Code Review
This pull request introduces a recency-bounded liveness detection mechanism (#1600) to prevent stale pre-incident failure streaks from being reported as live outages. It adds configuration for lookback days, tracks run timestamps, and introduces a new STALE state for unverified repos. The review feedback focuses on test portability, pointing out that the use of the GNU-specific date -u -d command and ISO 8601 parsing will fail on macOS/BSD environments, and suggests using hardcoded epoch integers and space-separated UTC timestamps instead.

Convert ISO 8601 timestamps to space-separated format for parsing, enabling the _el_to_epoch function to work correctly on both GNU and BSD date implementations. This addresses portability feedback on test environments that don't have GNU coreutils. Co-Authored-By: Claude Haiku 4.5 <noreply@anthropic.com>

Steering commit. This PR added an ISO-8601 normalization to _el_to_epoch:
normalized="${v//T/ }"
normalized="${normalized//Z/}"
Both substitutions are global, so they also rewrite characters inside a
timezone NAME. GitHub returns a PAT's expiry as "YYYY-MM-DD HH:MM:SS UTC",
and "UTC" contains a T — the value became "2026-08-01 12:00:00 U C", date
could not parse it, _el_to_epoch returned empty, and pat_expiry_state fell
through to UNKNOWN.
Observable effect: 4 of the 5 pat_expiry_state tests added by #1587 fail on
this branch and pass on main. The one that still passed is the
"unparseable value -> UNKNOWN" case — it expects the fall-through, so it
could not distinguish a genuinely bad value from a mangled good one. That is
also why this looked like an unrelated flake rather than a parser bug.
It matters beyond the tests: #1587's PAT scope/expiry monitoring is exactly
the "silent single point of failure" guard for GH_PAT_DON_PETRY. With every
expiry classified UNKNOWN, an approaching expiry would never raise EXPIRING —
the monitor would have shipped unable to do the job it was added for.
Fix: replace only the `T` that separates date from time (anchored regex) and
only a TRAILING `Z`. Also restores the "%Y-%m-%d %H:%M:%S %Z" BSD fallback
that the rewrite had dropped, keeping the named-zone form parseable there.
Verified locally against BSD date:
2026-08-01 12:00:00 UTC -> 1785585600
2026-08-31T03:19:00Z -> 1788146340 (matches the fixtures' `now`)
2026-08-31T03:19:00 -> 1788146340
garbage / empty -> empty
Regression tests added: the UTC suffix parses, ISO T/Z still parses, both
forms of one instant agree, and unparseable input still yields empty.

Summary
Recency-bounds the engine-liveness monitor (#1600): runs older than a configurable ENGINE_LIVENESS_LOOKBACK_DAYS window (default 3) are ignored when computing the preflight-failure streak; a stub whose newest run predates the window is reported as a distinct STALE (unverified) state that never fails the run; a genuine within-window OUTAGE still fails via a new workflow step gated on ENGINE_LIVENESS_SHOULD_FAIL. Includes 16 new bats tests, report/table updates with last-run timestamps, and AGENTS.md documentation.
Linked issue analysis
Closes #1600. All five acceptance criteria are substantively addressed, each with matching test coverage:
- Streak ignores runs outside a configurable window; default documented —
within_window_streakbreaks at the first out-of-window row (safe: rows are re-sorted most-recent-first, so everything after is older); default of 3 days documented inAGENTS.md, the script header, andENGINE_LIVENESS_DEFAULT_LOOKBACK_DAYS. - STALE is a distinct, non-failing state —
repo_liveness_from_runsreturnsSTALEwhen the newest run predates the window (or no runs exist);run_should_failcounts only within-window sustained OUTAGE repos. - ContentTwin regression shape — test reproduces the exact defect (two failures 3–6 days old, no runs since, now = the first real monitor run) and asserts
STALE. - Both directions asserted in one test — the same test asserts ≥2 within-window failures still yield
OUTAGE, and the new workflow step (if: always() && env.ENGINE_LIVENESS_SHOULD_FAIL == 'true') still fails the run, preserving #1587. - Last-run timestamp in the report —
render_liveness_tablegains a "Last run" column (em dash when empty), with a test.
Findings
No blocking findings. Notable strengths and non-blocking observations:
- The severity ranking correctly places STALE below FAIL (
OK < STALE < FAIL < OUTAGE) — unverified is not failing. gather_runs_tsvre-sorts API rows byrun_started_atdesc (the API orders bycreated_at), fixing a real ordering hazard flagged in review; a 200-with-no-runs stub is now counted as STALE rather than silently skipped — both bot findings were fixed and their threads resolved._el_to_epochnormalization is anchored (regex on the date-timeT, trailing-Zstrip only), preserving the"... UTC"PAT-expiry form; covered by four tests including a GNU/BSD-agreement check.- The failure signal moved cleanly from implicit script behavior to an explicit, gated workflow step; the script itself now always exits 0 on a completed scan (verified — no lingering outage
exit 1in the head script), so STALE cannot hold scheduled runs red. - Workflow change introduces no security smells: no new permissions, no untrusted-input interpolation (
ENGINE_LIVENESS_SUSTAINED_REPOSis a script-computed integer), pinned action SHAs untouched, and this workflow is org-private automation logic (not a frozen thin caller stub —caller-stub-freezecheck passed). - Secret scan: the
run_secret_scanningMCP tool is not available in this environment; the gitleaks CI check passed, and the diff contains no credential-like content. - All 10 review threads (codeant-ai, gemini-code-assist, coderabbitai) are resolved with substantive fixes; CodeRabbit's latest review is an APPROVED.
CI status
All required checks green at b2df87deaf77dc7df3fb55956bc596bc7bfc6ee7: shellcheck/ShellCheck, bats, unit-tests, actionlint, CodeQL, SonarCloud, gitleaks secret scan, AgentShield, caller-stub-freeze, holdout-guard, and all validation gates SUCCESS. One early review / review run was CANCELLED but superseded by two later SUCCESS runs of the same check. No failures.

Summary
Recency-bounds the engine-liveness monitor (#1600): adds a configurable ENGINE_LIVENESS_LOOKBACK_DAYS window (default 3), a distinct non-failing STALE state for repos whose newest run predates the window, and fails the run only on a within-window OUTAGE. Pure logic lives in engine_liveness_detect.sh with thorough bats coverage; the workflow gains a single gated fail step with no new permissions or secret exposure. Triage's low-risk read is confirmed as sound; classified MEDIUM here for the non-trivial logic change, still well within auto-approve gates.
Linked issue analysis
Closes #1600 (engine-liveness monitor reports stale pre-incident failures as a live OUTAGE). All acceptance criteria are substantively addressed: (1) streak computation ignores runs outside the configurable lookback window, default 3 days documented in AGENTS.md and the script header; (2) STALE is a distinct reported state (including workflow-exists-but-no-runs) that never fails the workflow; (3) a repo whose only failures predate the window is STALE, not OUTAGE — the exact ContentTwin reproduction from the issue is a bats test case; (4) a genuine within-window sustained outage still goes red, preserving #1587 behavior. The optional STALE-probe dispatch was not implemented, which the issue marked as optional.
Findings
No blocking findings.
- Correctness: gather_runs_tsv re-sorts runs by run_started_at descending before streak computation (the workflow-runs API orders by created_at), fixing the ordering hazard flagged in review. A workflow that exists (HTTP 200) with zero completed runs is now counted as STALE rather than silently skipped, closing a coverage gap. within_lookback uses an inclusive boundary with a test pinning it.
- _el_to_epoch normalization is anchored (regex on the date-T-time shape, trailing-Z strip only), avoiding the blanket T-substitution that would corrupt GitHub's 'YYYY-MM-DD HH:MM:SS UTC' PAT-expiry form; regression tests cover both forms agreeing.
- Workflow change is minimal and safe: one always()-guarded step gating on ENGINE_LIVENESS_SHOULD_FAIL, running after the fleet-alert step so a FLEET outage still opens its issue before the run goes red. No new permissions, no untrusted input in run blocks (env vars are internally derived integers/booleans). Not a frozen caller stub (caller-stub-freeze check green).
- All 10 review threads (codeant-ai, gemini-code-assist, coderabbitai) are resolved and their fixes are present in the head SHA, including the duplicate OUTAGE case arm and date-portability test rework (hardcoded epochs).
- Minor, non-blocking: _worse uses 'local -A' (bash 4+), consistent with existing mapfile usage in the script — fine on GitHub runners.
- Secret scan: run_secret_scanning MCP tool unavailable in this environment; gitleaks CI check passed and the diff contains no credential material.
CI status
All substantive checks green at b2df87d: shellcheck, ShellCheck, bats, unit-tests, unit, actionlint, CodeQL (actions + python), Agent Security Scan, agent-shield, Secret scan (gitleaks), SonarCloud quality gate, gh-aw-compile, caller-stub-freeze, and all validation gates SUCCESS. One superseded 'review / review' run shows CANCELLED; three later runs of the same check succeeded. CodeRabbit status SUCCESS. Review decision: APPROVED (coderabbitai + donpetry-bot).
