fix: macOS arm64 libchdb load failure and recoverable agent tool failures as error issues - #1222
Conversation
v26.1.0's macos-arm64 libchdb.so has its LC_SYMTAB string pool at file
offset 0x13148434, which is not 8-byte aligned. Newer dyld refuses it
("mis-aligned LINKEDIT string pool"), so `maple start` fails before it
opens chDB. This was ~205k maple-cli ServerError events on 2026-10-03,
across 0.0.11 (Homebrew), 0.0.18 and 0.0.22.
v26.7.3's string pool is aligned on both macOS arches and stays aligned
after the release workflow's ad-hoc codesign. Smoke-tested from source
on the new lib: fresh bootstrap, reopen of an existing store, OTLP
ingest, services/errors queries, first checkpoint, clean stop.
Existing local stores were written by v26.1.0, so the store-version gate
asks for `maple start --reset` on first start after updating.
…or issues The agent engine closes every failed execute_tool span as Error, and error_events_mv turns each into an error-issue occurrence. Most were the model writing bad SQL and correcting itself (failure_handling = returned-to-model): 437 ToolCallFailed occurrences on maple-chat since 2026-10-01. The chat turn runtime now wraps the tracer so a returned tool failure exports as Ok with error.type set. error.type keeps it counted as a failed call in Agent Sessions and the tool overview (the gateway stamps maple_ai.error from it). Propagated failures keep their Error span. Also name the traces table in run_sql's description: otel_spans, otel_traces and spans were the most common guessed table names.

Preserve existing stores before changing the default.
An installation with a v26.1.0 store must use maple start --reset after this upgrade, and that reset wipes local data. Provide a non-destructive migration or recovery path before making v26.7.3 the default.
The PR objective states that existing stores require reset and that reset wipes local data.

LIBCHDB_VERSION can be overridden from the environment, but the sha256 was selected by platform asset only, so any other version was checked against the default release's hash and always failed. Key the case on version/asset and reject an unpinned pair before downloading.
Maple review🟢 Confidence 4/5 · likely safe to merge Bumps the pinned libchdb to v26.7.3 in the local-binary build and keys its checksum table on version/asset, and stops agent tool failures the model recovers from being exported as Error spans. Only the build-script hunk is new since the previous review and it holds up.

AlertDeliveryErrortimeouts: delivery already runs through a leased, retrying queue, 5 at a time with a 15s cap, so a slow endpoint doesn't hold up other deliveries. The timeouts were Hazel's endpoint failing in two bursts.WarehouseQueryErrortimeouts: they hit one org's own ClickHouse, which didn't respond within the client timeout.
