fix(workspace-fs): emit from watchSingleFile only when the file changed - #9481
When fs.watch failed for a file, watchSingleFile polled and emitted update on every tick, so the document store reloaded the file every 2 s and could show a false external-change warning. It now keeps an ino:mtimeMs:size signature and emits only when it changes.
Left out: a same-size rewrite within one mtime tick is not reported. That only matters on coarse-mtime file systems.
How I tested it
Failing regression test that makes the file unreadable to force polling (skipped when run as root). It also checks that a real mtime change still emits. The bug was first found by checking every order of events in a small model, then reproduced against the real code.
packages/workspace-fs: bun run test gives 196 pass, 0 fail.
The unreadable-file trigger was checked on macOS only.

Summary by cubic
Fixes watchSingleFile emitting update on every poll when fs.watch cannot be installed, preventing repeated reloads and false external-change warnings. Polling now emits only when the inode, mtime, ctime, or size changes — including permission changes and same-size rewrites that restore the mtime — while watch-driven events always emit, since a watch event is evidence of a change even when the signature is unchanged on coarse-timestamp filesystems. The polling regression tests skip Windows and root.

Bug Fixes
File watching now detects same-size rewrites even when the original modification time is restored, and detects permission changes during polling.
File updates are reported when a watch event occurs, even if the file’s detected signature is unchanged. Polling also handles files that cannot be watched directly.
When fs.watch cannot be installed (EACCES, EMFILE, ENOSPC) the file is
polled, and every poll tick emitted update for an unchanged file. The
document store then reloaded every 2s and could report a false external
change. settle() now keeps an ino:mtime:size signature and emits only when
it differs, the same dedupe the directory branch already does.
watchSingleFile now compares file signatures during settling and records filesystem watch events. It emits file updates or creations when the signature changes or a watch event is pending. Tests cover same-size rewrites and polling behavior.

Track file changes and verify polling packages/workspace-fs/src/watch-file.ts, packages/workspace-fs/src/watch-file.test.ts
watchSingleFile records signatures on attachment and during settling. It clears the signature when the path is a directory or missing. It emits file updates or creations when the signature changes or a watch event is pending. Tests cover same-size rewrites, watch events, unchanged polling, timestamp changes, and permission changes.
A watcher error on an unchanged file may cause one extra reload. This is deliberate, and a repeating loop was not established, so the risk is bounded.

Force the polling path in this test.
On Linux, a non-root process with CAP_DAC_OVERRIDE can watch the unreadable file. The test can pass without polling, even if unchanged polls emit updates. Force fs.watch to fail and verify that polling occurs before checking for silence.

Update the test around fs.chmod so it reliably exercises polling: make fs.watch

A chmod, or a same-size rewrite that restores the mtime, left the
ino:mtime:size signature unchanged, so the dedupe dropped the update
that main still emitted. ctime moves on every content or metadata
change and userland cannot set it back.
A watch event is evidence of a change even when the file signature
did not move. On coarse-timestamp filesystems (HFS+, 1 s mtime/ctime)
two same-size writes inside one tick produce identical signatures, so
gating every settle on the signature dropped the second write's update.
The reload spam in ParadevAI#9462 came only from the poll tick, so only a
poll-driven settle dedupes now; a watch- or error-driven settle emits
as it did before.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.
Do not classify watcher errors as file changes.
If an FSWatcher emits an error while the file is unchanged, onWatchEvent() sets watchEventPending. The next settle then emits update despite the unchanged signature. If replacement watchers also fail asynchronously, each retry can produce another false update. Schedule a settle on errors without setting watchEventPending; reserve that flag for file-watch callbacks. (raw.githubusercontent.com)
