiling).
`on_run` and `on_act` call them after the Pause, as before. While a Run
is in flight, `_decide` asks the ceilings alone, against the completed
Runs. The guard on the outcome and the act-ceiling filter go: an act,
the act ceiling's among them, is not asked about while a Run waits, nor
are an approved item's failed approvals.

Boundary tests for a Pause, one that cannot be read and an unreadable
count of acts while a Run waits, and for r4 superseded. Two
test_transitions bases at spent attempts with a Run in flight. Seven
mutants on the in-flight path; two brakes mutants follow the move.
Decision 0003's row names the new tests, and sim/world.py names the two
assertions that fail at spread 1 on 40 seeds.

📝 Walkthrough

The kernel now checks completed Run ceilings while a Run is in progress. The change extracts ceiling checks, adds decision and transition tests, updates mutant targets, and changes the simulator’s uncovered-criterion fault frequency from every sixth seed to every third seed.

Explanation

The changed kernel code, decision record, tests, transition bases, mutant catalog, and simulator coverage support #158's decision behavior, testing, or documented coverage requirement. No unrelated implementation change is established.

Full details: Linked Issues check

Explanation

#158's first two coding requirements are implemented: kernel/decide.py checks completed Runs with approval_ceilings or run_ceilings while a Run is in flight, and the PR adds boundary tests and mutants. The simulator requirement is not met. sim/world.py sets UNCOVERED_SEEDS to 3, not every seed or the earlier required spread. Its comment states that the first 40 seeds miss paths required by other tests. A 300-seed invariant pass does not satisfy that coverage requirement.

Resolution

Set UNCOVERED_SEEDS to every seed or to the spread previously required by #158/``#16. Keep the boundary test and mutant, then verify the simulator paths and invariants across 300 seeds.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Allow an in-flight ceiling Hold during a Pause.

World.step() calls World.paused() whenever self.pausing() is true. World.paused() recomputes the unpaused decision, and its check at sim/world.py:615-618 currently requires a Pause deferral for every ceiling Hold. However, kernel.decide._decide() returns the ceiling Hold directly when an in-progress Run has exhausted completed-Run attempts or approvals. That Hold must remain visible during the Pause. Keep the Pause deferral check for admissions and acts, but exclude this in-flight ceiling Hold from it.

Assert the reason in both below-ceiling cases.

These assertions accept any reason with the stated state, outcome, and attention. Assert the expected reason for each case so a wrong deferral reason fails the boundary test. As per coding guidelines, “Assert the outcome and the reason code.”

Adversarial Code Review against config/rubric-pr-review.json

Outcome: PASS

Evaluated commit 6ed8a96:

• PR-1 (Requirements, Major): PASS. Accurately implements Issue #158 specifications and the Solution Architect rulings. In-flight runs honor completed run ceilings, separating ceiling checks from deferrals (run_ceilings, approval_ceilings) in kernel/brakes.py. Associated items (such as the simulator spread) are accounted for with follow-up Issue #168.
• PR-2 (Security, Blocking): PASS. All added code in kernel/ remains pure functional logic with zero side effects, zero I/O, and zero permission violations.
• PR-3 (Functional, Major): PASS. Added unit tests (test_a_run_in_flight_draws_a_hold_at_spent_attempts, test_an_act_waits_for_a_run_in_flight, test_a_run_in_flight_is_held_by_the_completed_runs_alone) cover the new behavior. All 7 new in-flight mutants and 2 updated brakes mutants in tools/mutants.py are verified killed by AssertionError in tests.
• PR-4 (Architecture, Blocking): PASS. Zero I/O or clock reads in kernel/. check_purity passes cleanly with 0 violations.
• PR-5 (Requirements, Major): PASS. Decision 0003 Boundaries table row correctly updated with boundary tests. check_context and check_decisions pass.

CodeRabbit, on PR #170. `World.paused()` required the Pause's deferral
wherever the unpaused decision was a ceiling Hold. Since #158 the Kernel
gives that Hold under a Pause while a Run of the item is in flight, so
the oracle would have flagged correct behaviour as "a Pause did not
defer". The 300 seeds did not reach it.

Now, while a Run of the item is in flight (admitted and not completed,
as the Kernel reads it), the ceiling Hold must show through the Pause
unchanged, and nothing is added to `stopped`: the Pause held nothing
back. Otherwise the deferral is still required, as before.

A test calls `paused()` with seed 297's Runs, the fourth queued or in
progress, under a Pause and one that cannot be read. It fails on the
old oracle and passes on this one, and it checks both sides: a hidden
Hold in flight breaks the invariant, and with no Run in flight a Hold
in the Pause's place still does.

Codex, P2 on #170. The in-flight path's `run_ceilings` call holds an
adjudicating item at `max_rounds` while a Run is queued or in progress,
but no test reached it: `test_rounds_are_bounded` has no Run in flight.

A boundary test: at `max_rounds`, with a ruling queued and one in
progress, the item is held; one round short, it is working; with no Run
in flight, held and admitted. A mutant on the in-flight call only, which
gives it a `max_rounds` the completed Runs cannot reach: the new test
kills it, and the older tests do not. Decision 0003's row names the test.
