    Dev-Lead Implementation Plan

    Issue: #189 β€” [Fleet Monitor] petry-projects/repo-template β€” .github/workflows/ci.yml

    Diagnosis (from the flagged runs)

All 6 flagged runs failed identically in the coverage job, step
    Coverage (shell/bats via kcov):

sudo apt-get install -y -qq bats kcov
E: Unable to locate package kcov
##[error]Process completed with exit code 100.

kcov is not in the Ubuntu runner's apt repositories, so every run died at
install. This is the same root cause as issue #152 (the prior Fleet Monitor
    flag for this workflow).

    Key finding

    • Every flagged failing run is on the outdated, unmerged branch
seed-scaffold/2026-09-07 (PR chore: seed org baseline file scaffoldΒ #188)
, whose ci.yml still installs kcov
      and pins actions/checkout@v6.0.2.
    • main already carries the [Fleet Monitor] petry-projects/repo-template β€” .github/workflows/ci.ymlΒ #152 fix. Its coverage job is
Coverage (shell/bats), installs only bats (with an apt-get update retry),
and never references kcov. main-branch CI is green.
    • Two regression guards already assert the fix and currently pass:
      tests/ci-apt-retry.bats and tests/ci-runner-setup.bats
("ci.yml never apt-get installs the uninstallable kcov package").

    Scope

Diagnose the flagged failure, confirm the fix + regression guards are present on
main, and verify all three CI jobs pass on the current tree. No source change
is warranted β€” the defect is already remediated on main; the flagged failures
originate from an outdated branch (#188) that predates the fix.

    Verification Checklist

    • Reproduce the failure signal from the run logs (kcov β†’ exit 100)
    • Confirm current ci.yml coverage job installs only bats, no kcov
    • Confirm regression guards exist and pass
    • Run all three CI jobs locally (build-and-test, coverage, secret-scan)
    • Run mandated .dev-lead/scripts/dev-lead-lint.sh

    Test Command

bats tests/**/*.bats (plus gitleaks detect for the secret-scan job)

Dev-Lead: Progress (in progress β€” not yet durable)

Provisional. Nothing is pushed yet; the automation posts the authoritative
record (with PR number + head SHA) after any push. Treat the notes below as
    intent/analysis, not delivered results.

    Diagnosis

All 6 flagged runs failed identically in the coverage job at
apt-get install -y -qq bats kcov β†’ E: Unable to locate package kcov
(exit 100). kcov is not in the Ubuntu runner apt repos. This is the same
root cause as the prior flag, issue #152
    .

Key finding β€” root cause is already remediated on main

    • Every flagged failing run is on the outdated, unmerged branch
seed-scaffold/2026-09-07 (PR chore: seed org baseline file scaffoldΒ #188)
, whose ci.yml still installs kcov
      and pins actions/checkout@v6.0.2.
    • main already carries the fix. Its coverage job is Coverage (shell/bats),
installs only bats behind an apt-get update retry, and never mentions
kcov. main-branch CI is green (verified via gh run list --branch main).
    • Regression guards for this exact defect already exist and pass:
      • tests/ci-runner-setup.bats β†’ "ci.yml never apt-get installs the
        uninstallable kcov package"
      • tests/ci-apt-retry.bats β†’ "no apt-using job installs the uninstallable
        kcov package"

Local check output (provisional β€” not the pushed run)

    # build-and-test / coverage (bats)
    1..114
    full-suite exit: 0

    # kcov regression guards
    tests/ci-apt-retry.bats + tests/ci-runner-setup.bats: 8/8 ok

    # secret-scan (gitleaks 8.30.1, checksum-verified)
INF no leaks found (exit 0)

    # mandated repo lint
    .dev-lead/scripts/dev-lead-lint.sh β†’ all checks passed

    Files touched (this workspace)

    • None. No source change is warranted: the defect that produced the flagged
failures is already fixed on main and guarded by passing regression tests.
Fabricating a change would violate "do not implement more than the issue
requests / do not modify unrelated files."

Notes / recommended follow-up (outside this repo's tree)

βœ… Auto-resolved by Fleet Monitor. This workflow's failure rate has been below the 10% threshold (or the workflow was removed) since 2026-09-08 (3 days) β€” the monitor stopped flagging it. Closing. It will reopen automatically if the workflow crosses the threshold again.
