fix(host-service): bound concurrent GitHub lookups in the PR runtime - #9486
With many projects open, the sidebar shows "GitHub unreachable", clears, and shows it again many times a day, while gh works (#8998). Every PR refresh spawned all of its gh calls at once. gh's 10 s timeout runs from spawn, so calls that answer in 0.4 s alone timed out waiting behind each other, and the unreachable gate read those timeouts as an outage. The recovery refresh then repeated the same burst.
What changed
Every GitHub lookup in the PR runtime takes one of 8 slots before it spawns gh. A queued lookup's timeout has not started yet.
The gate is checked before a lookup queues and again after it gets its slot. In a real outage the first lookups time out and trip it, and the rest fail fast without spawning gh.
The Octokit fallback gets the same 10 s deadline as gh, so a hung fallback releases its slot and trips the unreachable hold like a hung gh.
Explicit refreshes, such as a PR just created, go to the front of the queue.
Scope
The PR runtime only. The shared execGh and its other callers are unchanged.
The gate still treats a killed gh as unreachable. With the limit, a timeout means the call really hung.
Tradeoffs
The limit of 8 lookups, so at most 24 gh processes, spreads a full refresh of about 100 projects over tens of seconds instead of one burst. That is well inside the 5-minute refresh interval. A higher limit brings the contention back.
Blast Radius
Every GitHub lookup in the PR runtime now waits for a slot when 8 are running. A user-triggered refresh skips ahead of the background sweep.
Verification
New tests run a 96-project refresh against a fake gh in virtual time, with 4 processes at once, 400 ms per call, and a 10 s timeout. Before the fix, a healthy GitHub tripped the gate, and a hanging GitHub spawned 192 processes. After it, the gate stays open, and a hanging GitHub still trips it with fewer spawns than projects.
Another test hangs 8 Octokit fallbacks and checks that an explicit refresh still completes. The pull-request runtime suite passes (115 tests), and host-service typecheck and biome are clean.
With real gh on one machine, latency grew with concurrency (1 call: 0.4 s, 300 concurrent pulls?head= calls: up to 2.9 s). It did not reach the 10 s timeout there, so the reporter's timeouts on a busier machine are inferred from that trend.
Pull request updates now handle slow or unresponsive GitHub responses more reliably. Concurrent lookups are limited, and availability checks respond to successful and failed requests.
Timed-out requests no longer prevent workspace refreshes from completing. Repeated failures are detected before additional per-project processes are started.
GitHub command-line requests now use a 10-second default timeout, while explicitly configured timeouts are preserved.
…R sweep
A sweep over 96 projects spawns every gh lookup at once. With gh's
timeout running from spawn, calls that would each answer in 0.4 s wait
behind the others, get killed, and trip the unreachable hold (ParadevAI#8998).
A second test pins that a real outage still trips the gate, and that it
does so without one gh spawn per project.
The fleet sweep refreshed every project at once, and each project fanned
out a gh spawn per workspace and three per linked PR. gh's 10 s timeout
runs from spawn, so on a host with ~100 projects calls that answer in
0.4 s alone spent their budget waiting behind each other, were killed
with SIGTERM, and tripped the unreachable hold. The hold-recovery sweep
then fired the same burst again, so the sidebar warning flickered all day.
Every GitHub lookup now takes one of 8 slots before it spawns gh, so a
queued lookup's timeout has not started yet. The gate is checked after a
slot is granted: in a real outage the first lookups time out, trip the
hold, and the queued ones fail fast instead of each spawning a gh that
hangs. Explicit refreshes (probes) go to the front of the queue so a
user action does not wait behind a sweep.
FixesParadevAI#8998
GitHub pull-request lookups now have a concurrency limit of eight, and waiting probe lookups take priority. Octokit fallback requests use a configurable timeout that defaults to 10 seconds. Fleet tests cover healthy refreshes and timed-out calls across 96 projects.

execGh uses the exported GH_TIMEOUT_MS constant for its default timeout. The runtime manager accepts an Octokit timeout override, defaults it to GH_TIMEOUT_MS, and tracks active and queued lookups.

The runtime manager queues lookups when all eight slots are occupied, prioritizes probes, and releases each slot after the lookup. Octokit fallback requests use the configured timeout. Fleet tests cover healthy refreshes, a pending fallback, and timed-out calls.

…ctokit fallbacks
Eight sweep lookups whose gh fails over HTTP fall through to an Octokit
call that never settles. They hold every GitHub lookup slot, so a later
explicit refresh waits forever.
…s gh
Octokit is built with no request timeout, so a fallback request that
never settles held its lookup slot forever. Eight of them blocked every
later lookup, explicit refreshes included. Before the limiter a hung
request blocked only its own call.
The deadline rejects with ETIMEDOUT, which noteGitHubFailure already
classifies as unreachable, so a hung fallback trips the hold the same
way a hung gh does. The request is abandoned, not aborted: an
AbortSignal would have to go through five github-query helpers and
would not cover the token lookup in github(), and the hold stops new
requests from starting.
… slot
A sweep lookup that starts during a hold now fails at once instead of
waiting behind the running lookups only to fail after it gets a slot.
The check after the slot is granted stays, for a hold that opens while
the lookup waits. The early check throws before the slot count changes.

Prioritize an in-flight sync when an explicit refresh joins it.
During a large sweep, a workspace sync can wait behind ordinary lookups. An explicit refresh of that workspace waits for the sync before it can enqueue its priority probe. This can delay the PR creation response even after GitHub has created the PR. Promote the in-flight sync’s pending lookup work when an explicit refresh joins it, while retaining the bypass-cache rerun.

The healthy sweep can pass with more than eight lookups in flight. Hold several head lookups open and assert that a ninth cannot start until one of the first eight finishes. This tests the lookup limit without treating grouped detail requests as separate slots.
