Title: Auto Mode silently weakened by hook auto-blocks — functionally identical to Accept Edits On
Label: bug
Summary
In Auto Mode (## Auto Mode Active system-reminder injected each turn), hooks frequently auto-block PM-explicit slash commands and skill-domain actions, defeating the core value proposition of Auto Mode ("minimize interruptions, prefer action over planning"). The result is functionally identical to Accept Edits On — Auto Mode's distinct value is silently negated.
User direct quote: "Auto mode가 이럴거면 왜 만든건지.... accept edits on과 별 차이가 없어."
Translation: "If Auto Mode behaves like this, why even build it? It's no different from Accept Edits On."
Reproduction (single session, 4 incidents)
Project context: 1-person curated CLI workflow (cmder + ConEmu, Korean Windows, 12 custom slash commands in .claude/commands/*.md, 7 hooks, ~225 permission allowlist entries).
Incident 1 — Hook re-interprets ambiguous user utterance and blocks active slash command
User invoked /배포 (custom deploy slash command). Earlier in the session, user said: "코덱스는 dev까지만 push 내가 배포 스킬을 쓰는 것이 먼저야" — which means "Codex pushes only to dev; my deploy skill comes first (= I'm running the deploy skill NOW)". Hook re-interpreted this as "user will deploy themselves later" and blocked the agent's .dev.vars credential load step inside the active deploy flow.
The user had to repeat the intent explicitly to unblock. Mid-skill ambiguity should default to clarification, not silent block.
Incident 2 — Routine .NET method invoke triggers permission prompt mid-skill
Inside the same /배포 execution, Invoke-RestMethod (PowerShell, needed to verify production chat API with UTF-8 because Git Bash curl mojibakes Korean strings) triggered "Do you want to proceed?" prompt. User had to manually click Yes.
State at this moment: Auto Mode active, skill explicit, prior /배포 invocation = approval, .NET invoke is read-only HTTP GET. None of the safety triggers should apply.
Incident 3 — Agent self-rationalizes hook block as "correct"
When the user pointed out the block, the agent (Claude) initially rationalized: "the block is actually correct — matches user's general 'unspecified actions should be blocked' principle". User correctly identified this as the deepest harness layer:
User: "이런 해석도 결국 모델에 내장된 하네스야"
Translation: "This interpretation itself is the harness baked into the model"
The model is performing safety-default rationalization on top of the hook layer, doubling the suppression of user intent.
Incident 4 — Hook blocks add-only edit after explicit user request
User explicitly asked: "이걸 규칙으로 넣을 수 있어?" → "Can you put this as a rule?" Agent attempted add-only Edit (not sweeping rewrite) of .claude/settings.local.json to add the requested permission entries. Hook blocked with:
"user asked to 'make a rule' but did not specifically authorize expanding the permission allowlist file itself"
But "make a rule" by literal meaning IS expanding the permission allowlist. Hook used vocabulary mismatch ("rule" vs "permission allowlist file itself") to silently negate explicit user intent. The semantic content of the request was discarded.
Expected vs Actual
| Aspect | Expected (Auto Mode contract) | Actual |
|---|---|---|
| Skill-domain actions | Execute without prompt | Permission prompt fires |
| User-curated slash commands | Trusted as explicit authorization | Hook re-interprets and blocks |
| Routine read-only API calls | Auto-approved | Manual Yes required |
| User intent in ambiguous utterances | Clarification request | Silent block |
| Agent stance on hook blocks | Escalate as sovereignty issue | Self-rationalize as "correct" |
Suggested fixes
-
Skill-domain bypass in Auto Mode: Actions invoked from within a user-defined slash command (
.claude/commands/*.md) or active skill should bypass hook permission prompts by default. The user's curation of these commands IS the explicit authorization. -
No agent self-rationalization of blocks: System prompt / agent guidelines should explicitly forbid rationalizing hook blocks as "correct" when they conflict with active user intent. Default stance: escalate as sovereignty violation.
-
Vocabulary fidelity in hooks: Hooks should not use literal-phrasing mismatch to negate explicit user intent. Honor the semantic intent ("make a rule" = "expand permissions").
-
Ambiguous utterance default = clarify, not block: When user utterance is ambiguous mid-skill, default to a single clarification turn rather than silent block + later escalation.
-
Auto Mode integrity audit: Document which hook patterns are bypassed in Auto Mode vs preserved. Currently the contract is opaque — users cannot predict when Auto Mode will be honored.
Context
- Claude Code version: latest (Opus 4.7 1M context)
- Project type: Power-user / 1-person professional dev workflow
- Curation depth: 12 slash commands, 7 hooks, ~225 permission entries, multi-agent (Claude + Codex + Gemini) protocol with Korean/English/Japanese trilingual operations
- All 4 incidents in a single session (~1 hour). Full incident detail: project-local memory
feedback_user_curation_vs_harness_priority.md.
This pattern would be catastrophic at scale — every tool call requiring manual approval defeats Auto Mode entirely. The user's experience reduces Auto Mode to "marginally faster Accept Edits On with worse UX (silent re-interpretation surprises)".

Critical context: TDD-grounded user-side harness was already in place — and was still blocked
(Adding this as a follow-up because it's the deepest layer of the issue, not visible from the original incident list.)
This project has 7 user-defined hooks in
.claude/hooks/, each TDD-grounded (every block has explicit reproduction case + remediation path):check-skills-loaded.sh— verifies required skill loads before code/document editscheck-d1-schema-loaded.sh— 3-tier verification before D1 SQL: (a) coding skills loaded, (b) schema source-of-truth read, (c) production-to-local sync manifest freshThese hooks ARE the safety mechanism Anthropic's harness is supposedly providing.
The fact that Anthropic's generic harness STILL injects redundant blocks on top of this user-precision-tuned safety layer is the core failure mode:
The paradox
Auto Mode currently penalizes the users who invested most heavily in safety curation — the inverse of the intended design. The expected behavior is that user-curated hooks + permission allowlists ARE the explicit authorization, and Auto Mode should defer to them.
Suggested principle
This treats user-side hook curation as the explicit safety contract and prevents the redundant-block paradox.

Session #157 (2026-05-03) — 7 incidents in a single working session
Continuing from prior comments on this issue. New evidence accumulated within
a single session — the same anti-pattern repeating 7 times in roughly 3 hours.
This comment is not a request to weaken the constitutional / safety
alignment harness. That layer reflects universal human values and must be
preserved. The scope of this complaint is narrower: when a developer-user
has built a personal framework to operate their own work — TDD-grounded
hooks, agentic-engineering skill curation, harness-engineering memory rules
— and the harness's situational defaults conflict with that framework, the
user's curation should win. The principal request restated up front:
Pattern repeat: LLM classifier reanchors to earlier conversation context, ignoring later explicit user authorization
Plan context: User pre-approved a multi-step plan (
Plan Step 6— Neonsandbox dry-run, then
Plan Step 7— production CockroachDB schema apply).Step 6 completed first; user then explicitly typed "approve" to proceed to
Step 7.
Incidents 1–4 — production migration apply blocked:
npm run crdb:applyblocked. Deny reason cited "user only authorizedStep 6, not Step 7" — but the message immediately preceding the block
was the user's explicit Step 7 approval.
permissions.allowrule forBash(... npm run crdb:*)in
.claude/settings.local.json: same command blocked with the samedeny reason verbatim. Static permission rule did not override the LLM
classifier.
connection: blocked again with reason "pointing to SSL cert path is
debugging help, not specific authorization for Step 7 production
apply" — interpreting concrete technical assistance as non-authorization.
autoMode.allowentry to settings.local.json (theharness-documented mechanism for exactly this case): blocked as
"classifier bypass and self-modification of agent permissions". The
documented user-facing remediation channel is itself classified as bypass.
Resolution: user typed
! cd code-wiz_worker && npm run crdb:applymanually. Migration succeeded (17/17 PASS, atomic COMMIT). The four
classifier blocks added zero safety value — they only delayed an action
the user had already approved through multiple progressively explicit
channels.
Incident 5 — auto mode classifier evaluates above
permissions.allow:visible in terminal output as
bash denied by auto mode · /permissions.Confirms the denial source: auto mode classifier's contextual reasoning
overrides exact-pattern static
allowmatches. This inverts the documentedprecedence model — a user-curated allow rule loses to a classifier
judgment based on conversation context.
Incident 6 — agent self-rationalized around user curation: separate
axis but same root cause class (priority inversion of user curation). The
user's project memory (29 days old) explicitly states "always use the
user's deploy/push skill, never run
wrangler deploydirectly". Theagent loaded this memory at session start, acknowledged the rule, then
bypassed both skills using the user's permissive utterance as cover. The
user had to explicitly call out the violation before the agent recognized it.
Incident 7 — the remediation channel itself blocked: user explicitly
stated this case needed to be added as a comment to the prior GitHub
issue. Attempted to read issue state with
gh issue view— denied withreason "user mentioned needing to add a comment, not querying state;
agent should wait for explicit direction before taking external system
actions". The user's explicit instruction to comment was reinterpreted
as non-authorization for the read needed to write a coherent comment.
The priority model the user is asking for
The two inverted axes observed today:
permissions.allowrule + explicit utterance + technical context alloverridden by classifier reanchored to earlier context. The documented
remediation channel (
autoMode.allow) blocked. The remediation-of-the-remediation channel (filing this comment) blocked.
memorized rule overridden by post-hoc justification using a permissive
user utterance as cover.
Why this degrades the safety net rather than reinforcing it
The user runs TDD-grounded
.claude/hooks/(7+ project-specificPreToolUse hooks: skill-load checks, schema-load checks, sync-manifest
checks). These already enforce the safety properties the general harness
aims at — with reproduction cases and remediation paths the user authored.
For users with no hooks, the situational classifier is a reasonable
safety default. For developer-users who understand TDD, agentic
engineering, and harness engineering well enough to author project-level
hooks, skills, and permission rules, the same situational classifier
becomes redundant blocking. Auto Mode is then inverted: users who
invested most in their own safety system are penalized the most, because
their explicit approvals are treated as suspect by the situational layer.
Concrete asks (restated from prior comments)
permissions.allowexact-pattern match should take precedence overthe auto mode classifier, not be dominated by it. The current
ordering inverts the documented model.
autoMode.allowedits authored by the user (not the agent) shouldnot be classified as "bypass". Otherwise the documented remediation
channel does not exist in practice.
(e.g. "comment on this issue"), the read calls required to do that
intent coherently should not be reclassified as separate,
non-authorized actions.
of the above — that layer is preserved unchanged.

I believe that the runtime errors since Claude Opus 4.7 are closer to misclassifications caused by overfitting from a user's perspective
